Advanced ultrasound diagnosis of extrahepatic bile duct lesions.

J Med Ultrason (2001)

October 2024

Department of Gastroenterology, Iida Municipal Hospital, 438 Yawata-Machi, Iida City, Nagano, 395-8502, Japan.

Article Synopsis
  • Ultrasound (US) is the recommended first step for patients suspected of having biliary tract cancer due to its high specificity and sensitivity, but its interpretation can be challenging due to complex anatomy and gas interference.
  • Key techniques to improve visualization of the extrahepatic bile duct (EHBD) include positioning the patient in a left lateral decubitus position and using high-frequency transducers for magnified images.
  • Identifying specific features such as gallbladder distension and bile duct wall characteristics is crucial for diagnosing conditions like cholangiocarcinoma (CCA), with further evaluation through contrast-enhanced US or biopsies necessary when CCA is suspected.

Article Synopsis
  • The study aimed to identify factors affecting visual outcomes after vitrectomy in patients with rhegmatogenous retinal detachment and foveal detachment, focusing on pre-, intra-, and postoperative variables.
  • A retrospective analysis of 33 eyes revealed that preoperative total retinal detachment was linked to worse visual acuity, while intraoperative membrane peeling and delayed recognition of visual dysfunction contributed to higher metamorphopsia scores.
  • The study concluded that best-corrected visual acuity, metamorphopsia, and retinal sensitivity one year post-surgery are influenced by different factors, highlighting the complexity of postoperative recovery.

Introduction: Chemoimmunotherapy is widely used as the first-line management of advanced non-small cell lung cancer (NSCLC) in clinical settings. However, predictive factors associated with the development of immune-related adverse events (irAEs) and prognostic factors for NSCLC patients undergoing chemoimmunotherapy remains largely unexplored. Therefore, in this study, we aimed to evaluate predictive factors for irAE development and prognostic factors associated with chemoimmunotherapy in NSCLC patients.

Purpose: This study aims to evaluate and identify magnetic resonance (MR) findings of mural nodules to detect squamous cell carcinoma arising from ovarian mature cystic teratoma (SCC-MCT).

Methods: This retrospective study examined 135 patients (SCC-MCTs, n = 12; and benign MCTs, n = 123) with confirmed diagnoses across five different institutions between January 2010 and June 2022. Preoperative MR images for each patient were independently assessed by two experienced radiologists and analyzed following previously reported findings (PRFs): age, tumor size, presence of mural nodules, size of mural nodule, and the angle between mural nodule and cyst wall (acute or obtuse).

Introduction: Combination therapy of anti-programmed cell death 1 (PD-1)/programmed cell death ligand 1 (PD-L1) antibodies and platinum-based chemotherapy has been widely used as a first-line treatment for patients with unresectable advanced non-small cell lung cancer (NSCLC) in clinical settings; however, prognostic biomarkers associated with survival outcomes have not been sufficiently investigated.

Methods: We enrolled 147 previously untreated patients with advanced NSCLC who were treated with a combination therapy of anti-PD-1/-PD-L1 antibodies and platinum-based chemotherapy at eight institutions in Nagano Prefecture between December 2018 and April 2023. We evaluated the prognostic value of the geriatric nutritional risk index (GNRI), a systemic inflammatory nutritional biomarker calculated from body weight and serum albumin level, for patients with NSCLC treated with a combination therapy of anti-PD-1/-PD-L1 antibodies and platinum-based chemotherapy.

Background: Rechallenge therapy with epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI) is known to confer some clinical benefit for patients with metastatic EGFR-mutated non-small cell lung cancer (NSCLC). However, little is known about the efficacy of EGFR-TKI rechallenge after resistance to first-line (1L) osimertinib. This study aimed to assess the efficacy and safety of EGFR-TKI rechallenge therapy after resistance to 1L osimertinib in a Japanese clinical setting.

Two-stage surgery may be necessary when total tumor removal cannot be accomplished in the first surgery; however, the extent and condition in which the remaining tumor should be before the next surgery have not yet been established. There is a risk of postoperative hemorrhage in the residual tumor, especially in hypervascular tumors. We report a case of hypervascular choroid plexus papilloma (CPP) in a 22-year-old male patient where the preservation of intratumoral venous drainage was considered important to avoid hemorrhagic complications during a two-stage surgery.

Article Synopsis
  • Lobular endocervical glandular hyperplasia (LEGH) is a benign lesion but can lead to gastric-type adenocarcinoma, highlighting the need for early diagnosis using specialized staining techniques and assays.
  • Researchers analyzed the nuclei of LEGH and endocervical (EC) cells using Gabor filter features to distinguish between them, achieving high accuracy with a linear support vector machine (LSVM).
  • Results showed that LEGH cell nuclei have milder gradient features compared to EC cell nuclei, suggesting a clear morphological distinction that aids in their classification.

Background: Epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) have revolutionized the treatment of advanced non-small cell lung cancer (NSCLC) and contributed to the development of precision medicine. Osimertinib is a standard first-line (1L) treatment for -mutated NSCLC and has demonstrated superior survival benefits over previous-generation TKIs. However, resistance to osimertinib is nearly inevitable, and subsequent treatment strategies remain unmet medical needs in this setting.
