From April 2024, manufacturing of short-shaft ultrasonic scalpels( SNGHK) has ceased. A scalpel for laparoscopic surgery (HDH05) remains available;however, its long shaft is unsuitable for internal thoracic artery harvesting through a median sternotomy. We contrived to make the latter device easier to handle using a method that requires no specialized parts.

Article Synopsis
  • Hospital-associated disability (HAD) is a decline in the ability to perform daily activities during hospitalization, particularly affecting older adults. This study investigates how inpatient cardiac rehabilitation (CR) impacts the development of HAD in older patients undergoing transcatheter aortic valve implantation (TAVI).
  • Out of nearly 20,000 patients studied, those who participated in CR had a significantly lower prevalence of HAD (8.8%) compared to non-participants (14.2%), suggesting CR is beneficial in preventing this decline.
  • However, the positive effects were less pronounced in patients with low body mass index (BMI) or lower Barthel Index (BI) scores at admission, indicating some patients might not benefit as much from CR.

Article Synopsis
  • The study explores how regional factors affect participation in cardiac rehabilitation (CR) and includes a large cohort of 2.7 million inpatients in Japan from 2012 to 2021.
  • It categorizes hospitals into No-CR, High-CR, and Low-CR based on participation rates, finding that High-CR hospitals tend to have smaller service areas, more physicians per population, higher temperatures, and fewer primary industry workers.
  • Key regional characteristics influencing High-CR hospitals include a lower consumer price index and a tendency for a younger population, thus highlighting the importance of local economic and demographic factors in CR participation.

Repair of mitral paravalvular leak using left atrial appendage tissue.

Interdiscip Cardiovasc Thorac Surg

October 2024

Department of Cardiovascular Surgery, Ichinomiya Municipal Hospital, Ichinomiya, Japan.

Paravalvular leak after mitral valve replacement causes serious symptoms such as heart failure and haemolysis. However, whether re-replacement or direct leak site repair is the appropriate surgical treatment for this condition remains controversial. Herein, we describe a case of paravalvular leak repaired using left atrial appendage tissue with excellent results.

Article Synopsis
  • Most patients with essential thrombocythemia (ET) have mutations in JAK2, CALR, or MPL genes, which can lead to additional mutations when transforming into acute myeloid leukemia (AML).
  • In a study of paired samples from eight patients, researchers found that some JAK2-unmutated clones proliferated at AML transformation, even when JAK2-mutated clones were dominant in ET.
  • The findings suggest the existence of common initial clones in ET that can lead to transformed AML, highlighting the importance of further research on specific mutations like SMARCC2, UBR4, and ZNF143.

We previously reported the Marimo cell line, which was established from the bone marrow cells of a patient with essential thrombocythemia (ET) at the last stage after transformation to acute myeloid leukemia (AML). This cell line is widely used for the biological analysis of ET because it harbors mutation. However, genetic processes during disease progression in the original patient were not analyzed.

We report two case series in which we encountered the difficulties to retrieve disrupted devices during percutaneous coronary intervention and endovascular treatment. The broken devices were successfully and safely retrieved by the "flower technique" that creates multiple cut and slit in the tip of the guiding catheter. This technique can be easily and safely applied in situations where a balloon catheter or stent has been disrupted and device entrapment has occurred, as the technique simply involves manually placing multiple cuts or slits in the tip of the guiding catheter.

In Japan, systemic chemotherapy is the standard treatment for unresectable, advanced, or recurrent gastric cancer. However, numerous patients with gastric cancer do not receive late-line treatment because of the rapid progression of gastric cancer. Additionally, late-line treatments, such as nivolumab, trifluridine tipiracil (FTD/TPI), or irinotecan, have limited effects on improving clinical symptoms and delaying the onset of symptoms associated with cancer progression.
