Background: Limited large-scale, real-world data exist on the prevalence and clinical impact of discordance between fractional flow reserve (FFR) and nonhyperemic pressure ratios (NHPRs).

Methods: The J-PRIDE registry (Clinical Outcomes of Japanese Patients With Coronary Artery Disease Assessed by Resting Indices and Fractional Flow Reserve: A Prospective Multicenter Registry) prospectively enrolled 4304 lesions in 3200 patients from 20 Japanese centers. The lesions were classified into FFR+/NHPR-, FFR-/NHPR+, FFR+/NHPR+, or FFR-/NHPR groups according to cutoff values of 0.

Real-world data on venous thromboembolism (VTE) in Japanese patients with gynecological cancer are lacking. The GOTIC-VTE trial aimed to evaluate the frequency of VTE-associated events and risk factors at the time of cancer diagnosis and during 1-year follow-up. From July 2017 to February 2019, patients with endometrial, cervical, ovarian, tubal, or peritoneal cancer who underwent VTE screening within 2 months before registration, were enrolled.

Background: This study aimed to investigate prognostic factors for predicting the survival of patients with extensive-disease-stage small-cell lung cancer treated with chemoimmunotherapy.

Methods: Patients were classified according to overall survival (OS): favorable corresponded to an OS ≥ 24 months, moderate corresponded to an OS of 6-24 months, and poor corresponded to an OS < 6 months. Multivariate Cox regression analyses were used to evaluate prognostic factors.

Background: Cerebrospinal fluid (CSF) levels of orexin show a cyclic diurnal variation in healthy subjects, which is diminished in patients with certain diseases. However, possible circadian variations in orexin levels in critically ill patients remain unknown. In this study, we evaluated the orexin concentrations in the CSF and their diurnal variation in patients undergoing thoracic aortic aneurysm repair with lumbar intrathecal catheterization for CSF drainage after non-neurosurgery.

Background: While advanced gastrointestinal stromal tumors (GISTs) are primarily treated with tyrosine kinase inhibitors (TKIs), acquired resistance from specific mutations in KIT or PDGFRA frequently occurs. We aimed to assess the utility of circulating tumor DNA (ctDNA) as a modality of therapeutic decision-making in advanced GIST.

Methods: We conducted a pooled analysis of SCRUM-Japan studies for advanced GIST patients.

Article Synopsis
  • Concerns about pulmonary regurgitation after transannular patch repair (TAP) for Tetralogy of Fallot remain, leading researchers to compare various right ventricular outlet tract reconstruction techniques.
  • A comprehensive search identified 34 studies involving nearly 8,900 patients, focusing on outcomes like mid-term pulmonary regurgitation and postoperative mortality.
  • Results showed TAP had a higher risk of pulmonary regurgitation compared to techniques like TAP with neo-valve creation (TAPN) and valve-sparing (VS), with VS also linked to lower postoperative mortality and reduced ventilation time.

Article Synopsis
  • This study analyzed 389 patients aged 65 and older with cervical spinal cord injury (CSCI) but no bone injury to assess the effects of diabetes on neurological recovery.
  • The research found that there were no significant differences in neurological outcomes between diabetic patients and those without diabetes at 6 months post-injury.
  • Additionally, moderate-severe diabetes did not adversely impact recovery, suggesting that diabetic patients had similar improvements in neurological function as their nondiabetic counterparts.

  • Left ventricular noncompaction (LVNC) is a hereditary heart condition marked by unusual heart muscle structure, and this study specifically focused on biventricular noncompaction (BiVNC) in children to understand its clinical characteristics and genetic factors.
  • The research involved 234 pediatric patients and revealed that BiVNC often leads to serious complications, including a higher incidence of congenital heart disease and reduced survival rates compared to other heart conditions.
  • Findings indicated that patients with BiVNC frequently exhibited left ventricular dysfunction and a notable percentage had genetic variants linked to mitochondrial and developmental issues, emphasizing the need for thorough genetic screening for better patient outcomes.

Physical activity and renal outcome in diabetic and non-diabetic patients with chronic kidney disease stage G3b to G5.

Sci Rep

November 2024

Department of Nephrology, Faculty of Medicine, Institute of Medicine, University of Tsukuba, 1-1-1, Tennodai, Tsukuba, Ibaraki, 305-8575, Japan.

Article Synopsis
  • The study investigates the link between physical activity and outcomes in advanced chronic kidney disease (CKD) patients in Japan, focusing on those with eGFR < 45 ml/min.
  • Results show that active patients in CKD stage G3b have a lower risk of renal decline, while physical activity is also associated with reduced mortality, particularly for non-diabetic patients across stages G3b to G5.
  • Overall, the findings indicate that exercise can improve survival and renal health in advanced CKD, emphasizing its importance for both diabetic and non-diabetic patients, especially those in earlier stages.

Article Synopsis
  • * A nationwide survey tracked data from 70 pregnancies in 49 CML patients between 2002 and 2020, highlighting three pregnancy types: CML onset during pregnancy, unplanned pregnancies during TKI treatment, and planned pregnancies during treatment-free remission (TFR) or using interferon-alpha (IFN-α).
  • * The study found that planned pregnancies had a longer time from CML diagnosis compared to unplanned ones, and TFR or IFN-α were deemed safe pregnancy options—indicating the need for careful planning for pregnancies in women

Background: One of the most important and relatively frequent complications of aortic valve replacement is atrioventricular block. It typically occurs by direct injury of the infranodal conduction system due to intra-operative manipulation and persists post-operatively, necessitating permanent pacemaker implantation in many cases.

Case Summary: A 66-year-old man presented to our hospital after experiencing syncope while walking after drinking.

Patient respiration is characterized by respiratory parameters, such as cycle, amplitude, and baseline drift. In treatment planning using four-dimensional computed tomography (4DCT) images, the target dose may be affected by variations in image reconstruction techniques and respiratory parameters. This study aimed to optimize 4DCT image reconstruction techniques for the treatment planning of lung stereotactic body radiotherapy (SBRT) based on respiratory parameters using respiratory motion phantom.

Cost-utility analysis of proton beam therapy for locally advanced esophageal cancer in Japan.

PLoS One

September 2024

Department of Radiation Oncology & Proton Medical Research Center, Faculty of Medicine, University of Tsukuba, Tsukuba, Ibaraki, Japan.

Purpose: Proton beam therapy (PBT) has recently been included in Japan's health insurance benefit package for certain cancer types. This study aimed to determine the cost-effectiveness of PBT as a replacement for conventional three-dimensional conformal radiotherapy (3D-CRT) for locally advanced esophageal cancer (LAEC) that is not covered by social insurance.

Methods: We estimated the incremental cost-effectiveness ratio (ICER) of PBT as a replacement for 3D-CRT, using clinical evidence from the literature and expert opinions.

Article Synopsis
  • Recent studies have shown that higher levels of blood calciprotein particles (CPP) are linked to vascular calcification and cardiovascular issues in patients on hemodialysis, despite varying levels among individuals with similar phosphorus levels.
  • This study examined whether the ratio of serum CPP to serum phosphate (CPP/Pi ratio) could better predict cardiovascular events compared to traditional calcification tests (T) in 174 new hemodialysis patients.
  • Results indicated that the CPP/Pi ratio was a significant independent predictor of major heart and brain-related events, whereas traditional calcification propensity tests did not show a similar association.
