[Treatment approaches to flexion contractures of the knee].

Acta Orthop Traumatol Turc

September 2009

Istanbul Universitesi Istanbul Tip Fakultesi, Ortopedi ve Travmatoloji Anabilim Dali, 34093 Capa, Istanbul, Turkey.

The knee is the most affected joint in children with cerebral palsy. Flexion contracture of the knee is the cause of crouch gait pattern, instability in stance phase of gait, and difficulties during standing and sitting, and for daily living activities. It may also cause patella alta, degeneration of the patellofemoral joint, and stress fractures of the patella and tibial tubercle in young adults.

Objectives: We evaluated functional results of patients treated with open reduction and internal fixation with the parallel-plate technique for complex distal humerus fractures.

Methods: Twenty-one patients (14 males, 7 females; mean age 47 years; range 16 to 85) underwent open reduction with olecranon osteotomy and internal fixation with the parallel-plate technique for distal humerus fractures accompanied by highly intra-articular or metaphyseal comminution (n=10), intra-articular comminution and osteoporosis (n=7), and intra-articular and metaphyseal comminution with bone loss (n=4). According to the AO classification, there were 12 C3, six C2, and three C1 type fractures.

Objectives: The purpose of this retrospective analysis was to evaluate the management of chondral defects in knees with anterior cruciate ligament (ACL) disruption.

Methods: Twenty-seven patients (mean age 35 years; range, 20 to 48 years) were randomized to osteochondral autologous transplantation (OAT, n=14) or a microfracture (MF, n=13) procedure together with ACL reconstruction in the same session for symptomatic lesions of the articular cartilage in ACL-deficient knees. The mean follow-up period was 34 months (range 24 to 53 months) in the OAT group, and 49 months (range 24 to 97 months) in the MF group.

Cartilage symptoms in the knee may be encountered in various forms in orthopedic practice. The first stage from history to treatment is to establish the diagnosis; if the diagnosis is made as a cartilage lesion in the knee, priority must be given to current treatment approaches. The surgeon must provide information regarding etiology, defect thickness, lesion size, the containment of cartilage, localization, stability, meniscal integrity, anatomic alignment of the knee, previous assessments, radiological assessment (direct radiography, magnetic resonance imaging), and general, medical, and systemic conditions, and family history.

Objectives: In external fixation, the type and configuration of K-wires have a considerable effect on fixation stability. Indications for external fixation have recently increased in the treatment of various musculoskeletal pathologies in osteoporotic patients. This biomechanical study was designed to determine the effect of surgical washers used with olive wires on surface kinetics of the cortical bone.

Shoulder complaints are one of the leading reasons for a doctor's visit in athletes involved in overhead activities, and among them, dislocation and subluxation are very frequent clinical presentations. Although diagnosis and management of shoulder problems are more clear-cut in traumatic cases, atraumatic cases may be quite troublesome. In order to facilitate the interval between the presentation of complaints and institution of appropriate treatment, many issues should be appreciated including the anatomy, etiology, pathogenesis, clinical evaluation, and treatment of shoulder disorders.

Biomechanical studies have shown that the biceps tendon and the superior labrum have significant contributions to the glenohumeral stability. Since superior labrum anterior-posterior (SLAP) lesions pose diagnostic difficulties, they should be kept in mind when evaluating a painful shoulder. Although magnetic resonance arthrography is of great use in diagnostic studies, diagnostic arthroscopy is still the best method for these lesions.

Diagnosis and management of chronic dislocations of the glenohumeral joint remain challenging. These dislocations usually occur in debilitated patients with a poor bone quality and they often have a decreased rehabilitation potential. Treatment can only be successful with early recognition of the condition, careful patient selection, and implementation of appropriate surgical techniques.

Posterior instability of the shoulder remains a diagnostic and therapeutic challenge, with a growing incidence especially in athletes. Evaluation of the type of the instability, whether unidirectional or multidirectional, is very important with regard to treatment. Although many patients benefit from strengthening and balancing rotator cuff muscles and scapular stabilizers, surgical procedures may be required in those unresponsive to conservative treatment.

The main pathology in traumatic anterior instability of the shoulder is a Bankart lesion and capsuloligamentous laxity. Success in the treatment of glenohumeral instability mainly relies on an anatomical attachment of the fibrocartilaginous labrum and restoration of tension of the capsuloligamentous structures. Suture anchors are preferred for the fixation of soft tissues.

Anterior shoulder dislocation is the most common dislocation in the human body with a high recurrence rate and thus, has been one of the interesting subjects in orthopaedics and traumatology. There is still controversy about its evaluation and treatment. In particular, the type and duration of immobilization after reduction of the shoulder vary among centers.

A twenty-two-year-old male patient presented with complaints of muscle atrophy and shoulder pain. Clinical and radiological studies showed a ganglion cyst near the spinoglenoid notch and a diagnosis of compression of the inferior branch of the suprascapular nerve was made. Following open excision of the cyst by a posterior approach, the patient's complaints subsided.
