Background and Purpose- The American Heart Association and the American Stroke Association guidelines for early management of patients with ischemic stroke offer guidance to physicians involved in acute stroke care and clarify endovascular treatment indications. The purpose of this study was to assess concordance of physicians' endovascular treatment decision-making with current American Heart Association and the American Stroke Association stroke treatment guidelines using a survey-approach and to explore how decision-making in the absence of guideline recommendations is approached. Methods- In an international cross-sectional survey (UNMASK-EVT), physicians were randomly assigned 10 of 22 case scenarios (8 constructed with level 1A and 11 with level 2B evidence for endovascular treatment and 3 scenarios without guideline coverage) and asked to declare their treatment approach (1) under their current local resources and (2) assuming there were no external constraints.

Bisphosphonate-related osteonecrosis of the jaw (BRONJ), characterized by refractory bone exposure, has recently emerged as a serious side effect of bisphosphonate (BPs) treatment. We discuss novel insights that may help to improve the efficacy of BRONJ treatment and prevention. Our report highlights the following: (1) The presence of exposed bone in patients taking BPs does not necessarily reflect BRONJ, and diagnoses of oral ulceration with bone sequestration and malignancy must be excluded.

In our earlier study, noradrenaline (NA) stimulated ATP release from dorsal root ganglion (DRG) neurons as mediated via β(3) adrenoceptors linked to G(s) protein involving protein kinase A (PKA) activation, to cause allodynia. The present study was conducted to understand how ATP is released from DRG neurons. In an outside-out patch-clamp configuration from acutely dissociated rat DRG neurons, single-channel currents, sensitive to the P2X receptor inhibitor PPADS, were evoked by approaching the patch-electrode tip close to a neuron, indicating that ATP is released from DRG neurons, to activate P2X receptor.

A 64-year-old man had undergone a right total knee arthroplasty (TKA) as treatment for osteoarthritis of the knee. Six months after the TKA, the patient sustained a stress fracture of the left fifth metatarsal bone, which was a contralateral side of the TKA, without any apparent trauma or cause. The fracture was treated with internal fixation using a screw and low-intensity pulsed ultrasound treatment was added.

In liver surgery, accurate assessments of liver resection volume and anatomical variation are mandatory for preoperative planning of safe curative hepatectomy. In living donor liver transplantation (LDLT), estimation of hepatic venous drainage is important to avoid liver graft and donor residual liver congestion. This paper reviews the articles on simulation-guided liver surgery and describes our novel 3D hepatectomy simulation system for liver resection and transplantation.
