Article Synopsis
  • Facioscapulohumeral muscular dystrophy (FSHD) is a rare muscular dystrophy that increases the risk of serious complications during surgery, such as hyperkalemia and rhabdomyolysis.
  • This case report focuses on the anesthetic management of a child with FSHD who underwent an exploratory tympanotomy, highlighting the challenges involved due to the condition.
  • Notably, this is the first documented instance of pediatric general anesthesia for a patient with FSHD that also involved the use of sugammadex for reversing neuromuscular blockade.

  • Interstitial lung disease (ILD) is prevalent in rheumatoid arthritis (RA) and linked to higher mortality; this study assesses antifibrotics nintedanib and pirfenidone's real-world effectiveness and safety in RA-ILD patients.
  • A multicenter observational study included 40 RA-ILD patients, primarily treated with nintedanib; data on demographics, pulmonary function tests (PFTs), and adverse events (AEs) were collected and analyzed.
  • Results showed that antifibrotics halted the decline in forced vital capacity (FVC) and total lung capacity (TLC) after initiation, but diffusion capacity continued to decline, with 40% of patients experiencing adverse events.

  • - Renal colic is the most common abdominal pain in pregnancy and can lead to complications, requiring treatments like ureteral stent (JJ), percutaneous nephrostomy (PCN), or ureteroscopy (URS) for urolithiasis management.
  • - A systematic review analyzed 45 studies involving 3424 procedures in pregnant women, finding URS to be the most evaluated with similar success rates to non-pregnant patients, although complications like infections and preterm labor were noted.
  • - Despite a lack of high-quality evidence, URS, JJ, and PCN are deemed safe and effective in pregnancy, with URS being the preferred treatment option due to its efficiency and lower need for follow-up procedures

Acute embolic sequential bilateral occlusion is an extremely rare event and can be difficult to detect during mechanical thrombectomy (MT) under general anesthesia (GA). We describe a male with vascular risk factors who was admitted two hours after the sudden onset of aphasia, right-sided central facial palsy, homonymous hemianopsia, and ipsilateral hemiplegia/hypoesthesia. Thrombolytic intravenous treatment was started, and the patient had an allergic reaction and required intubation.

Non-functional alpha-cell hyperplasia with glucagon-producing NET: a case report.

Front Endocrinol (Lausanne)

September 2024

Unit for Multidisciplinary Research in Biomedicine (UMIB), School of Medicine and Biomedical Sciences (ICBAS), University of Porto, Porto, Portugal.

Article Synopsis
  • Alpha-cell hyperplasia (ACH) is a rare pancreatic condition with various forms and is often associated with pancreatic neuroendocrine tumors (pNETs), although its causes are not well understood.
  • A 72-year-old male with type 2 diabetes was found to have a non-functioning pNET during treatment for gallbladder issues, which revealed mild glucagon elevation and led to surgery.
  • Pathological analysis showed the pNET was glucagon-producing and occurred alongside ACH, emphasizing the need for specialized testing to diagnose such cases even when symptoms suggest a non-functioning tumor.

  • About one-third of Portuguese patients with congenital hypogonadotropic hypogonadism (CHH) have a genetic cause linked to 10 different genes, with a small percentage showing oligogenic inheritance.
  • CHH leads to delayed puberty and infertility due to issues with hormones like GnRH, LH, and FSH.
  • In a study involving 81 patients and 263 controls, genetic screening helped identify pathogenic variants, but many variants classified as uncertain complicate the understanding of the disease causes.

Meningeal lymphatic function promotes oligodendrocyte survival and brain myelination.

Immunity

October 2024

Department of Neuroscience, Mayo Clinic, Jacksonville, FL 32224, USA; Neuroscience Ph.D. Program, Mayo Clinic Graduate School of Biomedical Sciences, Mayo Clinic, Jacksonville, FL 32224, USA. Electronic address:

The precise neurophysiological changes prompted by meningeal lymphatic dysfunction remain unclear. Here, we showed that inducing meningeal lymphatic vessel ablation in adult mice led to gene expression changes in glial cells, followed by reductions in mature oligodendrocyte numbers and specific lipid species in the brain. These phenomena were accompanied by altered meningeal adaptive immunity and brain myeloid cell activation.

Sleep Breath

October 2024

Sleep and Non-Invasive Ventilation Unit, Centro Hospitalar Universitário de São João, Faculty of Medicine, University of Porto, Porto, Portugal.

Article Synopsis
  • Obstructive sleep apnea (OSA) is common and requires accurate diagnosis; this study compares the effectiveness of in-person vs. video instructions for type III sleep studies.
  • One hundred patients were divided into two groups, receiving either face-to-face or video instructions, and the technical quality of their sleep studies was evaluated.
  • Results showed that while both instruction methods produced valid studies, the video group had better oximetry signal quality, making it a viable option for OSA diagnosis without increasing errors.
