[The evaluation of efficiency of new methodology of personified registration and distribution of workload of pathoanatomists].

The original methodology developed by authors for personalized registration and distribution of workload of pathologists is implemented into a number of departments of pathological-anatomic service of the Irkutsk Oblast, including pathological-anatomic department of the Oblast State Budget Institution "The Irkutsk Municipal Clinical Hospital № 1". In order to evaluate efficiency of new methodology comparative analysis of workload volumes performed by pathologists with calculated standard workload was carried out for two periods i.e before application of new methodology and after its implementation.

[The analysis of medical personnel potential of stomatological service of the Subject of the Russian Federation considering its private sector].

The approaches to analysis of medical personnel of stomatological service continue to be based on principles formed in relation to state health care system that makes it difficult to objectively assess situation due to increasing proportion of specialists employed in its private sector. The study, targeted to comprehensive comparative analysis of stomatological medical personnel of state and private medical organizations in the Irkutsk Oblast, for the first time made it possible to assess provision of population of large region and its municipalities with stomatologists, considering specialists employed in private medical organizations, in dynamics and in context of separate specialties. The decrease in provision of population with stomatologists in state medical organizations by 12.

[The analysis of demand in pathoanatomists applying staff standards].

The objective analysis of state of medical personnel, along with assessment of real need for specialists, is the basis of improving activities of any health care service. In relation to pathologists, there is unique opportunity to perform similar analysis, based on application of current corresponding staff standards that consider volume of workload of physicians in order to determine required number of positions. The implementation of corresponding original methodology permitted to establish that the actual number of staff positions of pathologists in 2022 amounted up to average 40.

[The financial activity of district hospitals of the Irkutsk general-governorship in the end of XIX century].

The article presents results of the study of archive sources and reference publications. The unknown facts of subsidization of district hospitals of the Irkutsk general-governorship, items and amount of current and supernumerary expenses and sources of financial allocations are revealed. The task of increasing staffing of physicians in primary health care level is resolving by accreditation of medical workers. The sociological assessment of motives of employment and professional adaptation of graduates of medical university who started to work as district therapists and pediatricians after undergoing primary accreditation was carried out. [MEDICAL AND SANITATION PROVISION FOR THE REFUGEES IN THE IRKUTSK PROVINCE IN 1914-1916].

On the basis of the archival documents, the article studies and provides information on the creation of medical and sanitary bodies, public organizations to provide the refugees of the Irkutsk province with medical care in 1914-1916, to carry out comprehensive anti-epidemic measures to combat the emerging epidemics of diseases characteristic of wartime. The study shows the role of medical and sanitary bodies, committees, commissions to ensure the epidemiological well-being of the refugees. The dynamics of an increase in the number of beds, the number of hospitals, the opening of infectious barracks, an increase in financial allocations for the provision of medical assistance to refugees were noted.

The history of becoming of syphilitic hospitals at the North-Eastern outlying districts of Eastern Siberia in XIX century.

In the XIX century, process of joining of the North-East lands to the Russian empire was completed. The syphilis that spread in XVIII-XIX centuries stroke indigenous population of the Eastern Siberia and out-migrants from other territories. The article attempts to highlight little-studied aspects of organization of syphilitic hospitals and medical care of newly come population and non-Russian communities in northern Okrugs of Yakutsk, Kamchatka and Primorye areas of Eastern Siberia.

[The historical case of discussion on sanitary anti-epidemic activities during period of revolutionary events of 1917 in Irkutsk].

The article presents analysis of historical case of discussion about organization of anti-epidemic measures needed because of one-time arrival of large number of wounded and killed in consequence of counter-revolutionary mutiny in Irkutsk during defense of the White House country-house in December 1917. The problem of burial of fallen soldiers and Red Army men was determined during extraordinary session of the City Duma of Irkutsk with further publication of open letter to the Irkutsk Presidium of the Soviets. The well-reasoned positions expressed by physicians and engineers against burial of the killed in common grave on the banks of the Angara River received no support from the revolutionary government.

[The evaluation of efficiency of accreditation of graduates of medical universities as a measure supporting increase of numbers of physicians in primary health care section].

One of the most important tasks in transition to primary accreditation of health care professionals is increasing manning of primary health care section. It is achievable at the expense of opportunity for graduates of medical universities to find employment as district physicians and pediatricians without residency training. The purpose of the study was to assess impact on increasing of number of physicians of primary health care section of their admission to professional activities after primary accreditation.

[The trends of spreading of tuberculosis combined with HIV-infection among population the Siberian Federal Okrug].

In The Russian Federation, the current epidemic situation of tuberculosis is characterized by stabilization and decreasing of morbidity with simultaneous increasing of number of cases of tuberculosis combined with HIV infection within the structure of newly diagnosed diseases and contingents of patients with tuberculosis. In the Siberian Federal Okrug, the indices of morbidity of tuberculosis combined with HIV infection exceed similar ones by almost twice that testifies significant input into burden of morbidity of tuberculosis in the subjects of the Siberian Federal District and their role in further dynamics of morbidity. The shift in morbidity of tuberculosis combined with HIV infection towards increasing of age both in patients with diagnosed HIV infection for the first time and in entire accumulated contingent of HIV patients.

[The sociological aspects of organization of operation of district psychiatrists in modern conditions].

The article presents results of sociological survey of district psychiatrists that revealed actual difficulties and shortcomings in organization of medical care for mental patients. The problem of personnel high workload, inopportuneness and inadequate completeness of necessary medical care of mental patients are explained by a number of difficulties in organization of functioning of psychoneurological organizations. They included not only shortage of personnel and unsatisfactory material technical support of mental institutions, but also incompleteness of normative legal support mental care organization.

[The effect of medical organizational factors on morbidity of mental and behavior disorders].

Currently, the characteristic of dynamics of morbidity of mental disorders of population of the Russian Federation is its increasing simultaneously with changing in structure of contingents in the direction of decreasing number of patients subjected to dispensary monitoring. The study of influence of medical organizational factors on morbidity of mental disorders demonstrated the role of provision with psychiatrists in dynamics of prevalence of mental disorders. The improvement of management of patients with mental disorders should be supported by appropriate resources and adaptation of mental service to actual conditions, in particular, solving the problem of manpower deficiency of psychiatrists.

[The evaluation of personnel progression in state medical organizations of the Irkutsk Oblast].

The medical personnel reallocation is the most important resource to ensure sustainable functioning and development of medical organizations and health care system as a whole. The purpose of the study was to assess reallocation of medical personnel of state medical organizations in the subject of the Russian Federation exemplified by the Irkutsk Oblast in 2015-2019. The study permitted to test approaches to application of methods for analysis of reallocation of medical personnel in respect of physicians.

[About experience of developing concept of personnel policy in health care at the level of the subject of the Russian Federation].

The personnel policy is one of priority areas of developing national health care. Its effective implementation requires evidence-based development of relevant program documents, the most important of which is the Concept of health care personnel policy that currently exists only in a number of regions of the Russian Federation. In this regard, the purpose of the study was to develop approaches to scientific justification and development of the Concept of health care personnel policy at the level of region of the Russian Federation, as exemplified in the Irkutsk oblast.

[The comparative evaluation of policy documents of the Russian Federation concerning personnel policy in health care].

Nowadays, in Russia number of problems is present in effective personnel policy in health care. The National Concept of personnel policy in health care (2002) expired in 2010 and no new one was developed. However, corresponding program documents were adopted at the level of subjects of the Russian Federation.

[The comparative analysis of training of paramedical personnel on the territory of the subjects of the Russian Federation of the Siberian Federal Okrug].

The comparative analysis of training of nursing staff in the territories of subjects of the Russian Federation, included in the Siberian Federal Okrug was implemented, based on data of the federal statistical monitoring of corresponding educational organizations in 2018. The analysis of calculated indices permitted to identify uncoordinated nature of provision of nursing staff and volumes of its training at the level of number of subjects of the RF on the territory of the Siberian Federal Okrug, and group them according to combination of level of the given indicators. The assessment was implemented concerning validity of manpower policy in the case of nursing staff in territories of mentioned regions.

[The sociological evaluation of commitment to treatment in re-hospitalized patients].

The article presents the results of sociological assessment of commitment to treatment of repeatedly hospitalized patients in hospitals of the Irkutsk Oblast and the Republic of Buryatia, that permitted to study their medical and social characteristics, the level of awareness of their own health, their diseases and methods of their treatment are presented. Sampling was preceded by the selection of respondents with the support of experts that reduced the impact on the study results of various factors, along with commitment to treatment, which determines the effectiveness of the treatment process (effective treatment methods for the relevant nosology, quality of medical care, etc.).

[On the issue of work time measurement in phthisiatricians rendering ambulatory polyclinic care to adult and children population].

The formation of the number of medical personnel, the establishment of labor standards, the rational distribution and use of personnel are the most significant components of the system of labor rationing in health care. Standard norms of working time for TB doctors are not developed. There is an uneven workload and a plan implementation indicator in different offices and cabinets.

[The New Approaches to Organization of Medical Supplies Support of Patients in Hospital in Modern Conditions].

The article considers the results of complex scientific analysis of issues concerning medications' support of patients in hospital in modern conditions. The economic and social efficiency of proposed organizational methodological approaches to development of management of medications' support is demonstrated.

[The Sociological Aspects of Development of Organization and Enhancement of Quality of Hospital Medical Care of Patients with Tuberculosis].

The specially developed questionnaire on studying opinions of physician-phthisiatrician concerning hospital medical care of patients with tuberculosis was applied to survey sampling of 58 physicians phthisiatricians. The questions of questionnaire related to analysis of efficiency of hospital treatment of patients with tuberculosis, satisfaction with consultative and diagnostic activities, support with pharmaceuticals, discovering measures permitting to increase commitment of patients to treatment in conditions of hospital.

[Prevalence and risk factors for nephrolithiasis among young rural residents].

Aim: To determine the prevalence of nephrolithiasis, assess the significance of known risk factors, and identify new risk factors for nephrolithiasis among young rural residents.

Materials And Methods: This cross-sectional study was conducted in 5 settlements of the Irkutsk rural district of the Irkutsk region and involved 408 inhabitants (93 men, 315 women) aged from 18 to 47 years. The selection of candidates for the study was randomized.

The Purpose: To study effect of controllable moderate hypothermia on system of hemostasis in newborns with hypoxemic ischemic encephalopathy.

Materials And Methods: The retrospective analysis was carried out concerning 38 medical records of newborns with acute hypoxia of fetus (group I), 12 medical records of newborns with acute hypoxia of fetus against the background of chronic hypoxia of fetus (group II), 20 healthy newborns (group III) and 20 healthy adults. The thromboelastography was implemented in three stages: at first, third and sixth days of life of newborns.
