Background And Aims: Outcome-based education (OBE) focuses on clearly defined learner goals, offering a structured framework to achieve competency. This study explores the perspectives of trainees and trainers in Obstetrics and Gynaecology (O&G) in Ireland regarding facilitators, barriers, and challenges to implementing OBE.

Methods: A national cross-sectional survey was distributed to O&G trainees and trainers in Ireland.

Pharmacological and non-pharmacological interventions for pain control in outpatient hysteroscopy: A systematic review.

Background: Outpatient hysteroscopy (OPH) is an important diagnostic and therapeutic intervention in gynaecology. However, the most common reason for failure is pain. Currently, there is no consensus regarding analgesia for OPH amongst the literature.

In vitro evaluation of bioabsorbable poly(lactic acid) (PLA) and poly-4-hydroxybutyrate (P4HB) warp-knitted spacer fabric scaffolds for osteogenic differentiation.

Bioabsorbable textile scaffolds are promising for bone tissue engineering applications. Their tuneable, porous, fibre based architecture resembles that of native extracellular matrix, and they can sustain tissue growth while being gradually absorbed in the body. In this work, immortalized mouse calvaria preosteoblast MC3T3-E1 cells were cultured in vitro on two warp-knitted bioabsorbable spacer fabric scaffolds made of poly(lactic acid) (PLA) and poly-4-hydroxybutyrate (P4HB), to investigate their osteogenic properties.

Acidogenic fermentation of Ulva in a fed-batch reactor system: tubular versus foliose biomass.

The present study proposes a biorefinery of the macroalgae Ulva, focusing on evaluating two different morphologies of the species (foliose and tubular) during acidogenic fermentation in fed-batch reactors. Stage 1 of the study evaluates lyophilised foliose and tubular Ulva, whilst Stage 2 analyses the impact of ulvan extraction on volatile fatty acids yield and changes in carbohydrate availability. Acetic, propionic, and butyric acids were produced from each substrate, with peak concentrations of total VFAs recorded at 2179.

Status of Sepsis Care in European Hospitals: Results from an International Cross-Sectional Survey.

Rationale: Early detection, standardized therapy, adequate infrastructure and strategies for quality improvement should constitute essential components of every hospital's sepsis plan.

Objectives: To investigate the extent to which recommendations from the sepsis guidelines are implemented and the availability of infrastructure for the care of patients with sepsis in acute hospitals.

Methods: A multidisciplinary cross-sectional questionnaire was used to investigate sepsis care in hospitals.

Decision formation in perceptual decision making involves sensory evidence accumulation instantiated by the temporal integration of an internal decision variable toward some decision criterion or threshold, as described by sequential sampling theoretical models. The decision variable can be represented in the form of experimentally observable neural activities. Hence, elucidating the appropriate theoretical model becomes crucial to understanding the mechanisms underlying perceptual decision formation.

Exploring the perceptions of the effect of the COVID-19 pandemic on the mental well-being and medical education of medical students in Northern Ireland, in addition to the perceived barriers to seeking support; a qualitative study.

Introduction: The COVID-19 pandemic had a negative effect on population mental health. Medical students may have been particularly affected, whom prevalence of mental health conditions was already high before the pandemic hit, due to the difficult and stressful academic programme. In Northern Ireland specifically, mental well-being levels are the lowest across the UK; however limited research exists examining the medical student cohort.

Patient safety indicators for virtual consultations in primary care: A systematic review protocol.

Background: Virtual consultations are being increasingly incorporated into routine primary care, as they offer better time and geographical flexibility for patients while also being cost-effective for both patients and service providers. At the same time, concerns have been raised about the extent to which virtual care is safe for patients. As of now, there is no validated methodology for evaluating the safety nuances and implications of virtual care.

Risk-Based Screening of Atrial Fibrillation in General Practice (R-BEAT): A randomised Cross-over Trial.

Background: The optimal approach to the diagnosis of atrial fibrillation in primary care is unclear.

Aim: To determine if external loop recorder (ELR) screening improves atrial fibrillation detection in community dwelling adults with a CHA2DS2-VASc score of greater than two.

Efficacy and Safety of Different Atropine Regimens for the Treatment of Myopia in Children: Three-Year Results of the MOSAIC Randomized Clinical Trial.

Importance: Additional data are required regarding atropine treatment regimens for control of myopia progression.

Objective: To investigate the efficacy and safety of different atropine regimens for myopia in children.

Design, Setting, And Participants: This was a secondary analysis of the 3-year results of the 24-Month Myopia Outcome Study of Atropine in Children (MOSAIC) trial, called the MOSAIC2 trial.

Paediatric renal tumours: an update on challenges and recent developments.

Paediatric renal tumours include a broad range of neoplasms which largely differ, but also overlap to a smaller extent, with adult kidney cancer. These include the embryonal tumour nephroblastoma, which accounts for the majority of cases of kidney cancer in the first decade of life and, despite boasting a cure in ~ 90% cases, still presents clinical challenges in a small proportion of cases. A variety of less common mesenchymal tumours, including the mostly indolent congenital mesoblastic nephroma, clear cell sarcoma of kidney which continues to be associated with poor outcomes for higher stage disease, and the typically lethal malignant rhabdoid tumour, form the bulk of the remaining presentations in the first decade.

Performance of Noninvasive Tests for Metabolic Dysfunction-Associated Steatohepatitis and Liver Fibrosis Resolution after Bariatric Surgery.

Background: Noninvasive tests (NITs) to monitor metabolic dysfunction-associated steatohepatitis (MASH) progression and response to interventions are needed because of the risks of liver biopsy. A monocytes-based diagnostic test using perilipin-2 (PLIN2) and Ras-related protein-14 (RAB14) predict the severity of MASH and fibrosis. Here we compared the performances of PLIN2 and RAB14 with cytokeratin-18 (CK18) assessed by Ella™ or M65 ELISA in predicting MASH and fibrosis resolution following bariatric surgery in a longitudinal and histologically characterized cohort of individuals with obesity.

Importance: Previous research suggests that a greater capacity of health care organizations to address patients' health-related social needs (HRSNs) is associated with lower physician burnout. However, individual physician-level engagement in addressing HRSNs has not been fully characterized, and its association with physician burnout remains understudied.

CO Cryo-sorption as a Surface-sensitive Spectroscopic Probe of the Active Site Density of Single-atom Catalysts.

Quantifying the number of active sites is a crucial aspect in the performance evaluation of single metal-atom electrocatalysts. A possible realization is using adsorbing gas molecules that selectively bind to the single-atom transition metal and then probing their surface density using spectroscopic tools. Herein, using in situ X-ray photoelectron (XPS) and near edge X-ray absorption fine structure (NEXAFS) spectroscopy, we detect adsorbed CO gas molecules on a FeNC oxygen reduction single atom catalyst.

Plant-based cheese analogs: structure, texture, and functionality.

Plant-based cheese analogs have been developed using plant-based ingredients to mimic the appearance, structure, and flavor of conventional cheeses. Due to the complex composition and structure of cheese, developing plant-based cheese analogs that completely replicate its physicochemical, structural, sensory, and nutritional features is a highly challenging endeavor. Therefore, the design of the structure of plant-based cheese analogs requires a critical evaluation of the functional features of the selected ingredients and the specialized combination of these ingredients to create a desired structure.

Introduction: Smooth pursuit eye movements may be affected by head impacts. The EyeGuide Focus system is a simple, portable, test of visual tracking. This study investigated the diagnostic accuracy of EyeGuide measurements for detection of concussion during elite Rugby matches.

Human Health Surveillance During Animal Disease Emergencies: Minnesota Department of Health Response to Highly Pathogenic Avian Influenza Outbreaks, 2015 and 2022-2023.

Objectives: Highly pathogenic avian influenza (HPAI) poses an occupational risk for poultry workers, responders, and others in contact with infected birds. The objective of this analysis was to describe HPAI surveillance methods and outcomes, and highlight the challenges, successes, and lessons learned during the Minnesota Department of Health's (MDH's) public health response to HPAI outbreaks in Minnesota poultry flocks in the years 2015 and 2022-2023.

Methods: During both outbreaks, MDH staff attempted to contact all potentially exposed people and conduct a standardized interview.

Identifying Components of an Enhanced Recovery Pathway for Primary Cleft Palate Repair: A Scoping Review.

Objective: This study aims to map the existing sources of evidence on perioperative care and recovery strategies for primary cleft palate repair, to identify elements that should be included in an enhanced recovery pathway (ERP), and to identify gaps in current knowledge.

Design: Scoping review.

Setting: ERPs are evidence-based, patient-centered, multimodal, perioperative care pathways designed to reduce surgical stress and improve postoperative outcomes and are increasingly being reported in the cleft lip and palate literature.

Prognostic Value of Serum TMAO Measurement in Patients with STEMI: A Systematic Literature Review.

Background: Gut microbiota-derived metabolite Trimethylamine-N-oxide (TMAO) is increasingly recognized as a potential novel prognostic biomarker for cardiovascular disease. Our research work aimed to investigate the potential utility of TMAO measurement in patients with STelevation Myocardial Infarction (STEMI).

Methods: We performed a systematic literature search in PubMed from inception to the 1st of February 2024 to identify all studies examining the association between plasma TMAO levels and disease complexity or clinical outcomes in STEMI patients.

Is mesh related morbidity the real thread in ventral rectopexy? Results of a retrospective international multicentre comparative analysis of biologic versus synthetic mesh.

Aim: Ventral mesh rectopexy (VMR) is an established surgical treatment for rectal prolapse and outlet obstruction. In contrast to continental Europe, in the UK and US the use of synthetic mesh has been abandoned in favour of biologic mesh, due to concerns regarding mesh related morbidity. The current study investigated if either material is superior, in terms of clinical recurrence and mesh related complications.
