Recurrence of primary spontaneous pneumothorax after surgical treatment was often caused by overlooking bullae in surgical treatments, bullae regrowth, or bullae neogenesis. Herein, we present a very rare type of recurrence after surgical treatment, which was caused by lung laceration next to the adhesion created after the surgery. The patient was a 22-year-old volleyball player, and we presumed that sudden chest wall compression that occurred during volleyball displaced the lung next to the adhesion inwardly and caused the lung laceration.

Neurological surgery requires techniques and equipment to magnify the delicate brain structures and blood vessels that are difficult to discern with the naked eye. The introduction of surgical microscopes in the 1960s marked a breakthrough leading to the development of microsurgery and significant improvements in the safety and efficacy of neurosurgery. Subsequently, minimally invasive neurosurgery became prevalent, particularly with advancements in endoscopic technology in the 1990s.

  • A 57-year-old man came to the hospital after abnormal findings on a chest X-ray, which led to further imaging revealing solid nodules in his lung.
  • Surgery (lobectomy) was done to remove the lung nodule, which was identified as seminoma, a type of testicular cancer.
  • A PET-CT scan indicated high metabolic activity in the lung nodules and slight activity in the left testicle, but no tumor was found on ultrasound, leading to a rare diagnosis of testicular seminoma with single lung metastasis.

Article Synopsis
  • Cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy (CARASIL), now known as HTRA1-related cerebral small-vessel disease (CSVD), is caused by genetic variants in the HTRA1 gene, with recent findings indicating it can also affect heterozygotes, although they present unique challenges in detection and diagnosis.
  • MRI reviews of four cases in two families showed characteristic white matter lesions and dot-like lesions on T2*WI images, which are similar to findings in one known homozygous case, indicating a potential distinguishing feature of heterozygous HTRA1-CSVD.
  • The presence of these dot-like lesions suggests the need for deeper investigation to

Objectives: The influence of lung resection on cardiac function has been reported, and previous studies have mainly focused on right ventricular (RV) dysfunction. As few studies have analyzed changes in left ventricular hemodynamic variables caused by lung resection, we aimed to investigate the perioperative changes in left ventricular stroke volume (LVSV) caused by anatomical lung resection.

Methods: We enrolled 61 patients who underwent anatomical lung resection and perioperative LVSV monitoring.

The superior aortic recess is one of the superior portions of the transverse sinus which is located around the ascending aorta. The fluid collection of the superior aortic recess is sometimes revealed on chest computed tomography, and it becomes more difficult to differentiate from a cystic tumor or lymphadenopathy when the amount of collected fluid is large or the fluid is extended into another area. We report two cases of fluid collection in the superior aortic recess which was misdiagnosed as a cystic mediastinal tumor that underwent surgical resection.

Introduction: Chemoimmunotherapy is widely used as the first-line management of advanced non-small cell lung cancer (NSCLC) in clinical settings. However, predictive factors associated with the development of immune-related adverse events (irAEs) and prognostic factors for NSCLC patients undergoing chemoimmunotherapy remains largely unexplored. Therefore, in this study, we aimed to evaluate predictive factors for irAE development and prognostic factors associated with chemoimmunotherapy in NSCLC patients.

BACKGROUND Various resistance mechanisms of the epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) have been reported, and approximately half of the cases show a T790M point mutation as resistance to EGFR-TKI. In addition, 3-14% of cases of non-small cell lung cancer transform into small cell lung carcinoma (SCLC) during treatment. However, there are few reported cases in which 2 mechanisms of resistance have been observed simultaneously.

Introduction: Combination therapy of anti-programmed cell death 1 (PD-1)/programmed cell death ligand 1 (PD-L1) antibodies and platinum-based chemotherapy has been widely used as a first-line treatment for patients with unresectable advanced non-small cell lung cancer (NSCLC) in clinical settings; however, prognostic biomarkers associated with survival outcomes have not been sufficiently investigated.

Methods: We enrolled 147 previously untreated patients with advanced NSCLC who were treated with a combination therapy of anti-PD-1/-PD-L1 antibodies and platinum-based chemotherapy at eight institutions in Nagano Prefecture between December 2018 and April 2023. We evaluated the prognostic value of the geriatric nutritional risk index (GNRI), a systemic inflammatory nutritional biomarker calculated from body weight and serum albumin level, for patients with NSCLC treated with a combination therapy of anti-PD-1/-PD-L1 antibodies and platinum-based chemotherapy.
