The brain is highly innervated by sympathetic nerve fibres; however, their physiological purpose is poorly understood. We hypothesized that unilateral cerebral norepinephrine (NE) spillover, an index of cerebral sympathetic nerve activity (SNA), would be elevated when engaging the baroreflex [via lower-body negative pressure (LBNP; -20 and -40 Torr)] and respiratory chemoreflexes [via carbon dioxide (CO) administration (+8 Torr)], independently, and in combination. Twelve young and healthy participants (5 females) underwent simultaneous blood sampling from the right radial artery and internal jugular vein.

Wild-type transthyretin cardiac amyloidosis and aortic stenosis: Can carpal tunnel syndrome help distinguish the chicken from the egg?

J Intern Med

November 2024

Referral Center for Cardiac Amyloidosis, Mondor Amyloidosis Network, GRC Amyloid Research Institute and Cardiology Department, APHP Henri Mondor Hospital, Créteil, France.

Article Synopsis
  • - The study investigates the relationship between transthyretin wild-type cardiac amyloidosis (TTRwt-CA) and aortic stenosis (AS), proposing a bidirectional connection where each condition may influence the other and lead to systemic manifestations, including carpal tunnel syndrome (CTS).
  • - In a sample of 411 TTRwt-CA patients, approximately 70% had CTS, which correlated with younger age, more severe cardiac remodeling, and a higher frequency of extracardiac symptoms, while AS occurred in 21% of those with CTS and 31% without.
  • - The research indicates that CTS may differentiate two phenotypes of TTRwt-CA: one associated with systemic involvement and poorly calcified low-flow

Introduction: Deregulation of the cerebrovascular system has been linked to neurodegeneration, part of a putative causal pathway into etiologies such as Alzheimer's disease (AD). In medical imaging, time-of-flight magnetic resonance angiography (TOF-MRA) and perfusion MRI are the most common modalities used to study this system. However, due to lack of resources, many large-scale studies of AD are not acquiring these images; this creates a conundrum, as the lack of evidence limits our knowledge of the interaction between the cerebrovascular system and AD.

Introduction: With the implementation of low-dose computed tomography screening, multiple pulmonary tumor nodules are diagnosed with increasing frequency and the selection of surgical treatments versus systemic therapies has become challenging on a daily basis in clinical practice. In the presence of multiple carcinomas, especially adenocarcinomas, pathologically determined to be of pulmonary origin, the distinction between separate primary lung carcinomas (SPLCs) and intrapulmonary metastases (IPMs) is important for staging, management, and prognostication.

Methods: We systemically reviewed various means that aid in the differentiation between SPLCs and IPMs explored by histopathologic evaluation and molecular profiling, the latter includes DNA microsatellite analysis, array comparative genomic hybridization, TP53 and oncogenic driver mutation testing and, more recently, with promising effectiveness, next-generation sequencing comprising small- or large-scale multi-gene panels.

Importance: The use of antipsychotics, antidepressants, and benzodiazepines may influence the risk of mortality in people with schizophrenia. However, many observational studies have not accounted for immortal time bias (ITB), which occurs when there is a period during which patients in the exposed group are necessarily alive and misclassified as exposed (the period between start of follow-up and initiation of drug). Ignoring ITB may lead to misinterpretation of the association between these drugs and mortality.

Molecular Features of Calcific Aortic Stenosis in Female and Male Patients.

CJC Open

September 2024

Institut universitaire de cardiologie et pneumologie de Québec [Quebec Heart & Lung Institute], Université Laval, Québec City, Québec, Canada.

Over the past 15 years, sex-related differences in aortic valve (AV) stenosis (AS) have been highlighted, affecting various aspects of AS, such as the pathophysiology, AV lesions, left ventricle remodelling, and outcomes. Female patients were found to present a more profibrotic pattern of leaflet remodelling and/or thickening, whereas male patients have a preponderance of calcification within stenosed leaflets. The understanding of these sex differences is still limited, owing to the underrepresentation of female patients in many basic and clinical research studies and trials.
