Background: Hyperparathyroidism (HPT) often persists after successful kidney transplantation (KTx). Although parathyroidectomy (PTx) is an effective treatment option for post-KTx HPT, it is associated with postoperative elevation of serum creatinine levels. We hypothesized that pre-PTx calcimimetic treatment could alleviate the post-PTx increase in serum creatinine levels.

Factors Predicting Spontaneous Regression in Other Iatrogenic Immunodeficiency-Associated Lymphoproliferative Diseases.

Background: Other iatrogenic immunodeficiency-associated lymphoproliferative diseases (OIIA-LPDs) occur in patients taking immunosuppressive drugs (ISDs) for autoimmune diseases, such as rheumatoid arthritis and have been suggested to be associated with Epstein-Barr virus (EBV) infection. Although some patients regress spontaneously upon discontinuation of ISDs, factors predicting spontaneous regression (SR) remain controversial. Therefore, we conducted a retrospective observational study of the clinical characteristics and factors associated with treatment response, prognosis, and SR in patients diagnosed with OIIA-LPD.

Massive RNA Editing in Ascetosporean Mitochondria.

Ascetosporeans are parasitic protists of invertebrates. A deep sequencing ana-lysis of species within the orders Mikrocytida, Paramyxida, and Haplosporida using metagenomic approaches revealed that their mitochondria were functionally reduced and their organellar genomes were lacking. Ascetosporeans belonging to the order Paradinida have not been sequenced, and the nature of their mitochondria remains unclear.

Exercise-induced Right-to-left Shunt in a Patient with Combined Pulmonary Fibrosis and Emphysema.

Combined pulmonary fibrosis and emphysema (CPFE) is characterized by emphysematous lesions in the upper lung field and pulmonary fibrosis in the lower lung field and is often associated with pulmonary hypertension and severe exercise-induced hypoxemia (EIH). We herein report a 62-year-old man with CPFE who presented with severe EIH despite relatively preserved lung volumes. Cardiopulmonary exercise testing suggested exercise-induced right-to-left shunt (EIS) through a patent foramen ovale (PFO).

Human schistosomiasis is a serious parasitic disease caused by infection with Schistosoma japonicum. In order to establish monitoring sites in the S. japonicum endemic area in the Obitsu River basin in Chiba Prefecture, Japan, we investigated the habitat of the intermediate snail host, Oncomelania hupensis nosophora using geographic information systems (GIS).

Associations of employment status, working time and job satisfaction with sleep duration and sleep quality among the 50+ population in Japan.

Few studies have addressed the relationship between sleep and employment in Japan. We use four waves of the Japan Study of Aging and Retirement (JSTAR) to address the relationship between weekday sleep duration, self-reported sleep quality and four self-reported indicators of sleep quality and employment status (model 1, full sample), working hours and job satisfaction (model 2, working sample) among individuals aged 50 and over (N=7,082). We apply mixed effects models for the linear outcome of sleep duration and cumulative link mixed models for the sleep quality variables, controlling for socio-demographic and health characteristics.

Risk Factors for Liver Injury and Their Association with Treatment in Hospitalized Patients with COVID-19.

Objective This study investigated the frequency and risk factors of acute liver injury (ALI) and the association between ALI and treatment in hospitalized patients with coronavirus disease 2019 (COVID-19). Methods This was a single-center retrospective study of 631 hospitalized patients ≥18 years old who were diagnosed with COVID-19 and received treatment between July 1, 2020, and January 31, 2024. Demographic and clinical data were extracted from the electronic medical records.

Recurrent Vomiting due to Severe Hyponatremic-Hypertensive Syndrome in an Elderly Patient.

Hyponatremic hypertensive syndrome (HHS) is a rare disorder, particularly in adults. A 76-year-old woman presented with vomiting and severe hyponatremia. The patient had been hospitalized four times over the past four years due to hyponatremia and recurrent vomiting.

Immunoglobulin A Nephropathy in a Kidney Transplant Recipient with Kabuki Syndrome: A Case Report.

Kabuki syndrome is a rare genetic disorder that causes multiple congenital anomalies, including characteristic facial features reminiscent of Kabuki syndrome. It is often associated with congenital anomalies of the kidneys and urinary tract as well as immune abnormalities. While various autoimmune diseases have been reported in patients with this syndrome, only one case of membranoproliferative glomerulonephritis has been documented.

A Severe Case of Rhabdomyolysis Requiring Renal Replacement Therapy Following COVID-19 mRNA Vaccination.

A 60-year-old man who developed rhabdomyolysis and severe acute kidney injury (AKI) after the fourth COVID-19 mRNA vaccination, necessitating renal replacement therapy (RRT). The patient presented to the hospital two days post-vaccination with muscle pain in both lower extremities and anuria. Diagnostic tests revealed elevated creatinine kinase (CK) levels of 160,000 IU/L and serum creatinine levels of 6.

Rituximab as an Effective Treatment for New-onset Evans Syndrome and Systemic Lupus Erythematosus with Lupus Nephritis.

Evans syndrome (ES) is characterized by the development of autoimmune hemolytic anemia and immune thrombocytopenic purpura and is often linked to autoimmune diseases, such as systemic lupus erythematosus (SLE). Standard treatment includes prednisolone and intravenous immunoglobulin; however, relapse commonly occurs when prednisolone is tapered or stopped. Rituximab is increasingly used for refractory ES with SLE, although its efficacy in new-onset cases remains unclear.

Successful Resolution of Suspected Red Yeast Rice-induced Nephropathy.

Recently, several Japanese patients have developed interstitial nephritis or acute tubular injury/necrosis, both accompanied by Fanconi syndrome, after consuming a specific red yeast rice supplement. In most cases, the renal function did not fully recover even after discontinuation of supplements and glucocorticoid treatment. A 52-year-old woman developed interstitial nephritis and Fanconi syndrome after receiving supplements.

A Case of Multiple Myeloma in a Patient with Birt-Hogg-Dube Syndrome.

Birt-Hogg-Dubé syndrome (BHDS) is an autosomal dominant disease caused by germline folliculin (FLCN) mutations and it is characterized by skin folliculomas, pulmonary cysts, and renal cell carcinomas (RCC). We herein report the first case of a female patient with BHDS who was diagnosed with multiple myeloma. Daratumumab-based treatment was effective, and the patient remained responsive for over three years.

Delayed Methotrexate Elimination Following High-dose Methotrexate Prophylaxis in High-risk Diffuse Large B-cell Lymphoma.

Objective High-dose methotrexate (HD-MTX) is widely used as central nervous system (CNS) prophylaxis in patients with diffuse large B-cell lymphoma (DLBCL) who are at a high risk of CNS relapse. Ensuring safe prophylactic administration with minimal adverse events is a key concern; however, few studies have detailed the safety profile of HD-MTX prophylaxis in patients with high-risk DLBCL. We analyzed the adverse events associated with HD-MTX in this population, focusing on delayed MTX elimination.

Effect of Biologics against Interleukin-5 Administered to Patients with Eosinophilic Gastroenteritis.

Biologics against interleukin-5 were administered to five patients with eosinophilic gastroenteritis (EGE) and bronchial asthma (BA). BA and abdominal symptoms as well as changes in steroid dose and the blood eosinophil count were examined. The man-to-woman ratio was 1:4.

A Case of Cutaneous Arteritis Complicated by Crohn's Disease.

Cutaneous arteritis (CA) is a rare cutaneous manifestation of Crohn's disease. A 50-year-old woman with a 15-year history of CA was admitted to our hospital with a fever, abdominal pain, and hematochezia. Based on these symptoms and increased C-reactive protein levels, systemic vasculitis was considered.

The lack of self-recovery features and mechanical toughness has fundamentally limited the practical applications of ionic single-network hydrogels. Due to strong ionic interactions with Zr cation crosslinkers, intramolecular crosslinking nodes are found to evolve via the folding of alginate segments, revolutionizing alginate hydrogels with substantial self-recovery capability and enhanced mechanical strength. Although being restricted by like-charge repulsion still, this new crosslinking mechanism triples the reachable tensile strain of alginate/polyacrylamide double-network hydrogels, and largely improves self-recovery efficiency.

Statistical strategies to analyze local control after radiotherapy.

Historically, local progression, an outcome of effectiveness in curative radiotherapy, has not always been statistically analyzed with the same method. This article aims to elucidate the validity and relative usefulness of statistical strategies to analyze local control. In an application example, data were analyzed from a phase III trial comparing different radiation schedules for glottic cancer.

Genetic Background of Selected Hyperuricemia Causing Gout with Pediatric Onset.
