[Ten questions about myocardial infarction with non-obstructed coronary arteries].

G Ital Cardiol (Rome)

April 2024

U.O. Cardiologia, IRCCS Azienda Ospedaliera-Universitaria di Bologna, Dipartimento di Scienze Mediche e Chirurgiche - DIMEC, Alma Mater Studiorum, Università degli Studi, Bologna.

The term "MINOCA" refers to myocardial infarction with non-obstructed coronary arteries and has been used to define acute myocardial infarction with no angiographic evidence of significant epicardial coronary artery stenosis. Patients with MINOCA represent a rather heterogeneous group of acute coronary syndrome, and this may account for the wide variation in the incidence of MINOCA in different studies. Several pathogenic mechanisms have been suggested to underlie MINOCA, but the condition continues to represent a diagnostic and therapeutic challenge for the cardiologist.

Circ Cardiovasc Imaging

March 2024

Cardiology Unit, Cardiac Thoracic and Vascular Department, IRCCS Azienda Ospedaliera-Universitaria di Bologna (L.B., F.A., A.F., L.C., D.F., M.A., A.S., F.B., S.A., N.S., D.A., F.N., L.L., C.P.).

Background: Multimodality imaging is currently suggested for the noninvasive diagnosis of cardiac masses. The identification of cardiac masses' malignant nature is essential to guide proper treatment. We aimed to develop a cardiac magnetic resonance (CMR)-derived model including mass localization, morphology, and tissue characterization to predict malignancy (with histology as gold standard), to compare its accuracy versus the diagnostic echocardiographic mass score, and to evaluate its prognostic ability.

Division of Cardiology, Cardiocentro Ticino Institute, Ente Ospedaliero Cantonale, Lugano, Switzerland; Center for Cardiac Magnetic Resonance of the CHUV (CRMC), Lausanne University Hospital, Lausanne, Switzerland. Electronic address:

Article Synopsis
  • This study investigates the role of T2 values in the noninfarcted myocardium (NIM) after a heart attack (STEMI) to better understand tissue-level inflammation and its implications for patient outcomes.
  • The research involved 171 patients and found that higher T2 values in the NIM were linked to larger heart muscle damage, poor heart function, and did not correlate with typical inflammatory markers.
  • Patients with elevated NIM T2 values had a significantly higher risk of major cardiovascular events, particularly reinfarction, highlighting the potential of T2 mapping as a prognostic tool post-STEMI.

The use of subcutaneous ICDs (S-ICD) is growing over years despite increasing alerts on premature battery depletion (PBD) and lead fractures leading to unanticipated device replacements. In our single-centre study including 192 patients, per year analysis demonstrated that incidence of PBD is higher than previously reported with overall greatest battery replacement requirements around the fifth year of follow-up. The underlying issue appears to be limited to old series devices, but only a longer follow-up will clarify the real impact of this phenomenon on patient outcomes.
