These guidelines are the result of a joint effort from writing groups of the Brazilian Stroke Society, the Scientific Department of Cerebrovascular Diseases of the Brazilian Academy of Neurology, the Brazilian Stroke Network and the Brazilian Society of Diagnostic and Therapeutic Neuroradiology. Members from these groups participated in web-based discussion forums with predefined themes, followed by videoconference meetings in which controversies and position statements were discussed, leading to a consensus. This guidelines focuses on the implications of the recent clinical trials on endovascular therapy for acute ischemic stroke due to proximal arterial occlusions, and the final text aims to guide health care providers, health care managers and public health authorities in managing patients with this condition in Brazil.

Background: Fampridine is a broad-spectrum voltage-dependent potassium channel blocker that enhances synaptic transmission. The drug has been shown to be able to ameliorate conduction in demyelinated axons, thereby leading to improved gait in patients with multiple sclerosis (MS).

Objective: To assess the "real-life" efficacy and safety of fampridine prescribed for gait disorders in MS.

Purpose: The aim of this study is to describe some radiological features in the newborns with microcephaly caused by Zika virus infection during pregnancy.

Methods: We radiologically analyzed 13 cases of newborns with microcephaly born to mothers who were infected by the Zika virus in the early stage of pregnancy.

Results: The most frequently observed radiological findings were microcephaly and decreased brain parenchymal volume associated with lissencephaly, ventriculomegaly secondary to the lack of brain tissue (not hypertensive), and coarse and anarchic calcifications mainly involving the subcortical cortical transition, and the basal ganglia.

March 2016

Departamento de Medicina Interna Neurology, Universidade Federal Fluminense, Rio de Janeiro, RJ, Brazil.

Zika virus (ZIKV) is now considered an emerging flavivirosis, with a first large outbreak registered in the Yap Islands in 2007. In 2013, a new outbreak was reported in the French Polynesia, with associated cases of neurological complications including Guillain-Barré syndrome (GBS). The incidence of GBS has increased in Brazil since 2015, what is speculated to be secondary to the ZIKV infection outbreak.

Natalizumab is a therapeutic option for treating multiple sclerosis (MS) and is particularly efficacious for patients with highly active disease. A long washout period has been recommended between withdrawal of natalizumab and start of fingolimod (another option for treating MS). This long washout period has been associated with a significant increase in MS activity.

Fisher Grading Scale Associated with Language Disorders in Patients with Anterior Circulation Aneurysmal Subarachnoid Hemorrhage.

World Neurosurg

August 2015

Department of Neurosurgery, Hospital da Restauração, Recife, Brazil; Department of Neuropsychiatry and Behavioral Sciences, Federal University of Pernambuco, Pernambuco, Brazil.

Objective: To associate the presence of language deficits with varying scores of the Fisher grading scale in patients with subarachnoid hemorrhage in the period preceding the treatment of aneurysm in the anterior circulation, as well as to compare the scores of this scale, identifying the grades more associated with the decline of language.

Methods: Database analysis of 185 preoperative evaluations of language, through the Montreal Toulouse Protocol Alpha version and verbal fluency through CERAD battery, of patients from "Hospital da Restauração" with aneurysmal subarachnoid hemorrhage, divided according to the Fisher grading scale (Fisher I, II, III, or IV) and compared with a control group of individuals considered normal.

Results: The various scores of the Fisher grading scale have different levels of language deficits, more pronounced as the amount of blood increases.

Article Synopsis
  • A retrospective study analyzed 125 patients with chronic subdural hematomas (CSDH) treated at a hospital in Brazil from 2006 to 2008, using the Glasgow Outcome Scale to assess outcomes after 6 months.
  • The results showed a median patient age of 69, with most cases involving a history of trauma; 83.2% had a good outcome while the mortality rate was 11.2%.
  • Key findings indicate that higher Glasgow Coma Scale scores upon admission are linked to better outcomes, while recurrence negatively affects the prognosis; the study highlights that advanced age should not deter surgical intervention.*
