Med Sci Sports Exerc

November 2024

Division of Pediatric Oncology/Hematology, University Children's Hospital Basel, University of Basel, Basel, SWITZERLAND.

Introduction: Impaired physical fitness is a possible late effect among adult survivors of childhood cancer (ASCC). Our study describes lower body muscular strength and endurance among ASCC using the 1-minute sit-to-stand (1-min STS) test, compares them with the general population, identifies risk factors, and describes changes over time. Methods: In a prospective multicenter cohort study, we invited ASCC ≥18 years of age at study, diagnosed between ages 0-20 treated in five pediatric oncology centers across Switzerland from 1976-2017 who survived ≥5 years for a 1-min STS test.

The ARCR_Pred study was initiated to document and predict the safety and effectiveness of arthroscopic rotator cuff repair (ARCR) in a representative Swiss patient cohort. In the present manuscript, we aimed to describe the overall and baseline characteristics of the study, report on functional outcome data and explore case-mix adjustment and differences between public and private hospitals. Between June 2020 and November 2021, primary ARCR patients were prospectively enrolled in a multicenter cohort across 18 Swiss and one German orthopedic center.

Studies have shown that the human microbiome influences the response to systemic immunotherapy. However, only scarce data exist on the impact of the urinary microbiome on the response rates of bladder cancer (BC) to local instillation therapy. We launched the prospective SILENT-EMPIRE study in 2022 to address this question.

Introduction: Little is known about the clinical status of persons with HIV (PWH) who re-engage in care after an interruption. We evaluated the immunological and clinical characteristics of individuals re-engaging in care within the Swiss HIV Cohort Study.

Methods: Participants who re-engaged in care after an interruption >14 months with a viral load ≥100 copies/mL were classified as having interrupted ART.

Background: Anterior column realignment (ACR), using a lateral lumbar or thoracic interbody fusion (LLIF) approach to release the anterior longitudinal ligament (ALL), is a powerful technique to increase segmental lordosis. We here report our experience with the use of expandible LLIF cages for ACR.

Methods: Retrospective, single-center observational cohort study including consecutive patients treated by LLIF using an expandible interbody implant.

Introduction: Anti-retroviral therapy (ART) simplification strategies are needed for treatment-experienced people with HIV (PWH) and multidrug-resistant viruses. These individuals are commonly treated with boosted ART regimens and are thereby at risk for harmful drug-drug interactions (DDI). In this trial, we aim to assess the efficacy of the combination doravirine, dolutegravir and lamivudine (DOR/DTG/3TC) among people with a history of virological failure who receive boosted ART.

The dynamic range of immunoassays for heparin-induced thrombocytopenia.

J Thromb Haemost

November 2024

Department of Clinical Chemistry, Inselspital University Hospital Bern, Bern, Switzerland; Faculty of Medicine, University of Bern, Bern, Switzerland. Electronic address:

Article Synopsis
  • The study evaluates the diagnostic significance of two immunoassay tests for heparin-induced thrombocytopenia (HIT) using data from 1393 patients, aiming to clarify the reliability of test results across their ranges.
  • Findings reveal that while both assays correlate results to HIT diagnoses, the strength of this correlation differs, with the CLIA method showing a more pronounced increase in likelihood ratios compared to the ELISA method.
  • A web-based calculator is provided to help clinicians estimate the probability of HIT based on individual test results, enhancing decision-making in patient care.

Purpose: To report on the failure rates of an expandable interbody spacer, used for spinal instrumented transforaminal lumbar interbody fusion (TLIF) or posterior thoracolumbar interbody fusion (PLIF).

Methods: In this retrospective, single-center observational cohort study we reviewed consecutive PLIF/TLIF patients using the Catalyft™ PL and PL40 expandable titanium interbody implant (Medtronic, Minneapolis (USA)) between 07/2022 and 11/2023. We recorded patient demographics, surgical parameters, adverse events, radiological parameters and clinical outcomes according to the MacNab criteria.

High breast density is a risk factor for breast cancer and can reduce the sensitivity of mammography. Given the influence of breast density on patient risk stratification and screening accuracy, it is crucial to monitor the prevalence of extremely dense breasts within local populations. Moreover, there is a lack of comprehensive understanding regarding breast density prevalence in Switzerland.

Article Synopsis
  • The study explores the impact of HIV on infectious disease events in kidney transplant recipients, showing that these patients have similar survival rates to HIV-negative individuals.
  • It analyzed data from the Swiss HIV Cohort Study and the Swiss Transplant Cohort Study, focusing on demographic and clinical characteristics since 2008.
  • The results indicated that while 70.8% of the HIV-positive patients experienced infectious disease events, HIV itself was not a significant risk factor for these events post-transplant.

We aimed to investigate the association of preoperative copeptin, a new cardiovascular biomarker, with short- and long-term mortality in a cohort of adult patients undergoing cardiac surgery, including its potential as a prognostic marker for clinical outcome. Preoperative blood samples of the Bern Perioperative Biobank, a prospective cohort of adults undergoing cardiac surgery during 2019, were analyzed. The primary and secondary outcome measures were 30-day and 1-year all-cause mortality.

Metabolites

June 2024

Department of Anaesthesiology and Pain Medicine, Inselspital, Bern University Hospital, University of Bern, 3010 Bern, Switzerland.

Indoleamine 2,3-deoxygenase (IDO) plays an important role in the catabolism of the amino acid tryptophan. Tryptophan and its metabolites are key immune modulators. Increased IDO activity has been observed in various diseases and is associated with worse clinical outcomes.

Adjuvant Analgesics in Acute Pain - Evaluation of Efficacy.

Curr Pain Headache Rep

September 2024

Department of Anesthesiology, Rescue- and Pain Medicine, Cantonal Hospital of St. Gallen, St. Gallen, Switzerland.

Purpose Of The Review: Acute postoperative pain impacts a significant number of patients and is associated with various complications, such as a higher occurrence of chronic postsurgical pain as well as increased morbidity and mortality.

Recent Findings: Opioids are often used to manage severe pain, but they come with serious adverse effects, such as sedation, respiratory depression, postoperative nausea and vomiting, and impaired bowel function. Therefore, most enhanced recovery after surgery protocols promote multimodal analgesia, which includes adjuvant analgesics, to provide optimal pain control.
