Idiopathic infantile hypercalcemia (IIH) is characterized by hypercalcemia, nephrocalcinosis, vomiting, dehydration, and failure to thrive. It is caused by the presence of biallelic loss-of-function variants in the locus. Although hypercalcemia has been linked to the consumption of vitamin D-fortified milk, no reports have documented its role in triggering IIH in patients with variants.

Background: Chronic kidney disease (CKD) causes progressive and irreversible damage to the kidneys. Renal biopsies are essential for diagnosing the etiology and prognosis of CKD, while accurate quantification of tubulo-interstitial injuries from whole slide images (WSIs) of renal biopsy specimens is challenging with visual inspection alone.

Methods: We develop a deep learning-based method named DLRS to quantify interstitial fibrosis and inflammatory cell infiltration as tubulo-interstitial injury scores, from WSIs of renal biopsy specimens.

Carbohydrate restriction is not typically recommended for children with type 1 diabetes mellitus (T1DM) because of concerns regarding growth retardation, ketoacidosis, severe hypoglycemia, and dyslipidemia. There is no consensus regarding the effects of carbohydrate restriction on the growth of children with T1DM. However, some previously reported cases of T1DM exhibited growth retardation during carbohydrate restriction, whereas others showed no obvious impairment.

Article Synopsis
  • The study investigates how distal bypass surgery affects tissue blood pressure in direct (DRA) and indirect (IRA) revascularized angiosomes in patients with chronic limb-threatening ischemia (CLTI).
  • A total of 29 limbs from 27 CLTI patients underwent repeated skin perfusion pressure (SPP) measurements before and after surgery, showing improvements in tissue blood flow for both DRA and IRA.
  • The findings suggest that distal bypass surgery enhances tissue blood flow throughout the foot, not just in the connected angiosomes.

Background And Aim: We previously identified that ever-smoking and severe gastric atrophy in pepsinogen are risk factors for synchronous gastric cancers (SGCs). This study aimed to determine the association of alcohol drinking status or alcohol-related genetic polymorphism with SGCs and also stratify their risk.

Methods: This multi-center prospective cohort study included patients who underwent endoscopic submucosal dissection for the initial early gastric cancers at 22 institutions in Japan.

  • *A 68-year-old woman with multiple PDAAs underwent a two-stage hybrid therapy involving aorto-splenic bypass and resection of one aneurysm, resulting in spontaneous regression of the others.
  • *The case suggests that improved blood flow dynamics can lead to PDAA regression, raising questions about the necessity of additional endovascular therapy, though more research is needed to predict rupture risks.

We describe an autopsied case of systemic AA amyloidosis secondary to metastatic renal cell carcinoma presenting intractable diarrhea. Severe diarrhea was the major symptom for the diagnosis of AA amyloidosis. No renal symptoms which are common in AA amyloidosis secondary to renal cell carcinoma were shown because hemodialysis following bilateral nephrectomy had already been started 9 years before.

Background: Triple-negative breast cancer (TNBC) is characterized as highly immunogenic and lacks specific targeted therapies. Interleukin 17A (IL-17A) is a controversial cytokine and is known to have anti-tumor and pro-tumor roles depending on the tumor microenvironment. In addition, IL-17A has been recently implicated in the recruitments of neutrophil into tumor tissues.

  • The study investigates the link between lifestyle factors and synchronous gastric cancers (SGCs) in patients undergoing endoscopic submucosal dissection (ESD) for early gastric cancers (EGCs).
  • Out of 850 patients studied, 16.0% were found to have SGCs, with significant risk factors identified as current smoking and severe atrophic gastritis, particularly in those who had not eradicated Helicobacter pylori.
  • The findings highlight the importance of smoking and gastritis management in reducing the risk of SGCs for patients undergoing ESD for EGCs.

Article Synopsis
  • The study aimed to identify physical risk factors for osteochondritis dissecans (OCD) of the humeral capitellum in young baseball players aged 8-14, investigating how certain physical characteristics may contribute to the condition.
  • Conducted over a year, the research involved assessments of shoulder and hip range of motion, and a total of 3651 players were followed up, revealing that 71 (1.9%) developed OCD.
  • The results indicated that a higher Rohrer index and reduced hip internal rotation range of motion on the nondominant side were significant risk factors associated with the development of OCD among these young athletes.

Article Synopsis
  • - The study investigates how changes in soft-tissue structures affect shoulder function in baseball players, particularly focusing on the relationship between these changes and pathologic internal impingement.
  • - Involving 81 high-school baseball players, the research compares two groups: those with internal impingement and a control group, specifically analyzing humeral retroversion and ranges of motion in external and internal rotations.
  • - Findings reveal that the internal impingement group exhibited a significantly greater difference in soft-tissue-related external rotation compared to the control group, suggesting that soft-tissue changes are linked to internal impingement in throwing athletes.

Although hepatitis B surface antigen (HBsAg) removal is considered the goal of chronic hepatitis B treatment, it can rarely be achieved with nucleos(t)ide analogues (NAs). It has been reported that tenofovir disoproxil fumarate (TDF) is superior in reducing HBsAg compared with entecavir (ETV) in treatment-naïve patients; however, the effect of TDF in patients who have received NAs is still unclear. The aim of the present study was to evaluate the efficacy of switching from ETV to TDF in patients who were already receiving ETV.
