Among patients with negative initial biopsies of the prostate, 51 patients underwent total 59 repeat biopsies at the Department of Urology of Ikeda Municipal Hospital between January 1998 and April 2004. Overall 26 patients (44.1%) were confirmed to have cancer, 22 patients by second repeat biopsy (22/51), four patients by third biopsy (4/7) and none by fourth biopsy (0/1).

Purpose: To evaluate the feasibility of selective thrombolytic therapy for deep venous thrombosis (DVT)by using a small caliber catheter passed through a temporary vena cava filter.

Materials And Methods: During a 3-year period, 14 consecutive symptomatic patients (7 men, 7 women, age range, 35-80 years; mean, 62 years) were referred for treatment. Ten had acute DVT, and four were chronic.

Background: In Japan, generally, patients with chronic hepatitis C are aged. The aim of this study was to investigate the effect of interferon (IFN) therapy on the mortality of chronic hepatitis C patients over age 60.

Methods: Seven-hundred and seven patients with histologically proven chronic hepatitis C were enrolled in this study; 649 received IFN therapy (IFN group) and 58 did not (control group).

[Utility of SPECT in gallium scintigraphy].

Nihon Hoshasen Gijutsu Gakkai Zasshi

November 2002

Department of Radiology, Ikeda Municipal Hospital.

Whole-body gallium planar scintigraphy is a mainstay for the detection of tumors and inflammatory lesions. Recently, gallium SPECT (single photon emission computed tomography) has become more common in the clinical setting. This diagnostic modality is widely employed in our hospital, and lesions are actually detected by SPECT in some cases.

Objective: In patients with non-traumatic osteonecrosis of the femoral head (ONFH), the shoulder is one of the major affected sites secondary to the proximal and distal femur in cases of multiple osteonecrosis. The present study attempted to investigate whether technetium bone scintigraphy is useful for screening of non-traumatic osteonecrosis of the shoulder (ONS).

Design And Patients: A total of 170 shoulder joints in 85 patients with ONFH were evaluated by bone scintigraphy and the findings compared with those of magnetic resonance imaging (MRI).

The patient, an 81-year-old woman, had the chief complaint of macroscopic hematuria. Cystoscopy revealed a bladder tumor, which was determined by biopsy to be mucin-producing adenocarcinoma. Appendiceal carcinoma that invaded the bladder were diagnosed preoperatively by air-contract barium enema and magnetic resonance.

This is a case report of sciatic nerve palsy after total hip arthroplasty. Although the patient's symptoms became worse postoperatively, full recovery occurred after shortening of the calcar and femoral neck length. For acute sciatic nerve palsy patients with worsening of symptoms in the postoperative course in spite of hip and knee flexion, reexposure for early recognition of the sciatic nerve condition and reoperation by shortening the femoral neck may be an option.

We studied the effectiveness of a modular femoral neck system for the adjustment of femoral anteversion, femoral offset, and abductor lever arm during total hip arthroplasty (THA) using computed tomography data, and comparing findings in patients with a modular neck femoral component (116 hips) with those in patients with a cemented femoral component (23 hips). In the modular neck group, we intraoperatively evaluated various impingements using trial necks and heads, and selected the appropriate combination of modular neck and head from two types of straight neck, four types of anteverted/retroverted neck, two types of medialized/lateralized neck, and two types of varus/valgus neck. In the cement group, femoral anteversion was not changed using undersized stems, and an appropriate modular head was selected.

We evaluated the usefulness of 99mTc-methoxy-isobutyl-isonitrile (MIBI) dual phase scintigraphy for detecting hyperfunctioning parathyroid adenoma. We retrospectively reviewed 18 hyperparathyroid patients who received MIBI prior to neck exploration and compared the radiological findings of MIBI with ultrasonography (US) and magnetic resonance imaging (MRI). Fifteen patients were studied with MRI, and 17 patients were examined with US.

The histogenesis of sarcomatoid transformation in hepatocellular carcinomas (HCCs) and cholangiocellular carcinomas (CCCs) remains unclear. In the current case, microsatellite loss of heterozygosity (LOH) assay and mutational analysis of the p53 gene with immunohistochemical examination were performed to investigate the histogenesis of each component. The tumor was composed of two parts: one part comprised poorly differentiated HCC (HCC portion), while the other part included undifferentiated, sarcomatoid HCC and CCC (combined portion).

Hepatocellular carcinomas (HCCs) are thought to develop as well-differentiated tumors and progress to less-differentiated tumors. However, the genetic changes underlying the development and progression of HCCs are not well understood. Recent studies have shown frequent beta-catenin gene activation in HCCs by somatic alterations involving exon 3, resulting in the activation of the Wnt/Wingless signal transduction pathway.
