An Interesting Case of Vitamin D-Mediated Severe Hypercalcemia in a Patient With Renal Mass.

Hypercalcemia of malignancy (HCM) is the most common cause of hypercalcemia in hospitalized patients. The pathogenesis of HCM is often multifactorial. One of the rare causes of HCM is extra-renal production of 1,25-dihydroxyvitamin D (or calcitriol), which is often seen in patients with lymphoproliferative malignancies.

N-acetylcysteine prevents cholinergic and non-cholinergic toxic effects induced by nerve agent poisoning in rats.

Objective: Organophosphorus Nerve Agent, VX [(O-Ethyl S-diisopropylaminomethyl) methylphosphonothioate] compound interferes with acetylcholine signaling by targeting the AChE enzyme. Studies suggest that in nerve agents poisoning, non-cholinergic effects are also responsible for damage in peripheral tissues including long term damage in brain. Present study reports cholinergic and non-cholinergic effects of VX poisoning and their prevention by use of N-acetylcysteine (NAC) in addition to conventional antidotes atropine sulphate and 2-PAM chloride as an antioxidant.

Mutagenicity and carcinogenicity prediction of sugar substitutes: an approach with compound-gene interactions network.

Sugar substitutes are mostly artificial, man-made industrial products used as additives in food and beverages. Most of these substances flow through the digestive tract and food chains, becoming emerging organic contaminants in various abiotic and biotic environmental media. Here, we predict the mutagenicity and carcinogenicity of commonly used sugar substitutes using in silico based methods.

Microbial pathogens have developed resistance mechanisms to almost every antibiotic available. There is a need to synthesize or screen new natural compounds to combat the development of drug-resistant pathogens. One of the commonly used methods to evaluate the antimicrobial activity of two or more antibiotics involves a checkerboard assay, which is cumbersome, time-consuming, and expensive.

HybNet: A hybrid deep models for medicinal plant species identification.

Real-time plant species detection plays an important role in fields ranging from medicine to biodiversity conservation. Images captured under unconstrained environments, scale variations, different lighting conditions, leaf orientation, complicated backdrops, and leaflet structure make plant species recognition rigorous and time-consuming. Our study addresses this challenge by introducing three pioneering hybrid models, seamlessly integrating the strengths of convolution neural networks.

Chronic reactive synovitis in patients undergoing ACL reconstruction and augmentation with tape-type sutures.

Introduction: Arthroscopic Anterior Cruciate Ligament Reconstruction (ACLR) with internal bracing and augmentation using tape-type sutures (TTS) has gained popularity due to its biomechanical advantages. However, concerns have emerged regarding chronic reactive synovitis, which can lead to graft failure and the need for revision surgery. The purpose of this research is to determine the prevalence of chronic reactive synovitis after TTS-reinforced ACLR.

Radiomorphometric Parameters in Mandibular Panoramic Radiographs of Hypothyroid Patients: A Cross-Sectional Study.

Purpose: Thyroid hormones have a catabolic effect on bone mineral homeostasis. Hypothyroid patients have shown changes in bone mineral density with increased risk for osteoporosis and bone fractures. Radiomorphometric parameters on panoramic images are good indicators of bone mineral density.

Root canal retreatment (Re-RCT) cases have shortcomings due to the ineffective removal of the root canal filling material, eventually leading to endodontic failure. This study aims to test the comparative efficacy of retreatment files in removing calcium silicate-based sealer and epoxy resin-based sealer. Thirty-two single-rooted teeth were decoronated at 15 mm and bio-mechanical preparation was performed.

Background and objectives The persistent nature of diabetic foot ulcers (DFUs) is mainly attributable to compromised wound healing mechanisms, which are aggravated due to poor blood flow, neuropathy, and infection. Growth factors have become essential agents in the treatment of DFUs, serving as primary mediators that enhance wound healing through the stimulation of cell proliferation, migration, and angiogenesis. This prospective open-label, randomised, comparative, multi-centre, investigator-initiated study compared the safety and effectiveness of adjuvant therapy with topical application of autologous growth factor concentrate (AGFC) using the Healrex therapy kit (Wockhardt, India) versus standard of care (SoC) in DFUs.

Isolated absent pulmonary valve with intact ventricular septum in a young child: A rare case report.

Absent pulmonary valve syndrome, commonly linked with tetralogy of Fallot and ventricular septal defect, is a rare congenital condition. It is exceedingly rare to have an isolated absent pulmonary valve with an intact ventricular septum without cardiovascular shunt lesions, such as an atrial/ventricular septal defect or patent ductus arteriosus. This report presents a case of such rarity involving a young child with recurrent lower respiratory tract infections.

Prevalence of immunological aberrations and 22q11.2 deletion in children with conotruncal anomalies: A cross-sectional study.

Introduction: 22q11.2 deletion is associated with conotruncal anomalies and immunological aberrations. Given the common embryonic origin of conotruncus and thymus, conotruncal anomalies may be associated with immunological aberrations irrespective of 22q11.

Early and midterm results of Cook Formula stent in children with right heart disease: A single-center experience.

Background: Branch pulmonary artery (PA) stenosis must be addressed early to prevent right ventricular scarring and establish lung blood flow. Balloon-mounted stents are more useful in managing right ventricular outflow tract (RVOT) obstruction and PA stenosis.

Materials And Methods: We studied the clinical and angiographic data of children with congenital heart disease who underwent stenting for RVOT obstruction and branch PA stenosis using the Formula stent (Cook Medical, Limerick, Ireland) between 2018 and 2024 in a tertiary pediatric cardiac center in southern India.

Systemic artery aneurysms - A marker of high-risk Kawasaki disease.

Kawasaki disease, traditionally classified as medium vessel vasculitis, is known for the preferential involvement of coronary arteries. Infrequently, large systemic arteries might be involved and may affect the prognosis. Here, we present an infant with Kawasaki disease who had extensive involvement of large vessels such as the abdominal aorta and medium vessels of the extremities, along with giant coronary artery aneurysms.

Successful heart transplantation after 4 years of bridging with HeartWare HVAD for left ventricular noncompaction in an 11-year-old boy with prohibitively high pulmonary vascular resistance.

End-stage heart failure due to left ventricular noncompaction (LVNC) poses unique challenges for ventricular assist device implantation, like inflow cannula obstruction due to trabeculations. We report a case of an 11-year-old boy with LVNC who underwent successful HeartWare implantation as a bridge to transplant for high pulmonary vascular resistance and had successful heart transplantation after 4 years of HeartWare support.

Generalized arterial calcification of infancy - Fetal diagnosis to postnatal management.

Generalized arterial calcification of infancy is a rare entity with poor fetal and postnatal outcomes and high morbidity in survivors. Half of the cases are diagnosed intrauterine due to hemodynamic compromise, and the associated manifestations pose unique management challenges. We hereby report an account of a fetal diagnosis in a referral for hydrops with postnatal evaluation and management.

Diagnosis and management of residual atrial septal defect after surgical failure: A case series.

Residual defect after surgical closure of atrial septal defect is extremely uncommon. This communication reports four cases encountered in a tertiary care center during the last three decades. Clinical diagnosis was challenging, and the diverse presentations included acute ischemic stroke, cyanosis, and right ventricular volume overload.

Persistent left superior vena cava with retrograde flow and absent coronary sinus in a child with ventricular septal defect and patent ductus arteriosus.

Coronary sinus (CS) defects are rare congenital cardiac anomalies that occur in isolation or with other congenital heart diseases. Persistent left superior vena cava (LSVC) is a relatively common entity that usually drains into the CS, is of no hemodynamic consequence, and is easily diagnosed on echocardiography by a dilated CS and an antegrade flow toward the heart. However, a combination of LSVC and CS defect may reverse its flow direction and CS dilation may be absent.

Efficacy of early enteral feeding with supplemented mother's milk on postoperative outcomes of cardiac surgical infants: A randomized controlled trial.

Introduction: Congenital heart disease (CHD)-associated malnutrition is a systemic consequence of CHD. Dietary recommendations to fulfill nutritional requirements are lacking. This randomized controlled trial (RCT) was conducted to determine the efficacy of early enteral feeding with supplemented expressed breast milk (suppl-EBM) versus expressed breast milk (EBM) in improving the weight of postoperative cardiac surgical infants.

Transcatheter salvage of dying Glenn - An innovative strategy for postGlenn thrombosis.

PostGlenn thrombosis is an infrequent, serious complication requiring immediate attention, which is not uncommon in a resource-limited setting where patients are often lost to follow-up and have poor compliance with medications.
