Autoimmune metaplastic atrophic gastritis (AMAG) is a chronic immune-mediated form of gastritis characterized by damage to oxyntic cells, ultimately resulting in both iron deficiency with or without anemia and pernicious anemia. The current dogma is that AMAG is a disease of White Northern European women of advanced age. We, therefore, sought to examine the prevalence of AMAG in biopsies obtained from populations enriched for self-identified Hispanics for cross-comparison against data from previously reported populations enriched for self-identified White, non-Hispanic patients.

Article Synopsis
  • Isolated cardiac sarcoidosis can occasionally present with transient ischemic attacks (TIAs) or strokes as initial signs, emphasizing the need for doctors to consider it when patients have new neurological symptoms and cardiac disease indicators.
  • A case involving a 54-year-old woman shows how she experienced TIAs without initial heart dysfunction, but later developed heart issues (cardiomyopathy) and a left ventricular thrombus, linking her strokes to cardiac problems.
  • The patient was treated with anticoagulation and immunosuppressive therapies, leading to both the resolution of the thrombus and improvement in her heart function, underscoring the importance of investigating cardiac sarcoidosis as a potential cause of unexplained cerebrovascular incidents.

Renal cell carcinoma (RCC) with inferior vena cava (IVC) and right atrium (RA) tumor thrombus (TT) is a rare occurrence and its resection is surgical challenge. Management becomes even more difficult when the TT causes hepatic vein obstruction and leads to Budd-Chiari syndrome. We report a case of 68-year-old male with right RCC with IVC and RA TT with associated Budd-Chiari syndrome.

Renal transplantation remains the best treatment option of renal replacement for end-stagechronic kidney disease. Surgical advances in graft quality and implantation techniques have improved transplantation during the last two decades. This has reduced both urologic and vascular complications after implantation.

Background: Over the past 2 decades, emergency ultrasound has become essential to patient care, and is a mandated competency for emergency medicine residency graduation. However, the best evidence regarding emergency ultrasound education in residency training is not known. We performed a scoping review to determine the (1) characteristics and (2) outcomes of published structured training methods, (3) the quality of publications, and (4) the implications for research and training.

HIV screening in the emergency department (ED), including universal screening irrespective of risk assessments, has shown strong promise in past studies, identifying many new cases of HIV infection among those who lack access to traditional HIV testing services. Yet, over the years a consistent set of challenges and limitations have presented themselves in settings throughout the United States. We review considerations for evaluating and improving the success of ED-based HIV screening programs in the United States.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has rapidly spread throughout the world causing significant mortality in high risk patients with severe manifestations. To date, Remdesivir has been the only antiviral authorized by FDA as therapy for emergency use. One of the potential complications of this infection is cytokine storm, which optimal treatment remains unknown.

Congenital intrahepatic portosystemic venous shunts are rare vascular malformations which are incidentally discovered on imaging or once hepatic encephalopathy becomes clinically apparent. Surgical ligation and endovascular embolization are potential treatments.

En-Bloc Simultaneous Heart-Liver Transplantation in Adult Patients.

Ann Surg

December 2021

Department of Surgery, Division of Liver and Gastrointestinal Transplantation, Miami Transplant Institute, Miami, FL.

Introduction: Complexity of combined heart-liver transplantation has resulted in low adoption rates. We report a case series of adult patients receiving en-bloc heart-liver transplantation (HLTx), describe technical aspects, and discuss benefits of the technique.

Methods: Retrospective review of patients receiving en-bloc HLTx over 18 months, with clinical follow up to 1 year.

Spontaneous coronary artery dissection (SCAD) is characterized by tear of the inner layer in the coronary artery, creating a false lumen between the inner and central layer. Its infrequent incidence often leads to delay in diagnosis posing challenges in management. There are currently no guidelines for the treatment of this condition.

Quadriceps Tendon Autograft in Anterior Cruciate Ligament Reconstruction.

J Am Acad Orthop Surg

January 2020

From the Department of Orthopedic Surgery, Southern California Permanente Medical Group, Kaiser Permanente Los Angeles Medical Center, Los Angeles, CA (Dr. Mehran and Dr. Yang), and the Department of Orthopedic Surgery, University of Miami/Jackson Memorial Hospital Miami, FL (Dr. Damodar).

Quadriceps tendon (QT) autograft in anterior cruciate ligament reconstruction has received increased attention, as more studies compare clinical outcomes between QT, bone-patellar tendon-bone (BPTB), and hamstring tendon (HT) autografts. Biomechanically, QT has similar properties to BPTB and to HT. QT has shown equivalent stability, patient-reported outcome measures, strength and graft failure rates, and decreased donor site morbidity compared with BPTB and HT.

Shake It Off: A Randomized Pilot Study of the Effect of Whole Body Vibration on Pain in Healing Burn Wounds.

J Burn Care Res

April 2018

From the *DeWitt Daughtry Family Department of Surgery, University of Miami Miller School of Medicine, Florida; †Department of Occupational Therapy, Jackson Memorial Hospital. Miami, Florida; and ‡Department of Physical Therapy, Jackson Memorial Hospital. Miami, Florida.

Whole body vibration (WBV) has been shown to improve strength in extremities with healed burn wounds. We hypothesize that WBV reduces pain during rehabilitation compared to standard therapy alone. Patients with ≥1% TBSA burn to one or more extremities from October 2014 to December 2015 were randomized to vibration (VIBE) or control.

Continuous-flow left ventricular assist devices reduce short-term mortality and improve quality of life in patients with end-stage heart failure. Unfortunately, device-related complications remain common, with many patients experiencing adverse events within the first year. New literature suggests that rates of device-related thrombosis may be increasing since 2011, which is particularly troublesome given that this pathology can result in a disabling stroke, organ damage, and death.

The effect of transdermal scopolamine for the prevention of postoperative nausea and vomiting.

Front Pharmacol

June 2014

Department of Anesthesiology, The Ohio State University Werner Medical Center Columbus, OH, USA ; Department of Neurological Surgery, The Ohio State University Medical Center Columbus, OH, USA.

Postoperative nausea and vomiting (PONV) is one of the most common and undesirable complaints recorded in as many as 70-80% of high-risk surgical patients. The current prophylactic therapy recommendations for PONV management stated in the Society of Ambulatory Anesthesia (SAMBA) guidelines should start with monotherapy and patients at moderate to high risk, a combination of antiemetic medication should be considered. Consequently, if rescue medication is required, the antiemetic drug chosen should be from a different therapeutic class and administration mode than the drug used for prophylaxis.

Background And Purpose: Thickening of the LF is ascribed to buckling due to DSN. Uncertainty exists as to whether this can occur without DSN. Our primary hypothesis was that facet degenerative changes alone, independent of DSN, can thicken the LF.
