Background: Uncontrolled donation after circulatory death (uDCD) increases organ availability for kidney transplantation (KT) at the expense of a higher risk of primary graft nonfunction (PNF). At least half of the cases of PNF are secondary to graft venous thrombosis. The potential benefit from prophylactic anticoagulation in this scenario remains unclear.

Article Synopsis
  • Preterm birth and low birthweight (LBW) are likely linked to decreased physical fitness, though existing evidence has had mixed results.
  • *The study reviewed 52 studies with over 920,000 participants to assess the impact of these factors on various aspects of physical fitness, finding significant reductions in cardiorespiratory fitness and muscle strength.
  • *The analysis indicates that earlier gestational age and lower birthweight correspond with greater reductions in physical fitness across all ages.

Pregnancy, Reproductive Factors, and Female Heart Failure Risk and Outcomes.

Curr Heart Fail Rep

June 2024

Grupo de Investigación Cardiovascular Multidisciplinar Traslacional (GICMT), Área de Investigación Cardiovascular, Instituto de Investigación Hospital 12 de Octubre (imas12), Madrid, Spain.

Purpose Of Review: The purpose of this review is to provide an overview of recent evidence on female-specific risk factors related to reproductive status or pregnancy.

Recent Findings: Pregnancy-related factors, including hypertensive disorders and gestational diabetes, increase the risk of heart failure in women, while breastfeeding and hormone therapy may offer protection. Hypertensive disorders of pregnancy, gestational diabetes, polycystic ovarian syndrome, placental abruption, younger maternal age at first live birth, younger maternal age at last live birth, number of stillbirths, number of pregnancies, onset of menstruation before 12 years of age, shorter reproductive age, ovariectomy, and prolonged absence of ovarian hormones may increase the risk of heart failure in women.

Lifestyle and cardiovascular risk in working young adults: insights from a nationwide Spanish cohort.

Rev Esp Cardiol (Engl Ed)

October 2024

Physical Activity and Health Research Group (PaHerg), Instituto de Investigación Hospital 12 de Octubre (imas12), Madrid, Spain; Faculty of Sport Sciences, Universidad Europea de Madrid, Madrid, Spain.

Introduction And Objectives: This study aimed to describe the cardiovascular risk profile of working young adults from Spain and its association with lifestyle.

Methods: Participants (18-30 years) were recruited from a nationwide cohort of economically active adults insured by a large occupational risk prevention company, with data obtained from routine medical assessments. The participants were categorized as having an "unhealthy" cardiovascular risk profile based on the presence of prediabetes/diabetes, prehypertension/hypertension, or hypercholesterolemia, or a "healthy" profile if these conditions were completely absent.

Purpose: The gut microbiota plays important roles in health and disease. We questioned whether the gut microbiota and related metabolites are altered in monoclonal gammopathies and evaluated their potential role in multiple myeloma and its response to treatment.

Experimental Design: We used 16S rRNA sequencing to characterize and compare the gut microbiota of patients with monoclonal gammopathy of undetermined significance (n = 11), smoldering multiple myeloma (n = 9), newly diagnosed multiple myeloma (n = 11), relapsed/refractory multiple myeloma (n = 6), or with complete remission (n = 9).

Article Synopsis
  • Immunoglobulin A nephropathy (IgAN) is the most common type of kidney inflammation globally, with recent research highlighting the critical role of the complement system, especially the alternative pathway, in its progression.
  • Studies indicate that specific proteins like factor H-related proteins and the lectin pathway contribute to disease severity, and glomerular deposition of markers like C3 and C4d may predict worse outcomes.
  • The understanding of complement's involvement in IgAN has led to the development of new treatments targeting various components of the complement system, with several clinical trials currently underway to assess their effectiveness.

Front Pediatr

November 2023

Division of Neonatology, Department of Translational Medical Sciences, University of Naples Federico II, Naples, Italy.

Congenital cytomegalovirus (cCMV) infection is the most common congenital infection, with an estimated incidence of approximately one in 200 infants in high-income settings. Approximately one in four children may experience life-long consequences, including sensorineural hearing loss and neurodisability. Knowledge regarding prevention, diagnosis, and treatment increased in the recent years, but some challenges remain.

Engineering of reverse genetics systems for newly emerged viruses allows viral genome manipulation, being an essential tool for the study of virus life cycle, virus-host interactions and pathogenesis, as well as for the development of effective antiviral strategies. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is an emergent human coronavirus that has caused the coronavirus disease (COVID-19) pandemic. The engineering of a full-length infectious cDNA clone and a fluorescent replicon of SARS-CoV-2 Wuhan-Hu-1, using a bacterial artificial chromosome, is reported.

Systemic sclerosis (SSc) is an autoimmune connective tissue disease characterized by immune dysregulation and progressive fibrosis, typically affecting the skin, with variable internal organ involvement. Interstitial lung disease (ILD), with a prevalence between 35 and 75%, is the leading cause of death in patients with SSc, indicating that all newly diagnosed patients should be screened for this complication. Some patients with SSc-ILD experience a progressive phenotype, which is characterized by worsening fibrosis on high-resolution computed tomography (HRCT), a decline in lung function, and premature mortality.

An Pediatr (Engl Ed)

November 2023

Pediatric Infectious Diseases Unit, Pediatrics Department, Hospital Universitario 12 de Octubre, Madrid, Spain; Pediatric Research and Clinical Trials Unit (UPIC), Instituto de Investigación Hospital 12 de Octubre (imas12), Madrid, Spain.

Introduction: Fighting against antimicrobial resistance is a current priority, and further efforts need to be made to improve antimicrobial prescribing and reduce the spread of infections in paediatric care settings.

Methods: We conducted a prospective longitudinal study on the use of antimicrobials from the time the antimicrobial stewardship programme (ASP) was introduced in January 2016 to December 2017 (period 2 [P2]) in our children's hospital. We compared the obtained results on antimicrobial prescribing with retrospective data from the period preceding the introduction of the ASP (2014-2015, period 1 [P1]).
