The degree of fluid absorption during hysteroscopic surgery in patients pretreated with goserelin.

Study Objective: To assess the effects of pretreatment with the gonadotropin-releasing hormone analog goserelin on fluid absorption in patients undergoing hysteroscopic endometrial ablation.

Design: Prospective, randomized, placebo-controlled study.

Setting: A university-based clinic.

Unusual frontal osteoma, mimicking a haemangioma.

We report a patient with a frontal bone osteoma showing the classical findings of haemangioma. A craniectomy and total tumour excision was carried out.

A huge intramedullary epidermoid cyst: mimicking cauda equina ependymoma. Diagnostic failure of myelography and myelo-CT.

Intraspinal epidermoid tumors are rare and the intramedullary location is quite uncommon. In this report, a 48-year-old patient with intramedullary epidermoid cyst mimicking a cauda equina ependymoma is presented and diagnostic failure of conventional myelography and Myelo-CT is questioned and related literature is reviewed.

A huge cerebral hydatid cyst associated with small liver cyst: a comparison of growth rates and sizes.

There are still some unknown aspects of the natural course of echinococcus infestation, especially in the brain. The growth rate of the cysts in brain and in other tissues is matter of some discussion. In this report, a huge primary cerebral hydatid cyst associated with a small liver cyst is presented and a possible relationship between the growth rates and the sizes of the two cysts is discussed.

Positive pressure ventilation with the laryngeal mask airway in children.

We studied the safety of positive pressure ventilation (PPV) when using the size 2 laryngeal mask airway (LMA) in 46 ASA physical status I or II children (aged 38 +/- 21 mo) undergoing elective surgery. The LMA cuff was inflated in incremental steps to achieve a cuff leak pressure > or = 15 cm H2O. Abdominal circumference was measured before and after PPV in study patients, as well as in a control group managed with tracheal intubation.

Laparoscopic removal of a gallbladder remnant.

We describe a patient who had an incomplete open cholecystectomy 32 years previously and was hospitalized with jaundice due to a stone in the gallbladder remnant encroaching on the common bile duct. Despite extensive adhesions the gallbladder remnant was removed with laparoscopic techniques and the patient had an uneventful recovery. We conclude that previous surgery, even on the gallbladder, does not preclude the successful completion of a laparoscopic cholecystectomy.

Bejel in Malatya, Turkey.

Bejel is a form of endemic syphilis which is transmitted by non-venereal route. According to our knowledge, no cases of bejel have been reported from Turkey for more than 30 years. In this report, three children and their father who suffer from bejel are presented.

Cholesterol granuloma in the maxillary sinus.

A 35-year-old male complained of recurring headache and nasal obstruction over a 4-year period. A Caldwell-Luc operation was subsequently performed on the left maxillary sinus and a greenish appearing material was removed. Microscopic examination of the tissue specimen showed a granulomatous tissue with typical cholesterol clefts and inflammatory changes consistent with chronic sinusitis.

Intrathecal baclofen: does tolerance occur?

Concern over the development of tolerance in patients on continuous intrathecal baclofen therapy has arisen as this new form of treatment for spasticity has gained wider use. We have studied time-dose relationships in 18 spinal cord injured patients who have undergone intrathecal baclofen infusion pump implantation since February 1988 in our facility. Our data show that there was a significant increase in baclofen dosage needed to control spasticity during the first 12 months post implantation.
