Unlabelled: Antivirals such as nucleotide analogs (NAs) are potent inhibitors of hepatitis B virus (HBV) replication. However, NAs fail to diminish the signaling and mitogenic activities of the transactivator HBx protein. Earlier we have shown that thiourea derivative IR-415 (DSA-00) targeted HBx to down-regulate its target viral and host genes.

Background And Aims: Acute liver failure (ALF) has high mortality predominantly due to compromised immune system and increase vulnerability to bacterial and fungal infections.

Method: Plasma lipidome and fungal peptide-based-community (mycobiome) analysis were performed in Discovery cohort (40-ALF, 5-healthy) and validated in a validation cohort of 230-ALF using High-resolution-mass-spectrometry, artificial-neural-network (ANN) and machine-learning (ML).

Results: Untargeted lipidomics identified 2,013 lipids across 8 lipid-groups.

Article Synopsis
  • * The incidence of HCC varies significantly worldwide, with the highest rates in East Asia and sub-Saharan Africa due to the prevalence of hepatitis B and C viruses, while rising cases in Western countries are associated with increasing alcohol abuse and metabolic issues like steatosis liver disease.
  • * Prevention strategies differ by region and emphasize vaccination against hepatitis B, antiviral treatments, moderation of alcohol consumption, and lifestyle changes to tackle obesity and reduce the risk of MASLD-related HCC.

Article Synopsis
  • This study explores how the pharmacokinetics of propofol—specifically its required concentrations—varies in different phases (dissection, anhepatic, and neohepatic) during liver transplantation.
  • It involved 20 patients with chronic liver disease, and used a target-controlled infusion method to adjust propofol levels based on the bispectral index (BIS), which measures consciousness levels.
  • Results showed that the mean target concentration of propofol was highest during the dissection phase, significantly decreasing during the anhepatic and neohepatic phases, indicating a need for lower propofol dosages in those latter stages.

The limited replicative potential of primary hepatocytes (Hep) is a major hurdle for obtaining sufficient quantity and quality hepatocytes during cell therapy in patients with liver failure. Intrahepatic cholangiocyte organoids (ICOs) derived from intrahepatic bile ducts differentiate into both hepatocytes and cholangiocytes . Here, we studied effects of transplanting ICOs and Hep in chronic liver injury mice models.

Background/aims: Stereotactic body radiation therapy (SBRT) has evolved as a treatment alternative for advanced hepatocellular carcinoma (HCC) patients who are ineligible for other local therapies. Posttreatment responses are assessed by imaging modalities, serum AFP, and protein induced by vitamin K absence-II (PIVKA) II levels. Despite good specificity, both AFP and PIVKA-II have low to medium sensitivity.

Skin colour usually depends upon melanin, haemoglobin, and carotenoids. Pigmentary disorders indicate an increased amount of melanin, leading to a darker colour of the skin, called hypermelanosis. Addison's disease is a rare endocrinal disorder with severe oral and systemic manifestations.

Article Synopsis
  • Carcinoma of the gall bladder (CAGB) has a low survival rate, but analyzing bile samples can help identify molecular indicators for early detection of the disease.
  • A study on 87 bile samples found specific proteomic and metabolomic signatures associated with CAGB that correlated with increased inflammation and altered energy pathways compared to healthy controls.
  • The top four metabolites showed over 99% accuracy and sensitivity for CAGB detection in further validation tests, suggesting that bile analysis could lead to improved early diagnosis methods.

Article Synopsis
  • - The study aimed to evaluate the safety, effectiveness, and tolerability of Naltrexone in treating alcohol use disorder in patients with alcohol-related cirrhosis.
  • - After 4 weeks of treatment, there were notable reductions in liver enzyme levels, self-reported craving, AUDIT scores, and the number of drinking days, indicating potential benefits.
  • - The findings suggest that Naltrexone is safe and effective for managing alcohol use disorder in patients with liver complications, supporting its clinical use in these cases.

Background And Aim: Acute-on-chronic liver failure (ACLF) patients with hepatorenal syndrome (HRS-AKI) have limited response to vasoconstrictors and worse outcomes, requiring biomarkers for early detection.

Methods: In a prospective cohort of ACLF patients (n = 240), urine NGAL was performed in patients with the clinical diagnosis of HRS-AKI, while in a subset of patients (n = 30), a complete panel of 17 urinary biomarkers was assessed for identifying terlipressin non-response (T-NR).

Results: ACLF patients with HRS-AKI, aged 45.

Background: Bowel colonization with antimicrobial-resistant bacteria has been associated with worse clinical outcomes in patients with cirrhosis; however, it has not been studied in patients with acute-on-chronic liver failure (ACLF). We evaluated whether fecal isolation of carbapenem-resistant gram-negative bacteria (CR-GNB) among patients with ACLF affects short-term outcomes.

Methods: Patients of APASL-ACLF (n = 339) were screened between June 2020 and December 2021, and 150 were included.

Article Synopsis
  • Hypoglycemia can signal inborn metabolic errors, often seen in patients with liver issues, but there's no agreed-upon definition or diagnostic method.
  • The text presents four pediatric cases of different ages who showed hypoglycemia as their main symptom.
  • It also offers a systematic diagnostic guide for better identifying inherited metabolic liver diseases linked to hypoglycemia.

Mutations in doublecortin domain-containing protein 2 (DCDC2) lead to neonatal sclerosing cholangitis (NSC), and portal hypertension (PHTN). The objective of the study was to systematically evaluate PHTN, variceal bleeding, and outcomes of patients with DCDC2-related NSC. The study included children with homozygous or compound heterozygous variants in DCDC2.

Injectable hydrogels are a sub-type of hydrogels which can be delivered into the host in a minimally invasive manner. They can act as carriers to encapsulate and deliver cells, drugs or active biomolecules across several disease conditions. Polymers, either synthetic or natural, or even a combination of the two, can be used to create injectable hydrogels.

Since the Asian Pacific Association for the Study of the Liver (APASL) published guidelines on non-cirrhotic portal fibrosis/idiopathic portal hypertension in 2007, there has been a surge in new information, especially with the introduction of the term porto-sinusoidal vascular disorder (PSVD). Non-cirrhotic intra-hepatic causes of portal hypertension include disorders with a clearly identifiable etiology, such as schistosomiasis, as well as disorders with an unclear etiology such as non-cirrhotic portal fibrosis (NCPF), also termed idiopathic portal hypertension (IPH). This entity is being increasingly recognized as being associated with systemic disease and drug therapy, especially cancer therapy.

Purpose: The early post-transplant period after liver transplantation is critical, as recipients are highly susceptible to sepsis due to their immune-compromised state. This study aimed to identify the association between preoperative procalcitonin and early post-transplant sepsis and mortality at one month after living donor liver transplantation (LDLT).

Methodology: All patients who underwent LDLT from July 2021 to December 2021 were recruited prospectively.
