Niemann-Pick type C (NPC) is a disorder of the lysosomal metabolism due to biallelic pathogenic variants in NPC1 or NPC2. Intracellular deposit of lipids, mainly unesterified cholesterol, gives rise to oxidative damage in several tissues, mainly neurons, spleen and liver. This, in turn, is associated with a myriad of neurological / psychiatric and visceral symptoms, with wide variability in age of presentation, from prenatal / neonatal to adult-onset forms of the disease.

Background: Treating B-non-Hodgkin lymphoma (B-NHL) in lower-income countries is challenging because of imprecise diagnosis, the increased risk of fatal toxicity associated with advanced disease at presentation, and limited supportive care.
Procedure: Central American patients with newly diagnosed stage I or II B-NHL received a modified Berlin-Frankfurt-Münster (BFM) regimen including a prephase (prednisone, cyclophosphamide) followed by A/B/A courses (A: cytarabine, dexamethasone, etoposide, ifosfamide, methotrexate, and intrathecal therapy; B: cyclophosphamide, dexamethasone, doxorubicin, methotrexate, and intrathecal therapy). Those with stage III or IV NHL received additional courses (B/A/B), intensified for stage IV disease by additional vincristine and methotrexate doses.

It is a reality that, in the field of pediatrics, parents are the legal representatives of their children, which means that any medical decision in relation to the child should be taken with the intercession of at least one of his/her parents who -in the exercise of a subrogated autonomy and a legally granted representation- provide their informed consent. The problem arises when the parents refuse to give their informed consent or directly make decisions that will have a specific impact on their child's well-being, thus causing damage or at least a risk for damage. The challenge here is to reconcile the interests and rights at stake, always considering that the constitutional rule of interpretation in the case of a child are his/her best interests.
