Article Synopsis
  • - Human gene replacement therapies like onasemnogene abeparvovec (OA) use viral vectors to treat genetic disorders, but there's a risk of cardiac and liver toxicity, prompting monitoring after treatment.
  • - Preclinical studies on mice showed dose-related cardiac issues such as inflammation and early death at high doses, while similar effects were not seen in non-human primates or humans.
  • - While some patients experienced minor heart enzyme elevations after OA treatment, these were not linked to significant cardiac problems, suggesting that the toxic effects observed in mice don't apply to humans.

Maternal Bacterial Engraftment in Multiple Body Sites of Cesarean Section Born Neonates after Vaginal Seeding-a Randomized Controlled Trial.

mBio

June 2023

Clinical Microbiome Unit (CMU), Laboratory of Host Immunity and Microbiome, National Institute of Allergy and Infectious Diseases, National Institutes of Health, Bethesda, Maryland, USA.

Article Synopsis
  • Elective, prelabor C-sections limit newborn exposure to beneficial vaginal microbiota, potentially leading to differences in microbiota development and increased risks of immune and metabolic diseases.
  • A clinical trial compared vaginal seeding (applying maternal vaginal fluids to C-section newborns) against a placebo to see its effects on the microbiota in neonates.
  • Results showed that vaginal seeding improved transfer of maternal microbes and changed the composition of skin and stool microbiota, highlighting the need for further research to explore its long-term impacts.

Unlabelled: Accurate inpatient placement at the level of care needed for incoming transported patients can present challenges that may result in a rapid escalation in care following admission to the general inpatient unit. The Pediatric Early Warning System (PEWS) score is useful for early recognition of clinical deterioration. Therefore, we aimed to reduce rapid escalations in care for incoming transported patients via the implementation of an escalation algorithm with a goal of 80% PEWS score completion rate during transport within 6 months.

Seeking justice, equity, diversity and inclusion in pediatric nephrology.

Front Pediatr

December 2022

Division of Pediatric Nephrology, Children's National Hospital, George Washington School of Medicine, Washington, DC, United States.

Inequity, racism, and health care disparities negatively impact the well-being of children with kidney disease. This review defines social determinants of health and describes how they impact pediatric nephrology care; outlines the specific impact of systemic biases and racism on chronic kidney disease care and transplant outcomes; characterizes and critiques the diversity of the current pediatric nephrology workforce; and aims to provide strategies to acknowledge and dismantle bias, address barriers to care, improve diversity in recruitment, and strengthen the pediatric nephrology community. By recognizing historical and current realities and limitations, we can move forward with strategies to address racism and bias in our field and clinical practices, thereby cultivating inclusive training and practice environments.

Am J Kidney Dis

May 2023

Division of Nephrology, Seattle Children's Hospital; Department of Pediatrics, School of Medicine, University of Washington, Seattle, Washington. Electronic address:

Rationale & Objective: Accurate detection of hypertension is crucial for clinical management of pediatric chronic kidney disease (CKD). The 2017 American Academy of Pediatrics (AAP) clinical practice guideline for childhood hypertension included new normative blood pressure (BP) values and revised definitions of BP categories. In this study, we examined the effect of applying the AAP guideline's normative data and definitions to the Chronic Kidney Disease in Children (CKiD) cohort compared with use of normative data and definitions from the 2004 Fourth Report on the Diagnosis, Evaluation, and Treatment of High Blood Pressure in Children and Adolescents.

Together, heart failure and arrhythmia represent the most important cardiovascular sources of morbidity and mortality among adults with congenital heart disease (ACHDs). Although traditionally conceptualized as operating within 2 distinct clinical silos, these scenarios frequently coexist within the same individual; consequently the mechanistic, therapeutic, and prognostic overlap between them demands increased recognition. In fact, given the near ubiquity of heart failure and arrhythmia among ACHDs, there is perhaps no other arena within cardiology where this critical intersection is more frequently observed.

Arrhythmias in the neonatal period are common and can be classified as bradyarhythmias and tachyarrhythmias and as benign or non-benign. Neonatal arrhythmias are further differentiated between those with abnormalities in generation (non-sinus) and those with abnormalities in propagation. Because the neonatal myocardium is immature and operates at the peak of the Starling curve, significant changes in heart rate can result in a decline in cardiac output and compromise end-organ perfusion.
