Drug Treatments for Neurodevelopmental Disorders: Targeting Signaling Pathways and Homeostasis.

Curr Neurol Neurosci Rep

December 2024

Kennedy Krieger Institute and Department of Psychiatry & Behavioral Sciences-Child Psychiatry, Johns Hopkins University School of Medicine, Baltimore, MD, USA.

Purpose Of The Review: Preclinical and clinical evidence support the notion that neurodevelopmental disorders (NDDs) are synaptic disorders, characterized by excitatory-inhibitory imbalance. Despite this, NDD drug development programs targeting glutamate or gamma-aminobutyric acid (GABA) receptors have been largely unsuccessful. Nonetheless, recent drug trials in Rett syndrome (RTT), fragile X syndrome (FXS), and other NDDs targeting other mechanisms have met their endpoints.

Division on Substance Use Disorders, New York State Psychiatric Institute and Department of Psychiatry, Columbia University Irving Medical Center, 1051 Riverside Drive, Unit 120, New York, NY 10032, USA.

Individuals seeking treatment for their cocaine use often report depressive systems and nearly half meet criteria for major depressive disorder (MDD). This descriptive study aimed to assess the effects of the antidepressant venlafaxine alone and in combination with gabapentin on depressive symptoms, subjective effects of cocaine, and cocaine self-administration in depressed and non-depressed people who use cocaine. The effects of medication condition on mood and on the effects of smoked cocaine were compared between a group of clinically depressed people who use cocaine (n = 5) and a control group of non-depressed people who use cocaine (n = 5) using laboratory-based measures.

Stereological Analysis of the Rhesus Monkey Perirhinal and Parahippocampal Cortices.

J Comp Neurol

November 2024

Laboratory of Brain and Cognitive Development, Institute of Psychology, University of Lausanne, Lausanne, Switzerland.

The perirhinal and parahippocampal cortices are key components of the medial temporal lobe memory system. Despite their essential roles in mnemonic and perceptual functions, there is limited quantitative information regarding their structural characteristics. Here, we implemented design-based stereological techniques to provide estimates of neuron number, neuronal soma size, and volume of the different layers and subdivisions of the perirhinal and parahippocampal cortices in adult macaque monkeys (Macaca mulatta, 5-9 years of age).

Breaking barriers: why including boys and men is key to HPV prevention.

BMC Med

November 2024

HIV Center for Clinical and Behavioral Studies, Gender, Sexuality and Health Area, New York State Psychiatric Institute and Department of Psychiatry, Columbia University Irving Medical Center, New York, USA.

Despite the common belief that human papillomavirus (HPV) primarily affects women, it is imperative to recognize and address the impact of HPV on boys and men. Overlooking the health implications for males is a notable gap, as efforts have predominantly focused on preventing HPV-related infections in women. This commentary aims to emphasize the importance of HPV awareness and vaccination for boys and men by highlighting the prevalence and consequences of HPV infection, elaborating on the associated health risks, elucidating the benefits of vaccination, and urging readers to recognize the necessity of protecting males from HPV.

Introduction: Multinational treatment guidelines support providing cognitive remediation to people recently diagnosed with schizophrenia, but the feasibility of implementing the treatment on a large scale is less well understood.

Methods: This study took place between 2019 and 2023 at 14 clinics within a large network of programs providing early intervention services to people aged 16-30 experiencing nonaffective psychosis. Clinics were randomly assigned to deliver cognitive remediation as twice-weekly clinician-led groups (N = 5), cognitive remediation as once-weekly clinician-led groups with homework (N = 6), or treatment as usual (N = 3).

Department of Psychiatry, Interdisciplinary Centre Psychopathology and Emotion Regulation, University of Groningen, University Medical Center Groningen, Groningen, Netherlands.

Introduction: Psychosis often develops gradually along a continuum of severity. Little is known about the role of protective factors such as positive affect (PA) in the development of psychotic experiences (PEs). This study investigated i) the temporal (between-day) and contemporaneous (within-day) daily associations between PA and PEs in individuals at different early clinical stages for psychosis and ii) whether these associations differed per clinical stage.

Major depressive disorder (MDD) is a common psychiatric disorder for which pharmacologic standard-of-care treatments have limited efficacy, particularly among individuals with cognitive dysfunction. Cognitive dysfunction is observed in approximately 25%-50% of those with MDD, wherein response to standard-of-care medications is reduced. Vortioxetine is an approved antidepressant that has shown evidence of procognitive effects in patients.

Article Synopsis
  • - The study investigated the role of mGlu5 receptors in individuals with methamphetamine use disorder (MUD) compared to a control group, focusing on cognitive performance and brain activity measured by PET scans.
  • - Results showed no significant differences in mGlu5 levels between MUD participants and controls, but MUD individuals performed worse on certain cognitive tests, particularly in spatial working memory.
  • - Findings suggest that while mGlu5 receptors are not downregulated in abstinent MUD patients, targeting these receptors could potentially enhance cognitive functioning, especially in verbal learning tasks.

All definitions of treatment-resistant depression (TRD) require that patients have experienced insufficient benefit from one or more adequate antidepressant trials. Thus, identifying "failed, adequate trials" is key to the assessment of TRD. The Antidepressant Treatment History Form (ATHF) was one of the first and most widely used instruments that provided objective criteria in making these assessments.

Shifting From Best Practice to Standard Practice: Implementing Measurement-Based Care in Health Systems.

Child Psychiatry Hum Dev

June 2024

Center for Depression Research and Clinical Care, Peter O'Donnell Jr. Brain Institute and Department of Psychiatry, University of Texas Southwestern Medical Center, 5323 Harry Hines Blvd, Dallas, TX, 75390-9119, USA.

There is a high prevalence of untreated depression in adults and youth observed at the population level in the United States, and many who would benefit from treatment do not receive it. One proposed effort to increase access to care is the use of measurement-based care (MBC; repeated use of symptom measures for screening and treatment guidance) by primary care physicians to treat non-complex cases of depression. MBC has been shown to improve patient outcomes compared to care as usual, but there are barriers that need to be addressed at the health system level for effective implementation to occur.
