Ruptured distal anterior cerebral artery aneurysm presenting with acute subdural hematoma without subarachnoid hemorrhage.

We report a case of distal anterior cerebral artery (DACA) aneurysm presenting with subdural hematoma (SDH) without subarachnoid hemorrhage (SAH). A patient in his fifties presented with headache. Fluid-attenuated inversion recovery magnetic resonance imaging revealed SDH in the interhemispheric fissure and left frontotemporal region.

Multiple Small Foley Balloons for Large Cardiac or Vascular Lacerations.

Foley balloons have been used to secure emergency hemostasis for cardiac or vascular injuries since the 1960s. However, using a single large balloon may have drawbacks, such as insufficient hemostasis and blood flow impairment. We have encountered 3 major intraoperative laceration cases since 2011.

Trauma-Induced Thoracic Stent Graft Migration and Dissecting Aneurysm Rupture.

Thoracic endovascular aortic repair (TEVAR) is widespread. We report a case of graft migration and dissecting aneurysm rupture after trauma. An 82-year-old man with a history of TEVARs for chronic type B aortic dissection had sudden dyspnea and hemoptysis after falling from a stationary bicycle.

Developing a Prediction Model for Persistent Systolic Anterior Motion After Mitral Valve Repair.

Background: We hypothesized that the distance between the anterior leaflet tip and the interventricular septum is a novel predictor for systolic anterior motion (SAM) after mitral valve repair.

Methods: In this case-control study, we included 139 patients who underwent mitral valve repair for degenerative mitral regurgitation between November 2014 and August 2022. We conducted multivariable logistic regression analysis to investigate the impact of the predictors associated with persistent SAM.

Pleomorphic Carcinoma Arising at Previous Resection Margins of Ground-Glass Opacities.

Pleomorphic carcinoma of the lung is highly malignant and occurs mostly in the upper lobes of smokers; however, no reports exist of new occurrences from the resection margins of the previous ground-glass opacities. In our case, a 73-year-old man underwent an initial partial resection of the left upper lung for ground-glass opacities, with progressive thickening of the resection margin during the following 10 months. Radical resection was performed, resulting in the diagnosis of a newly arising pleomorphic carcinoma from previously resected margins.

Endocardial Anchoring Technique for Variant Supracardiac Total Anomalous Pulmonary Venous Return.

The endocardial anchoring technique is a novel modification of total anomalous pulmonary venous return repair that involves creation of an L-shaped flap of the pulmonary venous confluence, subsequently anchoring it to the endocardium. A wide and smooth pathway can be expected from the theoretical advantages of this technique, namely, a smooth inner surface of the anchored flap and traction force to extend the orifice of the connection. An application of this technique for a rare variant of supracardiac total anomalous pulmonary venous return suggests its potential to be an alternative to the conventional repair, especially in patients with a curved pulmonary venous confluence.

Long-term Survival in Stage IVb Thymic Carcinoma With Multidisciplinary Aggressive Treatment.

Thymic carcinoma, a rare aggressive tumor, necessitates multidisciplinary approaches for optimal prognosis. The role of surgical interventions in stage IVb thymic carcinoma, as classified by the TNM and Masaoka-Koga staging systems, remains controversial; although some patients present with resectable disease, others do not. We report a case with supraclavicular metastasis and sternal invasion.

Thoracoscopic Anatomic Segmentectomy Techniques for Rare Medial-Basal Segment (S7) Variants.

The medial-basal segment (S7) is the smallest of all the pulmonary segments and exhibits significant anatomic complexities. The procedures adopted to perform anatomic S7 segmentectomy differ according to branching pattern as the B7 may show 3 types. Although some studies have reported surgical procedures for thoracoscopic segmentectomy of the S7 and its anatomic variants, this type of segmentectomy remains challenging.

Modified Fixed Loop-in-Loop Technique in Mitral Valve Repair for Children.

For adults, the standard procedure for mitral valve repair of Carpentier classification type II mitral regurgitation is reconstruction with artificial chordae. In children, placement of artificial chordae of precise length between the papillary muscle and prolapsed mitral leaflet in the restricted mitral subvalvular space is technically difficult. We successfully performed mitral valve repair in 3 pediatric patients using a modified fixed loop-in-loop technique.

Successful Treatment of Multiple Systemic Artery-to-Pulmonary Artery Fistulas.

Systemic artery-to-pulmonary artery fistula (SAPAF) is an uncommon abnormal vascular connection between systemic and pulmonary arteries. SAPAF with 3 or more inflow vessels has rarely been reported. The definitive diagnosis is made by selective arterial angiography, but 3-dimensional computed tomography angiography is useful for delineating abnormal vessels.

Midterm Results of Fenestrated Frozen Elephant Trunk Technique for Acute Type A Aortic Dissection.

Background: There is limited experience and knowledge of the use of the fenestrated frozen elephant trunk (FET) technique in acute type A aortic dissection (ATAAD). This study's aims were to assess the clinical outcomes of the fenestrated FET technique for ATAAD and to identify its best practices and pitfalls.

Methods: This study included 101 patients who underwent emergency surgical aortic repair for ATAAD at our hospital between October 2018 and April 2023.

Appendage Resection and Pulmonary Vein Isolation on Minimally Invasive Coronary Artery Bypass Graft.

Minimally invasive coronary artery bypass grafting is becoming standardized; however, its small incision may result in a limited field of view. Challenges arise in performing left atrial appendage resection and pulmonary vein isolation from the same incision, whereas safety and feasibility are not well documented. Our report demonstrates safe achievement of left atrial appendage resection and pulmonary vein isolation from the same minimally invasive coronary artery bypass grafting wound site with a good surgical field of view.

Leaflet Perforation Using Cor-Knot in Minimally Invasive Aortic Valve Replacement.

A 38-year-old woman underwent minimally invasive aortic valve replacement with a 21-mm Inspiris aortic valve and Cor-Knot for type 0 bicuspid valve and severe aortic stenosis. Postoperative transthoracic echocardiography was uneventful. Four months later, she experienced shortness of breath.

Extralobar Pulmonary Sequestration Diagnosed as a Mediastinal Tumor and Resected by Robotic Surgery.

A 55-year-old woman with no significant medical history or symptoms was incidentally diagnosed with a mediastinal tumor during a routine health checkup. The intraoperative findings of robot-assisted thoracoscopic surgery led to the diagnosis of extralobar pulmonary sequestration (EPS), with multiple vessels connecting to the pulmonary artery and superior pulmonary vein. EPS, an extremely rare condition, is typically supplied with blood from the aorta.

Incidence of Underreferral to Multidisciplinary Evaluation in Severe Primary Mitral Regurgitation.

Early development of the calcified exoskeleton of the polyplacophoran mollusk, with insight into the evolutionary history of shell plates and spicules.

Recent molecular phylogenetic studies have raised two questions about the evolutionary history of the calcified exoskeleton of mollusks. The first question concerns the homology of the two types of skeleton: whether spicules and shell plates share an evolutionary origin. The second question is the homology of the shell plates between chitons and other mollusks, including gastropods and bivalves.

Neonatal acute liver failure cases with echovirus 11 infections, Japan, August to November 2024.

In 2022-23, several European countries reported paediatric acute liver failure (ALF) with enterovirus infection. In August-November 2024, three neonatal cases of ALF with echovirus 11 (E11) were reported in Tokyo, Japan. All neonates developed irreversible multiple-organ failure and died.

Room-temperature spin-conserved electron transport to semiconductor quantum dots using a superlattice barrier.

To realize the optical transfer of electron spin information, developing a semiconductor layer for efficient transport of spin-polarized electrons to the active layers is necessary. In this study, electron spin transport from a GaAs/AlGaAs superlattice (SL) barrier to InGaAs quantum dots (QDs) is investigated at room temperature through a combination of time-resolved photoluminescence and rate equation analysis, separating the two transport processes from the GaAs layer around the QDs and SL barrier. The electron transport time in the SL increases for a thicker quantum well (QW) of SL due to the weaker wavefunction overlap between adjacent QWs.

Electrochemical oxidative dearomatization of electron-deficient phenols using Br/Br catalysis.

An electrochemical method for the oxidative dearomatization of electron-deficient phenols by employing tetrabutylammonium bromide as a mediator under aqueous biphasic conditions is reported. This approach represents a safer alternative to the use of stoichiometric chemical oxidants and enables oxidative dearomative spirolactonization and spiroetherification reactions. Compared to previous approaches based on direct electrolysis, this strategy expands the substrate scope to electron-deficient phenols.

Jamming transition and normal modes of polydispersed soft particle packing.

The jamming transition of soft particles characterized by narrow size distributions has been well studied by physicists. However, polydispersed systems are more relevant to engineering, and the influence of polydispersity on jamming phenomena is still unexplored. Here, we numerically investigate jamming transitions of polydispersed soft particles in two dimensions.

Thoracoscopic Lobectomy for Right Upper Bronchial Atresia Combined With an Azygos Lobe of the Right Lower Lobe in an Infant Patient: A Case Report of a Rare Condition.

Bronchial atresia (BA) and azygos lobe (AL) are rare congenital pulmonary abnormalities in pediatric patients. We herein report an infantile case of BA combined with AL that was treated with thoracoscopic surgery. The patient was an 8-month-old boy who was preoperatively diagnosed with BA of the right upper lobe (RUL) combined with an AL using fetal magnetic resonance imaging and postnatal enhanced computed tomography.

Effectiveness of breast pumps in early postpartum breastfeeding for women after cesarean section: A systematic review.

Aim: We sought to assess the effectiveness of using a breast pump for nipple stimulation to promote breastfeeding in the early postpartum period after c-section in women with full-term infants.

Methods: Selection criteria were individual and cluster randomized control trials and quasi-randomized control trials that compared using a breast pump combined with standard care to promote breastfeeding in the early postpartum period after c-section with standard care. In this systematic review, guided by the Cochrane Handbook, we conducted comprehensive searches across databases such as Medline, Embase, CINAHL, Cochrane Library, and PsycINFO (Search: April 22, 2024).
