Objective: This study aimed to estimate the clinical utility of performing multi-gene pharmacogenetic testing on patients undergoing gynecologic surgery/procedure by evaluating the prescribing rate of Clinical Pharmacogenetics Implementation Consortium (CPIC) level A medications and frequency of drug-gene interactions (DGIs).

Methods: The electronic health record was queried for 76 current procedural terminology codes to identify gynecologic surgeries/procedures that occurred between 1 January 2015 to 31 December 2020 in patients with at least one of 152 international classification of disease codes. Prescription data for CPIC level A medications was extracted.

Article Synopsis
  • BMI might not accurately reflect the risk of obesity-related cancer (ORC) because metabolic issues can exist at various BMI levels.
  • A study of 20,593 postmenopausal women identified four types of metabolic health and found that those with metabolic dysfunction—regardless of weight—had an increased risk of ORC.
  • The research showed that overweight and obese individuals, especially those with metabolic dysfunction, faced a higher risk of developing ORC compared to metabolically healthy individuals with normal weight.

Background: Familial chylomicronemia syndrome (FCS) is an ultrarare inherited disorder. Genetic testing is not always feasible or conclusive. European clinicians developed a "FCS score" to differentiate between FCS and multifactorial chylomicronemia syndrome (MCS), a more common condition with overlapping features.

  • Left bundle branch area pacing (LBBAP) is a new approach to pacing that aims to improve heart function after aortic valve replacement, compared to the traditional right ventricular septal pacing (RVSP).
  • The Left Bundle BRAVE study is a clinical trial designed to compare LBBAP and RVSP in patients with severe aortic stenosis and high-degree heart block to see which method better preserves heart function.
  • Key outcomes being measured in the study include changes in heart strain and ejection fraction, along with other markers of heart health and patient performance over a 9-month period.

Proteome-Wide Genetic Investigation of Large Artery Stiffness.

JACC Basic Transl Sci

October 2024

Division of Cardiovascular Medicine, Hospital of the University of Pennsylvania, Philadelphia, Pennsylvania, USA.

The molecular mechanisms contributing to large artery stiffness (LAS) are not fully understood. The aim of this study was to investigate the association between circulating plasma proteins and LAS using complementary proteomic and genomic analyses. A total of 106 proteins associated with carotid-femoral pulse-wave velocity, a noninvasive measure of LAS, were identified in 1,178 individuals from the Asklepios study cohort.

There are a growing number of adult patients palliated to a Fontan circulation. As these patients age, many develop symptomatic heart failure (d'Udekem et al in Circulation 130:S32-S38, 2014) that is exacerbated by acquired comorbidities such as obesity and hypertension. Increased body mass index (BMI) and adiposity have been associated with worse hemodynamics and clinical outcomes in these patients (Yogeswaran et al in J Am Hear Assoc: Cardiovasc Cerebrovasc Dis 12:e026732, 2023).

Free Flap Reconstruction of the Oropharynx.

Facial Plast Surg Clin North Am

February 2025

Department of Otorhinolaryngology-Head and Neck Surgery, Hospital of the University of Pennsylvania, 5th Floor Ravdin, 3400 Spruce Street, Philadelphia, PA 19104, USA. Electronic address:

Reconstruction of the oropharynx plays a critical role in preserving quality of life after surgical resection of oropharyngeal carcinoma. Free tissue is one of several reconstructive options, which can closely approximate native oropharyngeal anatomy and lead to favorable functional outcomes in carefully selected patients. Here, the authors provide an overview of the indications, treatment options, functional outcomes, potential complications, and future considerations for free flap reconstruction of the oropharynx.

Treatment options for patients with metastatic castration-resistant prostate cancer include the use of radioligand therapy with Lu-PSMA-617. Although Lu-PSMA-617 can selectively target prostate cancer cells, salivary glands express PSMA on the apical lumen of the acinar epithelium. Xerostomia resulting from the use of radioligand therapy is common.

Background: An improved understanding of the natural history in NYHA functional class I patients with obstructive hypertrophic cardiomyopathy (oHCM) is needed.

Objectives: Using a multicenter registry (SHaRe [Sarcomeric Human Cardiomyopathy Registry]), this study described the natural history in patients with oHCM who were classified as NYHA functional class I at the initial visit compared with patients classified as NYHA functional class II and reported baseline characteristics associated with incident clinical events.

Methods: Incident events assessed included a composite of NYHA functional class III to IV symptoms, left ventricular ejection fraction <50%, atrial fibrillation, stroke, ventricular arrhythmias, septal reduction therapy, ventricular assist device or transplantation, or death.

  • Understanding hip anatomy and development is key for diagnosing femoral neck stress fractures in athletes, which are caused by repetitive mechanical stress.
  • Various risk factors such as hormonal issues and hip shape contribute to these fractures.
  • While X-rays are commonly used for initial assessment, MRI is preferred for accurately differentiating between stress reactions and fractures, with treatment options ranging from surgery to effective nonsurgical methods.

Article Synopsis
  • A study evaluated the performance of large language models (LLMs), specifically ChatGPT and Bard, in pathology by comparing their scores on multiple-choice questions against those of 16 pathology trainees from two hospitals.
  • ChatGPT outperformed both Bard and the trainees, scoring an average of 82.2%, while Bard scored 49.5% and junior and senior trainees scored 45.1% and 56.0%, respectively.
  • The consistency of ChatGPT's responses was notably high (80%-85%), while Bard showed more variability (54%-61%), indicating the potential of LLMs in pathology education but emphasizing the need for further development and human input.

Approach to Evaluating Superficial Soft Tissue Masses by Ultrasound.

Radiol Clin North Am

January 2025

Department of Radiology, Hospital of the University of Pennsylvania, Philadelphia, PA, USA. Electronic address:

The purpose of this article is to give a systematic approach to the ultrasound evaluation of superficial soft-tissue masses. The knowledge of proper technique in image acquisition, the characteristic sonographic appearances of the most common masses, their potential pitfalls, and the location of the lesions can help establish a confident diagnosis. Where the sonographic features of the masses are inconclusive, a percutaneous biopsy is effective in establishing the definitive diagnosis.

Article Synopsis
  • * A study analyzed 101 patients across four US centers, finding low complication rates (5.9% thromboembolic events and 0% procedure-related mortality), and satisfactory aneurysm occlusion at follow-ups (58% at 6 months and 74.8% at 6-12 months).
  • * The results suggest that FRED X stenting is a safe and effective option for treating unruptured intracranial aneurysms, although more long-term data is needed for comprehensive evaluation.*
