Multidimensional Results and Reflections on CAR-T: The Italian Evidence.

Int J Environ Res Public Health

February 2023

Centre for Research on Health Economics, Social and Health Care Management, LIUC-Università Cattaneo, 21053 Castellanza, Italy.

The present study aims at defining the economic and organizational impacts of the introduction of chimeric antigen receptor T-cell therapy (CAR-T) in Italy, for the management of diffuse large B-cell lymphoma (DLBCL) patients in third-line therapy, defining the overall level of sustainability for both hospitals and the National Healthcare System (NHS). The analysis focused on CAR-T and Best Salvage Care (in the following BSC), assuming the Italian hospital and NHS perspectives, over a 36-month time horizon. Process mapping and activity-based costing methodologies were applied to collect the hospital costs related to the BSC and CAR-T pathways, including adverse event management.

Article Synopsis
  • - The study analyzes the outcomes of surgical resection for cerebellar brain metastases in elderly patients (≥65 years old), focusing on 48 individuals with an average age of 70.8, and highlights the challenges of surgery due to their chronic health issues.
  • - Results indicated that complications were common, with 29% experiencing perioperative neurological and systemic issues, but 71% of patients could continue with additional treatments like radiation or chemotherapy post-surgery.
  • - The findings suggest that while selected elderly patients may benefit from surgery, especially those with better pre-operative health indicators, a higher rate of complications and lower overall survival rates necessitate careful consideration before proceeding.

Article Synopsis
  • Metastatic lymph node involvement is a crucial factor affecting survival outcomes in head and neck squamous cell carcinomas (HNSCCs), particularly impacting overall and disease-specific survival rates.
  • Laryngeal and hypopharyngeal cancers have a high likelihood of spreading to the central neck compartment (CNC), yet the prevalence and implications of this involvement are not well understood, leading to ongoing debates about hidden metastasis rates.
  • The review aims to clarify current management strategies for CNC involvement in these cancers, address evidence gaps and inconsistencies, and provide an updated anatomical and surgical overview of the CNC.

Background: WHO grade II and III meningiomas are more invasive than grade I malignancies and determine patients' shorter overall survival. Their tendency to recur after treatment has represented an important therapeutic challenge because of the limited treatment strategies at recurrence. Angiogenesis and mechanistic target of rapamycin (mTOR) activation are two of the main features of higher grade meningiomas, determining invasiveness and tendency to relapse.

Background: the tight and reciprocal interaction between cancer and hemostasis has stimulated investigations on the possible role of hemostatic biomarkers in predicting specific cancer outcomes, such as disease progression (DP) and overall survival (OS). In a prospective cohort of newly diagnosed metastatic gastrointestinal (GI) cancer patients from the HYPERCAN study, we aimed to assess whether the hemostatic biomarker levels measured before starting any anticancer therapy may specifically predict for 6-months DP (6m-DP) and for 1-year OS (1y OS). Methods: plasma samples were collected and tested for thrombin generation (TG) as global hemostatic assay, and for D-dimer, fibrinogen, and prothrombin fragment 1 + 2 as hypercoagulation biomarkers.

Introduction: Bone metastases (BMs) are a negative prognostic factor in patients with non-small cell lung cancer (NSCLC). Although immune-checkpoint inhibitors (ICIs) have dramatically changed the therapeutic landscape of NSCLC, little information is available on BMs from NSCLC treated with ICIs alone or in association with bone-targeted therapy (BTT) such as zoledronate or denosumab.

Methods: From 2014 to 2020, 111 of the 142 patients with BMs secondary to NSCLC extrapolated from the prospective multicenter Italian BM Database were eligible for analysis.
