Article Synopsis
  • Pneumomediastinum, often occurring silently post-trauma, complicates diagnoses especially when paired with other conditions like pneumothorax and pneumoperitoneum, largely due to the Macklin effect where air escapes from ruptured alveoli.
  • An 18-year-old male involved in a car crash was found with severe pneumothorax and subcutaneous emphysema but showed no signs of significant esophageal or bronchial injury upon further evaluation.
  • Despite alarming initial imaging, the patient's treatment was conservative, allowing for a gradual healing process and eventual discharge without complications, illustrating the need for careful assessment in trauma management.

Purpose: Non-contact anterior cruciate ligament (ACL) injuries are influenced by the anatomic and biomechanical characteristics of the lower limb. The combination of knee valgus, hip internal rotation and tibial external rotation are important contributors to non-contact ACL injuries. In this study, we aimed to evaluate the effect of femoral anteversion on the incidence of ACL injuries among athletes.

Volar Plate Draping (VPD) for Severely Comminuted Proximal Phalanx Head Fractures.

Indian J Plast Surg

December 2024

The Department of Plastic, Hand and Reconstructive Microsurgery, Ganga Medical Center and Hospitals, Coimbatore, Tamil Nadu, India.

While proximal phalangeal joint injuries with comminution of the base of the middle phalanx are common injuries, proximal interphalangeal (PIP) joint fracture dislocations with an intact base of middle phalanx and a comminuted head of proximal phalanx are rare. Volar plate arthroplasty and other described techniques prevail for the former injury, while the latter does not have any supportive literature on the exact method of management. We herein present a 20-year-old male with a severely comminuted head of proximal phalanx fracture with dislocation of the PIP joint, which was not reconstructable but was managed successfully with a novel technique of volar plate draping that resurfaced the raw phalangeal head.

Penile Reconstruction with Radial Forearm Free Flap-Present State of the Art.

Indian J Plast Surg

December 2024

Department of Plastic and Reconstructive Surgery, Amrita Hospital, Faridabad, Haryana, India.

Article Synopsis
  • Patients with penile defects, whether congenital or acquired, experience significant psychological distress, and the free radial artery forearm flap method has emerged as a preferred technique for reconstruction.
  • A retrospective study over 16 years reviewed various cases, modifying the initial design to enhance urethral function and stability in the procedure.
  • Results indicated that most patients regained sensation and could urinate standing; however, some experienced complications, and aesthetic outcomes varied, highlighting the technique's potential for effective rehabilitation.

Introduction: There is a risk of iatrogenic vascular injuries during robotic-assisted laparoscopic excision of diaphragmatic endometriosis. Although studies are limited, the first reported case of a suprahepatic inferior vena cava (IVC) injury during robotic diaphragmatic endometriosis excision was successfully treated using a fibrin sealant patch, preventing exsanguination and conversion to laparotomy.

Case Description: A 36-year-old female with a history of recurrent catamenial pneumothorax and two prior video-assisted thoracoscopic surgeries to treat diaphragmatic endometriosis presented to our clinic with right-sided shoulder pain and a chest tube in place.

Pre-hospital Associated Factors of Survival in Traumatic Out-of-hospital Cardiac Arrests: An 11-Year Retrospective Cohort Study.

Arch Acad Emerg Med

December 2024

Department of Emergency Medicine, Faculty of Medicine, Ramathibodi Hospital, Mahidol University, Bangkok 10400, Thailand.

Article Synopsis
  • Traumatic out-of-hospital cardiac arrest (TOHCA) is a significant public health issue in Thailand, heavily influenced by high accident rates and inconsistent prehospital care.
  • A study analyzed data from the Emergency Medicine System over 11 years, finding that of 35,724 TOHCA patients, about 18.5% survived to hospital admission, with key factors influencing survival including stopping bleeding, intubation, and defibrillation.
  • The research suggests that improving prehospital management—such as fluid administration and timely intervention—can enhance survival rates for TOHCA victims.

  • The study assesses the return to performance of MLB pitchers after undergoing ulnar collateral ligament (UCL) surgery using advanced analytics and pitch-tracking metrics.
  • At 1, 2, and 3 years post-surgery, the data showed that only 1.6% of pitchers returned to play within the first year, but the rates increased significantly to 71.9% at 2 years and 82.0% at 3 years.
  • The research highlights the importance of modern metrics, examining factors like expected fielding independent pitching and velocity, to evaluate pitchers' recovery and performance levels after surgery.

Article Synopsis
  • Acetabular chondrolabral delamination (ACD) is a prevalent hip injury with few effective treatments; this study explored the use of PLGA conical nail fixation as a potential method.
  • The experiment involved 24 pigs with surgically induced ACD, divided into three groups: a control (no treatment), an acute fixation group using PLGA nails, and a chronic fixation group using spacers before PLGA nails.
  • Results indicated that the pigs in the acute fixation group showed significantly better cartilage regeneration and proteoglycan deposition compared to the control and chronic groups, suggesting PLGA nails are effective for treating ACD.

Background: Benign bone lesions are a common incidental finding in athletes during workup for musculoskeletal complaints, and athletes are frequently advised to halt participation in contact sports. There are no current guidelines to assist clinicians in referring patients with these lesions to a subspecialist or in advising athletes on the safety of returning to sport.

Purpose: To assist sports medicine physicians in appropriate referral for patients with benign bone lesions through presentation of a literature review and the case of an adolescent athlete with a benign bone lesion in a location with a significant fracture risk.

Background: Anterior labroligamentous periosteal sleeve avulsion (ALPSA) lesions are associated with recurrent shoulder instability and higher rates of failure after capsulolabral repair compared with similarly treated Bankart lesions. Although these lesions can portend poor outcomes, there are limited data on the associated conditions and postoperative course in a young, active population.

Purpose: To evaluate the mid- to long-term clinical course and failure rates after ALPSA repairs and assess features associated with these outcomes.

  • Regular investigations into low back injuries (LBIs) in NCAA sports are essential for understanding their causes and improving prevention and rehabilitation efforts.
  • The study analyzed data from the NCAA Injury Surveillance Program over a 10-year period, identifying higher injury rates in women's gymnastics and men's tennis, with significant differences in injury rates between competition and practice events.
  • Findings revealed that chronic and recurrent LBIs were more common in women's sports, while overall LBI rates were comparable between men's and women's sports; injuries were generally linked to noncontact and overuse incidents.
