The epidermis over a dermatofibroma may show changes that range from simple hyperplasia to the proliferation of basaloid cells, which can become morphologically indistinguishable from basal cell carcinoma. The existence of a true basal cell carcinoma overlying a dermatofibroma is infrequent. These basaloid proliferations have usually been considered to be the result of the inductive effect of the fibrohistiocytic proliferation of the dermatofibroma on the epithelial cells of the hair follicle; therefore, it would be a reactive phenomenon and not truly neoplastic.

Case Report: A 63-year-old male with a history of hypertension, hypercholesterolemia and chronic smoking, presented with a loss of the temporal visual field of right eye (RE). A restriction of the temporal field of RE and also of the nasal field of left eye was clinically suspected when the patient was tested by confrontation. A kinetic perimetry confirmed the existence of a right homonymous hemianopia that was very eccentric in both eyes (beyond the central 50 degrees).

Although the procedure has been available for more than 40 years, constant technical progress in surgical and percutaneous revascularization continues to bring the benefits of revascularization therapy to new groups of patients. Therefore the indications and limitations of this approach need to be reviewed and updated periodically.

The immunohistochemical analysis of the spindle cell lipomas has been for the most part limited to the study of S-100 protein, CD34, and Bcl-2 reactivity. To evaluate the immunoexpression of desmin and actins in spindle cell lipomas of different histological subtypes a retrospective immunohistochemical study of 25 spindle cell lipomas using archival formalin-fixed, paraffin-embedded tissue was performed. Strong positivity of the spindle cell component for desmin was found in 4 of 25 cases (16%).
