Acute Kidney Injury (AKI) is frequently observed in hospitalized patients in intensive care units, often caused by renal ischemia-reperfusion injury (IRI). IRI disrupts the function of various 'remote organs' such as the lungs, pancreas, intestine, liver, heart, and brain through inflammation, oxidative stress, apoptosis, leukocyte infiltration, and increased urea and creatinine levels. Gender differences in renal IRI-induced injury are noted.

Using an Isolated Working Rat Heart Model to Test Donor Heart Preservation Strategies.

J Vis Exp

October 2024

Cardiac Physiology and Transplantation, Victor Chang Cardiac Research Institute; Heart and Lung Transplant Unit, St Vincent's Hospital; Faculty of Medicine, University of New South Wales.

Optimization of donor heart preservation solutions has played a key role in reducing ischemia-reperfusion injury in donor hearts during organ retrieval, transportation, and transplantation. Previous work from our laboratory showed that the addition of glyceryl trinitrate and erythropoietin to donor heart preservation solutions could significantly improve cardiac functional recovery after prolonged cold storage and in donation after circulatory death hearts. This supplementation protocol has been implemented in clinical use in transplant units around Australia, Belgium, and the United Kingdom.

Background: There is significant variability among pathologists in the histopathological interpretation of the endomyocardial biopsy (EMB) for acute cellular rejection (ACR), and assessment of variability in the interpretation of antibody-mediated rejection (AMR) has not been reported. In contemporary practice, the strategy of allograft surveillance with donor-derived cell-free DNA (dd-cfDNA) compared to EMB has not been compared with a focus on long-term clinical outcomes beyond acute rejection (AR).

Methods: The Genomic Research Alliance for Transplantation is a multicenter, prospective cohort study that enrolled patients from 2015 to 2020.

A tendon's ordered extracellular matrix (ECM) is essential for transmitting force but is also highly prone to injury. How tendon cells embedded within and surrounding this dense ECM orchestrate healing is not well understood. Here, we identify a specialized quiescent Scx/Axin2 population in mouse and human tendons that initiates healing and is a major functional contributor to repair.

BIVV003 is a gene-edited autologous cell therapy in clinical development for the potential treatment of sickle cell disease (SCD). Hematopoietic stem cells (HSC) are genetically modified with mRNA encoding zinc finger nucleases (ZFN) that target and disrupt a specific regulatory GATAA motif in the BCL11A erythroid enhancer to reactivate fetal hemoglobin (HbF). We characterized ZFN-edited HSC from healthy donors and donors with SCD.

Article Synopsis
  • Poor agreement among lung transplant pathologists in assessing rejection has been noted, making inter-rater reliability crucial for effective patient care and clinical trial inclusion criteria.
  • A survey and diagnostic sessions with nine pathologists from eight centers evaluated their practices and scoring of high-risk histopathologic findings related to chronic lung allograft dysfunction (CLAD).
  • The results showed good inter-rater reliability for identifying high-risk findings after these alignment sessions, indicating that collaborative discussions improved consensus among pathologists.

J Heart Lung Transplant

February 2025

Division of Geographic Medicine and Infectious Diseases, Department of Medicine, Tufts Medical Center, Boston, Massachusetts.

Article Synopsis
  • Infections after orthotopic heart transplantation (OHT) can lead to serious health issues, prompting a study to evaluate the impact of different types of mechanical circulatory support (MCS) before surgery.
  • Researchers analyzed data from 320 OHT patients at Tufts Medical Center between 2014 and 2022, focusing on the risk of various infections within a year post-transplant based on whether patients received temporary or durable MCS pre-surgery.
  • The findings showed that there was no significant difference in the time to first infection between those who received temporary MCS and those with durable MCS, suggesting that the type of MCS used before OHT may not influence post-operative infection rates.

Article Synopsis
  • The study aimed to assess the likelihood of recurrent Clostridioides difficile infection (CDI) in solid-organ transplant (SOT) recipients during the year following their transplant.
  • Researchers analyzed data from 191 SOT patients who had their first CDI episode, finding a 12% recurrence rate and highlighting that severe CDI and metronidazole monotherapy significantly raised this risk.
  • The findings suggest that using metronidazole alone increases the chances of a recurrence, indicating a need for improved treatment strategies to prevent repeated infections in these patients.
