[Barrett's esophagus: histochemical difference between goblet cells and blue cells].

Rev Med Inst Mex Seguro Soc

September 2023

Instituto Méxicano del Seguro Social, Centro Médico Nacional del Bajío, Hospital de Especialidades No. 1, Departamento de Anatomía Patológica. León, Guanajuato, México.

Background: Barrett's esophagus (BE) is the replacement of the usual esophageal mucosa by a simple columnar epithelium with the presence of goblet cells (GC) of intestinal type. It has been related to different risk factors such as gastroesophageal reflux disease (GERD), inappropriate consumption of irritating foods, smoking and overweight. There are CC mimic cells, known as blue cells (BC), which make the diagnosis of BE difficult, due to the lack of a precise definition of the nature and location of the gastroesophageal junction and the microscopic variations in this area.

[Comparative analysis of bleed volume with visual technique].

Rev Med Inst Mex Seguro Soc

September 2023

Instituto Mexicano del Seguro Social, Centro Médico Nacional del Bajío, Hospital de Especialidades No. 1, Servicio de Anestesiología. León, Guanajuato, México.

Background: Blood loss estimation in a surgery is made by anesthesiologists by means of visual technique, which is not reliable because it can change depending on the judgement of every person, or his/her work experience, which is why it is considered something subjective. Therefore, the results obtained could lead to make mistakes with the exact amount of bleeding, mismanaging unnecessary hemoderived transfusions or administering unnecessary drugs.

Objective: To compare the blood volume and its visual calculation between Anesthesiology residents and anesthesiologists.

[LDH-neutrophil-lymphocyte index as a predictor of 28-day mortality in patients with COVID-19].

Rev Med Inst Mex Seguro Soc

September 2023

Instituto Mexicano del Seguro Social, Centro Médico Nacional del Bajío, Hospital de Especialidades No. 1, Servicio de Infectología. León, Guanajuato, México.

Background: Coronavirus disease 2019 (COVID-19) represents the greatest health crisis of our times; it was declared by WHO a pandemic in March 2020. The risk of presenting a severe disease is inter-individual, since it varies according to age, comorbidities, and immunological status, in addition to the type of SARS-CoV-2 variant. The neutrophil/lymphocyte ratio (NLR) and lactic dehydrogenase (LDH) are widely used markers to assess the severity and predict the course of the disease in patients with COVID-19, with a direct relationship of higher value-worse prognosis.

[Association of neutrophil, lymphocyte, platelet ratio with acute kidney injury in sepsis].

Rev Med Inst Mex Seguro Soc

May 2023

Instituto Mexicano del Seguro Social, Centro Médico Nacional del Bajío, Hospital de Especialidades No. 1, Unidad de Cuidados Intensivos. León, Guanajuato, México.

Background: Acute kidney injury (AKI) is frequent in sepsis (25 to 51%), with high mortality (40 to 80%) and long-term complications. Despite its importance we do not have accessible markers in intensive care. In other entities (post-surgical and COVID-19) the neutrophil/lymphocyte and platelet (N/LP) ratio has been associated with acute kidney injury; however, this relationship has not been studied in a pathology with a severe inflammatory response such as sepsis.

[Dysnatremias and their association with morbidity and mortality in patients with COVID- 19].

Rev Med Inst Mex Seguro Soc

August 2022

Instituto Mexicano del Seguro Social, Centro Médico Nacional del Bajío, Hospital de Especialidades No.1, Servicio de Medicina Interna. León, Guanajuato, México.

Article Synopsis
  • COVID-19 has led to a significant health crisis, emphasizing the need for risk assessment at hospitalization to identify high-risk patients with potential complications.
  • Dysnatremia, particularly hypernatremia, has emerged as a key predictor of mortality in COVID-19 patients, with this study aiming to assess its impact on 28-day mortality and related hospital outcomes.
  • The study analyzed 722 COVID-19 patients, finding that hypernatremia correlated with increased mortality rates, higher risks for acute kidney injury (AKI), and greater needs for invasive mechanical ventilation (IMV) during hospital stays.

Article Synopsis
  • Hyperchloremia, an imbalance of chloride levels in the body, may increase morbidity and mortality, particularly in critically ill patients, and this study explores its link to COVID-19 outcomes.
  • The study aimed to see if a sodium-chloride difference of less than 31 mEq/L within 24-48 hours of COVID-19 admission predicts 28-day mortality and other complications like the need for mechanical ventilation and kidney issues.
  • Results indicated that while a sodium-chloride difference below 31 mEq/L doesn't predict higher death or ventilation rates, it is linked to a greater risk of acute kidney injury and longer hospital stays for COVID-19 patients.
