The Cox-maze III procedure is the gold standard for the surgical treatment of atrial fibrillation. Because of its complexity, this procedure has not been widely accepted by cardiac surgeons. We describe a technique that allows one to work on the mitral valve while more safely performing the standard 'cut-and-sew' Cox-maze III procedure at the same time.

Background: Pulmonary vein isolation has been employed to eliminate paroxysmal atrial fibrillation. However, the long-term outcome in terms of long-standing persistent atrial fibrillation is unclear.
Methods: One hundred patients with rheumatic mitral valve disease and long-standing (>1 year) persistent atrial fibrillation were operated on between July 1998 and June 2007.
