When a decision has been reached to proceed with coronary bypass surgery, this represents a therapeutical option for a partnership between surgical revascularization and an optimized multifaceted medical treatment. We report the guidelines implemented in our department concerning the secondary prevention therapy after coronary bypass surgery. The antiplatelet therapy (and its association with coumadin), the bradycardizing treatments (with a special reference to the approach of unappropriate sinus tachycardia), the specific anti-ischemic therapy, the treatment of dyslipidemia, and the antihypertensive therapy are reviewed as well as the use of additional anti-atheromatous measures, the approach of hyperuricemia and hyperhomocysteinemia, and the oral antidiabetic treatment.

Objectives: The authors report a retrospective study of patients who underwent a Bentall procedure in the Cardio-Thoracic Surgery Department of Hospital Santa Maria, Lisbon.
Material And Methods: Data were obtained from 42 patients who underwent the Bentall procedure between 1991 and 2008. Different parameters were compared as well as the short and long term results.

Lemierre's syndrome (LS) is an uncommon but potentially life-threatening complication of an anaerobic oropharyngeal infection, affecting young adults and adolescents. The disease is characterised by a septic thrombophlebitis of the internal jugular vein and "metastatic" infections, which can be followed by fulminant sepsis and rapid death. More recently, it has been reported a recrudescence of this condition, which could be attributable to alterations in antibiotic usage patterns.

The compartment syndrome is a common and severe clinical condition within the scope of peripheral vascular surgery and it is accompanied by a high risk of a limb loss or even the patients' death. Its etiology is linked to the acute limb ischemia or trauma and its management consists essentially in the descompressive fasciotomies, associated to diverse pharmacological agents with doubtful efficacy. The main features of this clinical entity and the criteria for therapeutic interventions are subjected to analysis and discussion.

Endovascular treatment through the arterial or venous way is part of the management of diverse lesions in the head and neck, often as complementary of surgery, in highly vascularized tumors or single vascular lesions. Concerning the vascular pathology, the intra-arterial occlusive therapy can be used in some types of head and neck malformations, isolated or in association to surgery, to accomplish the complete removal of the "nidus" of the malformation, or for cosmetic purposes. Regarding the post traumatic arteriovenous fistulas, intra-arterial or intravenous embolization has been widely used and reported.

Isolated lesions of the extra-thoracic common carotid arteries are unusual in clinical practice. Contrarily to the well known lesions of carotid bifurcation, its natural history is poorly understood and indications for surgical management are not well defined. Painful shoulders are a frequent pathology and its specific diagnosis and therapeutic control are common challenges to medical professionals. To solve these problems a non invasive morphologic method of study--Real Time Ultrasonography--is presented.
