Background: Iron refractory iron deficiency anemia (IRIDA) is a rare autosomal recessive type of anemia characterized by unresponsiveness to oral iron therapy and partial response to parenteral iron therapy. In this article, we report the clinical presentation of four patients with IRIDA admitted to our clinic, including their laboratory values at admission and after oral and parenteral iron treatment, and the analysis of their mutation(s) in TMPRSS6 gene.

Case: Four patients from different families, aged between 3 and 14 years, two girls and two boys, two of whom were from consanguineous marriages, who were diagnosed with iron deficiency anemia in primary health care institutions and referred to our clinic because of inadequate response to oral iron treatment were included.

Background: Anaemia is common in mothers and infants in the first year postpartum, especially in sub-Saharan Africa. We evaluated whether treating anaemia in the second trimester of pregnancy with a single dose of intravenous iron, ferric carboxymaltose, compared with standard-of-care oral iron could alleviate anaemia in postpartum women and their infants.

Methods: REVAMP (ACTRN12618001268235), an open-label, individually randomised, controlled trial done across nine urban and five rural health centres in Malawi, recruited women if they were in the second trimester of singleton pregnancy, had a capillary haemoglobin concentration of less than 10·0 g/dL, and had a negative malaria rapid diagnostic test.

In this study, the sodium carboxymethylcellulose (CMC) and xanthan gum (XG) were used to prepare the CXM-Fe hydrogels (CMC: 20 mg/mL, XG: 10 mg/mL) with the addition of Mytilus edulis protein hydrolysate‑iron (MEPH-Fe) complexes. The incorporation of MEPH-Fe complexes formed a denser network structure and the CXM-Fe hydrogels had better pH stability as well as gastrointestinal retention ability. Compared with ferrous sulfate and MEPH-Fe complexes, the CXM-Fe hydrogels at moderate doses (Fe:2 mg/kg) showed impressive recovery effects on iron deficiency anemia (IDA) mice in terms of hematological indices, organ coefficients and iron content, antioxidant capacity, and remarkedly attenuated the infiltration of inflammatory cells as well as the levels of inflammatory factors in iron deficiency-induced colonic inflammation.

While intravenous (IV) iron repletion is an effective tool to treat anemia and improve outcomes in chronic kidney disease (CKD), guideline laboratory definitions of iron deficiency differ, resulting in variability in screening and repletion strategies. This study sought to describe current practices surrounding identification and treatment of iron deficiency in CKD and then implement a pharmacist-led protocol to optimize care at a tertiary medical center. This single center, retrospective, pre- and post-protocol implementation study of adults with CKD admitted to the inpatient setting first analyzed historic practices for iron deficiency screening and treatment, followed by deployment of a pharmacist-driven protocol for iron deficiency screening and treatment.
