Penicillin allergy is the most commonly reported drug allergy, with prevalence rates ranging from 6% to 31% across various populations and geographic areas. The penicillin allergy label is linked to higher mortality and morbidity rates, extended hospital stays, increased readmission rates, and a greater reliance on second-line antibiotics. Research indicates that nearly 99% of those labeled as penicillin-allergic can tolerate the drug.

Crit Care

December 2024

Department of Anesthesia and Intensive Care Unit, Regional University Hospital of Montpellier, St-Eloi Hospital, PhyMedExp, INSERM U1046, CNRS UMR, University of Montpellier, 9214, Montpellier Cedex 5, France.

Background: Ultra-protective ventilation is the combination of low airway pressures and tidal volume (Vt) combined with extra corporeal carbon dioxide removal (ECCOR). A recent large study showed no benefit of ultra-protective ventilation compared to standard ventilation in ARDS (Acute Respiratory Distress Syndrome) patients. However, the reduction in Vt failed to achieve the objective of less than or equal to 3 ml/kg predicted body weight (PBW).

While the phenotypic diversity of childhood wheezing is well described, the subsequent life course of such phenotypes and their adult outcomes remain poorly understood. We hypothesized that different childhood wheezing phenotypes have varying longitudinal outcomes at age 26. We sought to identify factors associated with wheezing persistence, clinical remission, and new onset in adulthood.

Extensive macular atrophy with pseudodrusen-like appearance (EMAP) was first described in France in 2009 as a symmetric and rapidly progressive form of macular atrophy primarily affecting middle-aged individuals. Despite the recent identification of a significant number of cases in Italy and worldwide, EMAP remains an underrecognized condition. The clinical triad typical of EMAP consists of vertically oriented macular atrophy with multilobular borders, pseudodrusen-like deposits across the posterior pole and mid-periphery, and peripheral pavingstone degeneration.

Cesarean delivery and blood DNA methylation at birth and childhood: Meta-analysis in the Pregnancy and Childhood Epigenetics Consortium.

Sci Adv

November 2024

Division of Chronic Disease Research Across the Lifecourse (CoRAL), Department of Population Medicine, Harvard Medical School, Harvard Pilgrim Health Care Institute, Boston, MA 02215, USA.

Article Synopsis
  • - Children born via cesarean delivery have a greater risk of various health issues compared to those born vaginally, but the exact reasons are still not fully understood.
  • - A meta-analysis involving over 12,000 participants found six specific DNA methylation markers in newborns linked to cesarean delivery, but these markers did not persist into childhood.
  • - The study indicates that cesarean delivery affects certain blood cell proportions at birth, but further research is necessary to understand its long-term impacts on child health.

Assessing the role of environment in Pseudomonas aeruginosa healthcare-associated bloodstream infections: a one-year prospective survey.

J Hosp Infect

December 2024

HydroSciences Montpellier, IRD, CNRS, Montpellier University, Hospital Hygiene and Infection Control Department, University Hospital of Montpellier, Montpellier, France.

Article Synopsis
  • - The study aimed to explore how the hospital environment contributes to Pseudomonas aeruginosa bloodstream infections (BSIs) in patients who have been hospitalized for over 48 hours in a tertiary-care hospital.
  • - Researchers examined clinical records and conducted environmental investigations to identify sources of contamination, finding that most patients were male, older, and had prior antibiotic use, with the main infection routes being urinary and cutaneous.
  • - Although the environment was suspected to be a major source of these infections, only 15% of nosocomial BSIs were linked to the hospital, suggesting that Pseudomonas might have transitioned to being more of a community-acquired pathogen rather than strictly a hospital-related one.
