Article Synopsis
  • AI chatbots, particularly GPT-4 and Grok, were evaluated for their responses to clinical questions about implant-based breast reconstruction guidelines set by the Japan Society of Plastic and Reconstructive Surgery.
  • GPT-4 showed significantly better performance than Grok in accuracy, informativeness, and readability, according to assessments from plastic surgery specialists and fellows.
  • The findings indicate that while GPT-4 may help interpret clinical guidelines, there is still a risk of misinformation, underscoring the need for further research on AI's role in breast reconstruction surgery.

Article Synopsis
  • * This study involved 90 ESRD patients on hemodialysis, divided into low- and high-FGF21 groups, and tracked mortality and cardiovascular events over roughly 64 months.
  • * Results showed that high FGF21 levels correlated with a significantly higher mortality rate (28.3% vs. 9.1%), but did not affect the rate of cardiovascular events, indicating that FGF21 could be a useful marker for predicting mortality in ESRD patients.

Dose Timing of an Angiotensin II Receptor Blocker/Calcium Channel Blocker Combination in Hypertensive Patients With Paroxysmal Atrial Fibrillation.

J Clin Hypertens (Greenwich)

October 2016

Division of Cardiovascular Medicine, Department of Medicine and Department of Sleep and Circadian Cardiology, Jichi Medical University School of Medicine, Shimotsuke, Tochigi, Japan.

It has long been thought that there is a close association between hypertension and atrial fibrillation (AF). However, the efficacy of an angiotensin II receptor blocker for the prevention of organ damage in hypertensive individuals with AF is still controversial. The present study was a multicentered, prospective, randomized, open-label clinical trial investigating the differences in the effect of treatment with telmisartan/amlodipine combination tablets on blood pressure (BP) levels and BP variability between morning and bedtime administration in hypertensive patients with paroxysmal AF, using ambulatory BP monitoring (ABPM) and home BP.

Obstructive sleep apnea (OSA) is common in patients with renal disease, and an association between OSA and proteinuria has been proposed. However, the effect on proteinuria of OSA treatment with continuous positive airway pressure (CPAP) is unknown. We experienced a case of severe OSA, where proteinuria was clearly improved after CPAP initiation without any changes of medication or body weight.
