[Metastatic basal cell carcinoma with a SMO gene mutation. A case report].

The authors describe a clinical case of a patient with metastatic basal cell carcinoma and secondary resistance to vismodegib. A mutation of unknown clinical significance was found in the gene encoding the transmembrane receptor protein Smoothened (SMO) protein, which suggests a defect in the Sonic Hedgehog (SH) pathway and may cause tumor resistance to the prescribed drug. Comprehensive genomic profiling and analysis have been used to diagnose the tumor.

[The organization of pre-clinical studies of bactericidal and wound healing effects of the impulse photoherapy device "Zarya"].

The purpose of the study is to determine therapeutic effectiveness of the pulsed high intensity optical irradiation device "Zarya" exemplified by treatment of model wounds in laboratory animals and to compare with traditional methods of wound treatment. The prototype of "Zarya" device was used whose operating principle was based on pulsed irradiation of affected areas with high intensity optical radiation in continuous spectrum generated by pulsed xenon lamp. The therapeutic effect of the "Zarya" device was compared with effectiveness of the certified medical ultraviolet irradiator based on low-pressure mercury lamp and also with known wound-healing and antibacterial medication Levomekol ointment.

[The medical social problems and methods of diagnostic of cognitive disorders in patients taking "Spice"].

The article presents the results of pilot study implemented to determine cognitive disorders in patients taking "Spice". The purpose of research is to identify targets of psychological correction interventions to prevent cognitive deficiency in patients addicted to synthetic cannabinoids. The pilot sampling consisted of 30 patients taking "Spice".

[The psycho-physiological aspects of morbidity of cadets of military educational institutions].

The purpose of the study is to determine and to estimate the psycho-physiological factors affecting morbidity of cadets of military educational institutions. The study was carried put using sampling of 79 male cadets of the Academy of the Federal Penal Service of Russia. The cluster analysis was applied to indices of morbidity and psychological characteristics of cadets.

[Effect of an antioxidant on vascular wall cell apoptosis markers after reconstructive operations].

Aim: This study was aimed at determining Bcl-2 and Bax proteins expression before and after reconstructive-repairing operations in patients with atherosclerosis obliterans of lower extremities and at assessing the effect of an antioxidant (vitamin E at a dose of 100 mg once daily for 1 month after surgery) on the dynamics of changes of Bcl-2 and Bax proteins in the postoperative period.

Patients And Methods: The study included a total of 60 patients with stage III-IV lower limb atherosclerosis obliterans. All patients underwent reconstructive-repairing operations on the arteries of the aortofemoral segment.

[The modification of predictors of course of acute period of craniocerebral trauma under implementation of stream model of medical care support].

The specificity of course of acute period of craniocerebral injury and organization of medical care support are the factors determining outcomes for this category of patients. The purpose of the study is to investigate changes in predictors of course of acute period course of craniocerebral injury under implementation of stream model of medical care organization. The sampling included 150 patients with moderate and severe craniocerebral injury based on data obtained in 2013 and 2019, respectively.

The results of survey of psychiatrists and narcologists are presented.

[Carnitine chloride reduces the severity of experimental hyperhomocysteinemia and promotes lactate utilization by mitochondrial fraction of the rat epididymis].

Hyperhomocysteinemia is a risk factor for many diseases, including reproductive disorders in men. L-carnitine is used in medical practice to correct impaired bioenergetic conditions; in patients with idiopathic forms of infertility its effects are associated with improvement of the sperm parameters. However, the effect of exogenous L-carnitine on the level of homocysteine in the gonadal tissues, as a risk factor for impaired fertility, has not been investigated yet.

[The experience of SERVQUAL technique application in measuring satisfaction of patients with medical services quality].

The implementation of system of quality control conforming to requirements of interstate ISO 9001 standards in in the medical organization, the important role is assigned to indicator of satisfaction of patients with quality of medical care as a feedback channel. The study purpose was to investigate satisfaction of patients with quality of medical services in medical organization rendering hospital medical care on the basis of SERVQUAL technique. The analysis of results of survey of 339 patients treated in the twenty-four-hour department of municipal hospital demonstrated rather high rating of perception of quality of rendered medical services that made up to 4.

[The analysis of failures of intra-familial relationships in families with children suffering of diabetes mellitus].

The article presents the results of study of in-family (child-parents) relationships in families with children suffering with diagnosed diabetes mellitus type I. The purpose of study is to investigate the disorders of family relationships in families of such kind. The established disorders allow to develop psychotherapeutic measures both for children and their parents.

[Markers of the course of obliterating atherosclerosis of lower limb arteries].

Aim: The purpose of the study was to determine the level of substances damaging the vascular endothelium, as well as to assess their effect on the functional state of the endothelium and the course of obliterating atherosclerosis of lower limb arteries.

Patients And Methods: The study included a total of 112 people, subdivided into three groups: those with an unfavourable course of obliterating atherosclerosis of lower limbs arteries (n=48) - group 1, patients with obliterating atherosclerosis of lower limbs arteries with a conventionally favourable course (n=48) - group 2, and apparently healthy volunteers (n=16). In all subjects, the following parameters were analysed: stable metabolites of nitric oxide II, endothelin-1, homocysteine and basal insulin.

[Clinical anatomy of deep femoral vessels in the area of femoral triangle].

Aim: The purpose of this study was to specify the anatomy of the deep femoral artery and deep femoral vein within the femoral triangle.

Material And Methods: The study was based on the data of anatomical dissection of vessels in the area of the upper third of the femur (20 specimens ) and ultrasonographic duplex angioscanning of patients undergoing routine examination of the vascular system (40 patients, 50 lower extremities). Ultrasonography was performed using linear and convex transducers (frequency 3-13 and 3-5 MHz).

The article presents the results of questionnaire survey of 412 patients with chronic dermatitis residing in Moscow. The purpose of study was to evaluate awareness of patients with chronic dermatitis and residing in the capital of Russia about medical services provided and their rights to receive them.

[The associations of indices of echocardiography with polymorphism of genes of angiotensinogen and angiotensin receptor type I in examined patients with chronic rheumatic heart disease].

The purpose of study is to evaluate associations of polymorphism of genes encoding components of renin-angiotensin system and indices of echocardiography in examined patients with rheumatic heart disease (RHD). The sampling consisted of 128 patients with RHD and average age 58,96±0,34 years. The echocardiography was implemented using Philips Affinity 50 machine.

The data from the Russian part of the European registry for the management of Helicobacter pylori infection (European Registry on the management of Helicobacter pylori infection, protocol: "Hp-EuReg") allows us to analyze the real clinical practice of diagnosis and treatment of H. pylori and compare it with international recommendations.
