Vortioxetine versus sertraline in metabolic control, distress and depression in Mexican patients with type 2 diabetes.

Background: Depression in patients with type 2 diabetes (T2D) is often undiagnosed and remains untreated, leading to poor therapy adherence and ill health-related outcomes. We evaluated the effect of vortioxetine versus sertraline in the treatment of depression, distress and metabolic control in subjects with T2D and depression.

Methods: Participants were selected from the Clinic for Diabetes, diagnosed with depression when the score was ≥14 in the Hamilton Depression Rating Scale, and verified by a psychiatrist in agreement with the DSM-5 instrument (Diagnostic and Statistical Manual of Mental Disorders, fifth edition).

[Injury of the anterior branch of the posterior interosseous nerve].

Introduction: Posterior interosseous nerve syndrome, a branch of the radial nerve at the level of the forearm, is characterized by the motor function loss of some or all of the muscles innervated distally.

Clinical Case: A 26-year-old male with a history of proximal radius fracture associated to radial nerve injury, treated with osteosynthesis 7 years earlier, with full recovery, who currently presented intense pain 4 cm distal to the radial head, accompanied by paralysis of Extensor pollicis longus, Extesnor pollicis brevis and Abductor pollicis longus, with paresis of the Extensor indicis propius, in which a diagnosis of entrapment syndrome of the anterior descending branch of the posterior interosseous nerve (SNIP) was performed.

Discussion: The conservative management of SNIP is indicated during the first 8-12 weeks, if no improvement is found, the indication for surgical exploration is indicated, and the removal of osteosynthesis material is controversial.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a potentially lethal disease, whose characteristic ventricular tachycardias are adrenergic-dependent. Although rare, CPVT should be considered in the differential diagnosis of young individuals with exercise-induced syncope. Mutations in five different genes (RYR2, CASQ2, CALM1, TRDN, and TECRL) are associated with the CPVT phenotype, although RYR2 missense mutations are implicated in up to 60 % of all CPVT cases.

Syphilis and HIV infections continue to threaten the safety of blood banks in countries where altruistic donations are rare. The aim of this study of blood donors to the Centro Estatal de la Transfusion Sanguínea de Veracruz (Mexico) was to determine changes in the prevalence of syphilis and HIV, and to identify factors associated with these infections. A total of 109,054 blood donors were retrospectively analyzed from 2007 to 2014.

  • The study evaluates the diagnostic accuracy of the ALPHA Histoplasma urine antigen (HUAg) test for detecting progressive disseminated histoplasmosis (PDH) in HIV patients, particularly in Mexico.
  • In a sample of 288 patients suspected of having PDH, the ALPHA-HUAg showed a sensitivity of 67.1% and a high specificity of 97.5%, indicating it is fairly good at confirming PDH but may miss some cases.
  • Despite its high specificity, the low sensitivity means that ALPHA-HUAg can't be solely relied on for diagnosis, and a culture-based approach is recommended to account for potential co-infections.

Background: With the availability of high-quality asthma guidelines worldwide, one possible approach of developing a valid guideline, without re-working the evidence, already analysed by major guidelines, is the ADAPTE approach, as was used for the development of National Guidelines on asthma.

Methods: The guidelines development group (GDG) covered a broad range of experts from medical specialities, primary care physicians and methodologists. The core group of the GDG searched the literature for asthma guidelines 2005 onward, and analysed the 11 best guidelines with AGREE-II to select three mother guidelines.

Objective: Introduction of pneumococcal conjugate vaccines (PCV) targeted against a limited number of serotypes substantially decreased invasive (IPD) and non-invasive pneumococcal diseases (NIPD) but it was accompanied by non-vaccine type replacement disease. After 9 years of introduction of PCV in Mexico, we analyze the evidence of the indirect effects on IPD and NIPD serotype distribution among groups not targeted to receive the vaccine.
