Article Synopsis
  • The study focused on the incidence and characteristics of postinjury multiple organ failure (MOF) in severely injured trauma patients across five trauma centers in New South Wales, Australia.
  • Out of 600 polytrauma patients studied, 23% developed MOF, with the majority showing symptoms by day 3, and no new cases occurring after day 13.
  • Cardiac failure was the most common organ failure observed, with different mortality rates among organ failures, indicating that while MOF is rare in the general population, it is a significant risk in severely injured patients.

Injury

February 2024

Department of Traumatology, John Hunter Hospital, Locked Bag 1, Hunter Region Mail Centre, Newcastle, NSW, Australia; Discipline of Surgery, School of Public Medicine and Health, University of Newcastle, Newcastle, NSW, Australia. Electronic address:

Introduction: Distal femur fractures (DFF) are common, especially in the elderly and high energy trauma patients. Lateral locked osteosynthesis constructs have been widely used, however non-union and implant failures are not uncommon. Recent literature advocates for the liberal use of supplemental medial plating to augment lateral locked constructs.

Introduction: Identifying an underlying germline cancer predisposition (CP) in a child with cancer has potentially significant implications for both the child and biological relatives. Cohort studies indicate that 10%-15% of paediatric cancer patients carry germline pathogenic or likely pathogenic variants in cancer predisposition genes, but many of these patients do not meet current clinical criteria for genetic testing. This suggests broad tumour agnostic germline testing may benefit paediatric cancer patients.

Introduction: Social determinants of health (SDH) are contributors to health inequities experienced by some children with cerebral palsy and pose barriers to families engaging with complex and fragmented healthcare systems. There is emerging evidence to support 'social prescribing' interventions that systematically identify SDH concerns and refer patients to non-medical social care support and services to address their needs. To date, social prescribing has not been trialled specifically for children with neurodevelopmental disabilities, including cerebral palsy, in Australia.

The influence of weight-bearing status on post-operative mobility and outcomes in geriatric hip fracture.

Eur J Trauma Emerg Surg

October 2022

Department of Traumatology, John Hunter Hospital, Lookout Rd, New Lambton Heights, NSW, 2305, Australia.

Purpose: We hypothesized that unrestricted or full weight-bearing (FWB) in hip fracture would increase the opportunity to mobilize on post-operative day 1 (POD1mob) and be associated with better outcomes compared with restricted weight-bearing (RWB).

Methods: Over 4 years, 1514 geriatric hip fracture patients aged 65 and above were prospectively recruited. Outcomes were compared between FWB and RWB patients.

Article Synopsis
  • Procedural anxiety in oncology patients is a common issue, often triggered by fear of medical procedures like imaging and treatment, with current management primarily relying on anxiolytic medications which aren't suitable for all patients.
  • This systematic review will explore the effectiveness of non-pharmacological interventions to reduce procedural anxiety for cancer patients undergoing radiotherapy.
  • The review will analyze various studies using established databases to assess the impacts of these interventions on self-reported anxiety levels and additional outcomes such as physiological measures and treatment-related factors.

Background: Recent trauma guidelines recommend non-operative management for grade III splenic injury without contrast extravasation on computed tomography. Nevertheless, such recommendations rely on low-quality evidence, and practice variation characterizes clinical management for this type of injury. We aimed to identify the role of eleven selected clinical factors influencing the management of grade III splenic injury without contrast extravasation by expert consensus and a modified Delphi approach.

Aim: The routine use of prophylactic percutaneous endoscopic gastrostomy (PEG) tubes for nutrition support during radical chemoradiation for head and neck cancer has been suggested to result in PEG dependency. This research aimed to determine the rates of gastrostomy dependency at the Calvary Mater Newcastle (CMN) where PEGs are routinely used and to identify potentially modifiable risk factors.

Methods: All patients with head and neck cancer planned for curative chemoradiation with a prophylactic PEG inserted were included in this review.

Current strategies to predict and manage sequelae of posterior urethral valves in children.

Pediatr Nephrol

October 2018

Department of Paediatric Urology and Surgery, John Hunter Children's Hospital, Locked Bag 1, Hunter Region Mail Centre, Newcastle, NSW, 2310, Australia.

Article Synopsis
  • - Posterior urethral valves (PUV) are a leading cause of chronic kidney disease (CKD) in children, primarily due to how bladder dysfunction develops after treating the obstruction.
  • - Advances in prenatal diagnosis and treatment have not significantly improved long-term kidney health, but better patient selection might help in the future.
  • - Proactive management strategies for bladder dysfunction, along with ongoing research into prognosis and early kidney damage markers, are essential for improving outcomes, especially for those needing renal transplants.

Differential methylation at in CD4 T cells is associated with multiple sclerosis independently of .

Clin Epigenetics

April 2018

Centre for Information Based Medicine, Hunter Medical Research Institute, Locked Bag 1, Hunter Region Mail Centre, Newcastle, 2310 Australia.

Article Synopsis
  • This study investigates the role of DNA methylation in multiple sclerosis (MS) using an independent cohort of female patients with relapsing-remitting MS (RRMS) to confirm earlier findings regarding differential methylation regions (DMRs).
  • Researchers extracted genomic DNA from CD4 T cells of RRMS patients and healthy controls, analyzing the methylation patterns to identify significant DMRs associated with the disease.
  • The results reinforced earlier discoveries of specific hypomethylated and hypermethylated DMRs, particularly in the MHC region, suggesting that epigenetic modifications may significantly influence MS pathology regardless of other factors like treatment and age.
