Article Synopsis
  • SNPs linked to chronic kidney disease (CKD) show different impacts on urinary uromodulin levels in Black and White populations, with unknown factors in African groups.
  • Clinical studies involving young and middle-aged adults reveal that Black individuals have lower uromodulin levels and higher prevalence of CKD-risk SNPs compared to Whites.
  • Findings highlight significant ethnic variations in genetic and clinical factors affecting uromodulin, suggesting the need for tailored approaches in kidney function research and medicine.

Background: Identifying the subset of patients at risk for developing persistent pain after surgery is clinically important as they could benefit from targeted prevention measures. In this prospective study, we investigated if the preoperative assessment of the individual susceptibility to developing experimentally induced secondary hyperalgesia is associated with post-thoracotomy pain at 2 months.

Methods: Forty-one patients scheduled to undergo a posterolateral thoracotomy were recruited before surgery and followed prospectively for 2 months.

Systemic vasculitides are autoimmune diseases characterized by inflammation of blood vessels. They are categorized based on the size of the preferentially affected blood vessels: large-, medium-, and small-vessel vasculitides. The main forms of large-vessel vasculitis include giant cell arteritis (GCA) and Takayasu arteritis (TAK).

Background: Selective biomarkers may improve outcomes in patients with recurrent or metastatic head and neck squamous cell carcinoma (R/M HNSCC) treated with immune checkpoint inhibitor therapy. We investigated three independent biomarkers for association with efficacy in the randomized, phase III KESTREL study (NCT02551159) of first-line durvalumab monotherapy or durvalumab plus tremelimumab versus the EXTREME regimen: programmed cell death ligand-1 (PD-L1) immunohistochemistry, blood tumor mutational burden (bTMB) via circulating tumor DNA, and neutrophil-to-lymphocyte ratio (NLR).

Methods: Tumor or blood samples from patients enrolled in the KESTREL study were analyzed for PD-L1, bTMB, and NLR.

Healthcare expenses are increasing, as is the utilization of laboratory resources. Despite this, between 20% and 40% of requested tests are deemed inappropriate. Improper use of laboratory resources leads to unwanted consequences such as hospital-acquired anemia, infections, increased costs, staff workload and patient stress and discomfort.

[F]FDG-PET/CT in Polymyalgia Rheumatica: An Update and Future Aspects.

Semin Nucl Med

May 2024

Polymyalgia rheumatica (PMR) is an inflammatory disorder usually diagnosed in patients older than 50 years of age. It is characterized by sudden onset pain and prolonged morning stiffness in the scapular and/or pelvic girdle, sometimes debilitating and accompanied by constitutional symptoms such as weight loss. In approximately 20% of the cases, it is linked to giant cell arteritis (GCAV) representing a disease continuum.

Chronic immune diseases mainly include autoimmune and inflammatory diseases. Managing chronic inflammatory and autoimmune diseases has become a significant public health concern, and therapeutic advancements over the past 50 years have been substantial. As therapeutic tools continue to multiply, the challenge now lies in providing each patient with personalized care tailored to the specifics of their condition, ushering in the era of personalized medicine.

Elevated Protein Induced by Vitamin-K Absence-II (PIVKA-II) has been shown to be an adverse prognostic factor in HCC patients undergoing liver transplantation (LT). No definitive data are available about the impact of PIVKA-II concerning post-LT recurrence in patients not secreting (≤ 20 ng/mL) alpha-fetoprotein (AFP). An observational retrospective study of the East-West HCC-LT consortium is reported.

Article Synopsis
  • - Small-for-size syndrome (SFSS) is a significant complication that can occur in up to 20% of patients after living donor liver transplantation (LDLT), requiring careful planning and surgical considerations to prevent it.
  • - A conference held in January 2023 led by international experts focused on developing evidence-based recommendations for predicting and managing SFSS in LDLT, utilizing a structured evaluation system.
  • - The final recommendations from one of the conference's working groups emphasize best practices in donor and recipient selection, surgical techniques, and critical care strategies to reduce the risk of SFSS in patients undergoing LDLT.

Article Synopsis
  • BI 894999, a novel oral BET inhibitor, shows strong antitumour activity in advanced solid tumours, as evidenced by preclinical studies and a Phase Ia/Ib dose-finding trial.
  • The trial enrolled 77 patients and evaluated various dosing schedules, with reported Grade ≥3 dose-limiting toxicities (DLTs) including thrombocytopenia and other significant side effects.
  • Max tolerated doses were established at 1.5 mg for Schedule A, 2.5 mg for Schedule B, and 6.0/3.0 mg for Schedule C, leading to further evaluation of BI 894999 in a Phase Ib trial.

Article Synopsis
  • The study aimed to create and validate a radiomic model using [F]FDG PET scans to predict overall survival in patients with head and neck squamous cell carcinoma (HNSCC).
  • It involved analyzing two datasets of patients treated with afatinib, extracting radiomic features from their PET scans, and using these features alongside clinical data to develop a Cox regression model for predictions.
  • The radiomic model showed better performance in predicting survival than clinical models alone, suggesting that combining radiomic features with clinical variables could enhance predictions, though further validation with larger patient groups is needed.

Background: Patients with recurrent or metastatic head and neck squamous cell carcinoma (R/M HNSCC) have a poor prognosis. The phase III KESTREL study evaluated the efficacy of durvalumab [programmed death-ligand 1 (PD-L1) antibody] with or without tremelimumab [cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4) antibody], versus the EXTREME regimen in patients with R/M HNSCC.

Patients And Methods: Patients with HNSCC who had not received prior systemic treatment for R/M disease were randomized (2 : 1 : 1) to receive durvalumab 1500 mg every 4 weeks (Q4W) plus tremelimumab 75 mg Q4W (up to four doses), durvalumab monotherapy 1500 mg Q4W, or the EXTREME regimen (platinum, 5-fluorouracil, and cetuximab) until disease progression.

Update on Imaging of Inflammatory Arthritis and Related Disorders.

Semin Nucl Med

March 2023

Department of Nuclear Medicine, Cliniques universitaires St-Luc and Institute for Experimental and Clinical Research (IREC), Université Catholique de Louvain, Brussels, Belgium.

Arthritis and other rheumatic disorders are very frequent in the general population and responsible for a huge physical and disability burden to affected patients as well as a major cost to the society. Precise evaluation often relies on clinical data only but additional imaging may be required i) for a more objective assessment of the disease status, such as in rheumatoid arthritis (RA) or ankylosing spondyloarthritis (AS), ii) for providing prognostic information and evaluating response to treatment or iii) for establishing diagnosis, in patients with unclear clinical picture, such as polymyalgia rheumatica (PMR) and large-vessel vasculitis (LVV). Besides radiological techniques (x-rays, ultrasound, and MRI), functional and molecular imaging has emerged as a valid tool for this purpose in several disorders.
