Amyloid light chain (AL) amyloidosis is a disease in which ALs, which are proteins with fibrous structures, are deposited in systemic organs, causing functional impairment. Diagnosis is often difficult because of non-specific and varied symptoms. We report a case of systemic AL amyloidosis that was diagnosed as a result of repeated syncope.

Article Synopsis
  • - The study focuses on the increasing detection of wild-type transthyretin (ATTR) amyloidosis and evaluates the effectiveness of Tc-labeled pyrophosphate (Tc-PYP) scintigraphy for diagnosing ATTR cardiac amyloidosis in patients across Japan from 2018 to 2022.
  • - Out of 180 patients, 135 were analyzed, with 62 testing positive for Tc-PYP; researchers developed a scoring system based on five clinical factors to predict positivity, revealing that a higher score correlates with a greater likelihood of positive results.
  • - The study concludes that combining commonly available clinical data can significantly enhance the pretest prediction accuracy for Tc-PYP scintigraphy, aiding clinicians in the early diagnosis of ATTR amyloidosis

Article Synopsis
  • The study aimed to develop guidelines for antibiotic use during tooth extractions in cancer patients on denosumab, as there's a risk of medication-related osteonecrosis of the jaw (MRONJ).
  • It analyzed 68 cancer patients' antibiotic administration practices and their correlation with MRONJ after tooth extractions over a 10-year period.
  • Results showed a lot of inconsistency in antibiotic usage among hospitals, with amoxicillin being the most common; however, no significant link between MRONJ occurrence and the dosage or duration of amoxicillin was identified, suggesting a single preoperative dose may be sufficient.

Because the β-lactam ring has a molecular structure similar to that of gamma-aminobutyric acid (GABA) neurotransmitters, it binds to GABA A receptors and inhibits GABAergic transmission, causing AAE. The possibility of antimicrobial-associated encephalopathy should be considered in cases of neurological or psychiatric symptoms after initiating an antimicrobial regimen.

  • A study investigated the effects of continuing steroid therapy beyond 3 years for patients with autoimmune pancreatitis who maintained remission during an initial 3-year period.
  • Out of 211 patients, those who continued steroid therapy had a lower relapse rate compared to those who stopped, but they also faced a higher risk of developing macrovascular complications.
  • The findings suggest that while ongoing steroid use may prevent relapse, it also raises concerns about serious long-term health issues, particularly for patients with renal involvement.

A 2-year-old girl developed dermatitis with atypical brown crusts after ethanol disinfection. Since many Oriental people have genetically reduced acetaldehyde dehydrogenase type 2 (ALDH2) activity, ethanol disinfection causes acetaldehyde to accumulate in the skin, resulting in dermatitis.

The Immunology of Takotsubo Syndrome.

Front Immunol

November 2023

Division of Community Medicine and Career Development, Kobe University Graduate School of Medicine, Kobe, Japan.

Takotsubo syndrome (TTS) is a disorder characterized by transient cardiac dysfunction with ventricular regional wall motion abnormalities, primarily thought to be caused by the effects of a sudden catecholamine surge on the heart. Although the majority of patients exhibit prompt recovery of their cardiac dysfunction, TTS remains associated with increased mortality rates acutely and at long-term, and there is currently no cure for TTS. Inflammation has been shown to play a key role in determining outcomes in TTS patients, as well as in the early pathogenesis of the disorder.

Objectives: The combination of chemotherapy and immune checkpoint inhibitors (chemo-ICI) has become the new standard of treatment for extensive-stage small cell lung cancer (ES-SCLC). Recently, slight changes in interstitial shadows, defined as interstitial lung abnormalities (ILA), have been identified. In patients with ES-SCLC who received chemo-ICI, there are limited data on the incidence of drug-induced interstitial lung disease (D-ILD) in daily practice and the association between the development of D-ILD and ILA in the baseline computed tomography (CT).
