Article Synopsis
  • Chiari malformation type I (CM-I) is common in patients with adolescent idiopathic scoliosis (AIS) and its necessity for decompression surgery in asymptomatic patients is unclear.
  • A study involving 40 AIS patients evaluated changes in cerebellar tonsil position using MRIs before and after posterior spinal fusion (PSF).
  • Results showed significant spinal curve corrections after PSF, but no notable change in cerebellar tonsil position was detected post-surgery, indicating that CM-I decompression may not be necessary for asymptomatic AIS patients.

Article Synopsis
  • - The study aimed to compare the effectiveness of platelet-rich plasma (PRP) versus standard corticosteroid injections in relieving pain and improving function in patients with rotator cuff tendinopathy, addressing previous conflicting findings and limitations in research.
  • - Conducted as a double-blind randomized controlled trial with 100 patients (50 receiving PRP and 50 receiving corticosteroids), the study assessed pain relief and functional improvement over a 12-month period using various outcome scores.
  • - Results showed that the PRP group had significantly greater improvements in pain scores and shoulder function compared to the corticosteroid group at the 12-month follow-up, indicating PRP may be more effective for this condition.

[Use of frozen osteochondral allograft for the treatment of cartilage defects of the knee. Results at seven-year follow-up].

Medicina (B Aires)

April 2024

Sección de Artroscopia y Prótesis de Rodilla, Instituto de Ortopedia y Traumatología, Carlos E. Ottolenghi, Hospital Italiano de Buenos Aires, Buenos Aires, Argentina.

Introduction: Knee osteochondral lesions represent a frequent pathology within young active patients. One possible indication for severe lesions or in case of impossibility of harvesting an autograft is the use of fresh frozen allograft. The objective of this study was to retrospectively analyze functional results and failure rate after osteochondral transplants using fresh frozen allografts.

Background: Whether the use of PRP as an adjuvant of rotator cuff repairs leads to improved tendon healing and better functional outcomes remains unclear in clinical evidence.

Purpose: The main purpose of this study was to assess whether the use of leukocyte-poor platelet-rich plasma (LP-PRP) as an adjuvant to arthroscopic rotator cuff repair (ARCR) decreases the rate of retears compared with a control group. The secondary objective was to analyze whether LP-PRP improves patient-reported outcomes.

Centro de Cadera "Sir John Charnley", Instituto de Ortopedia y Traumatologia Prof. Dr. Carlos E. Ottolenghi, Hospital Italiano de Buenos Aires, Capital federal C1198AAW, Buenos Aires, Argentina.

Background: The ExeterTM Universal cemented femoral component is widely used for total hip replacement surgery. Although there have been few reports of femoral component fracture, removal of a broken femoral stem can be a challenging procedure.

Case Summary: A 54-year-old man with a Dorr A femur sustained a refracture of a primary ExeterTM stem, two years after receiving a revision using a cement-within-cement technique (CWC) through an extended trochanteric osteotomy (ETO).

Determining the true availability of resources and understanding the level of training of surgeons involved in the treatment of patients with pelvic fractures and haemorrhagic shock is critical. In the herein study, the availability of technical, technological, and human resources for the care of this injury in Latin America region was analysed, and the preferences of orthopaedic trauma surgeons when performing interventions for the diagnosis and treatment of patients with pelvic trauma and associated haemorrhagic shock was described. A cross sectional web-based survey containing questions on knowledge, attitudes, and practices with respect to imaging resources, emergency pelvic stabilization methods, and interventions used for bleeding control was sent to 948 Latin America orthopaedic trauma surgeons treating pelvic fractures in the emergency department.

Introduction: Bisphosphonates are commonly prescribed to mitigate the effects of osteoporosis, a condition associated with an increased risk of fracture. However, despite their success, there has been growing concern regarding the long-term use of bisphosphonates because of the increased risk of atypical subtrochanteric fractures of the femur (AFFs). The objective of this study was to describe a cohort of patients with AFFs and evaluate the risk of evolving to non-union, the time of consolidation associated with the location of the fracture, the time of use of the drug, and the implant involved in its treatment.

Postoperative bone infection is a severe complication in the treatment of fractures. The management of this pathology is challenging, but recent advances have been made to achieve standardization that can help diagnosis and decision-making. However, we are unaware of studies validating these models in Latin America.

Background: Ankle fractures are commonly treated with non-surgical or surgical (preferably ORIF) approaches based on fracture characteristics. The postoperative care regimens vary widely, and the impact of very early mobilization on complications and reoperation rates in ankle fractures treated with ORIF remains unclear.

Methods: A retrospective analysis was conducted on ankle fractures treated between January 2014 and November 2020.

[Translated article] The use of an anatomical implant compared to a straight LCP decreases extraction in posterior humeral MIPO.

Rev Esp Cir Ortop Traumatol

June 2023

Servicio de Ortopedia y Traumatología, Hospital Italiano de Buenos Aires, Instituto de Ortopedia y Traumatología «Carlos E. Ottolenghi», Potosí, Buenos Aires, Argentina.

Article Synopsis
  • The study aimed to compare the need for hardware removal in patients who underwent posterior minimally invasive plate osteosynthesis (MIPO) with either a straight 4.5mm locking plate or a 3.5mm anatomically contoured plate for mid-distal humeral fractures.
  • Results showed that 18% of patients with the straight plate required hardware removal due to discomfort, while none in the anatomical plate group did, indicating a significant difference (P 0.009).
  • The conclusion drawn suggests that using the 4.5mm straight locking plate is associated with higher discomfort and a greater likelihood of needing the implant removed compared to the pre-contoured plate.

Introduction: Acetabular fractures are devastating injuries that can affect patients of different ages, as well as being associated with high or low energy trauma. Conversion to total hip arthroplasty (THA) results in increased complication rates, resource use and costs compared to primary THA due to osteoarthritis. The aim of this paper is to describe a retrospective cohort of patients over 65 years of age who presented with an acetabular fracture and were treated with open reduction and internal fixation (ORIF).

Objectives: Obtaining a blood-free surgical field is critical during carpal tunnel decompression (CTD) to identify anatomic structures and avoid iatrogenic injury. A tourniquet is often used to minimize bleeding and improve visualization. However, it may be associated with discomfort and intolerance when sedation is not employed.

Purpose: The management of scoliosis and kyphoscoliosis in patients with Type 1 Neurofibromatosis (NF-1) among spinal surgeons is still challenging due to the severity of the deformity especially in dystrophic deformity types. This rapid and progressive condition is likely to be associated with dislocated rib heads into the spinal canal, hence representing a real dilemma on the decision making between its resection versus not resection during the corrective surgery, especially in patients with normal neurological status. The objective of this publication is to discuss the management options in this patient population through a literature review.

Introduction: Closed reduction and fixation using a cephalomedullary nail represents the accepted management of unstable intertrochanteric fractures. Cut-through has been described as a complication associated with the treatment. Although a hip arthroplasty may be the most predictable revision method, a non-prosthetic option can lead to similar results.
