Mapping peripheral and abdominal sarcopenia acquired in the acute phase of COVID-19 during 7 days of mechanical ventilation.

Sci Rep

March 2023

Department of Physical Therapy, Federal University of Pernambuco (UFPE), 173, Aníbal Fernandes Avenue, Cidade Universitária, Recife, Pernambuco, 50740-560, Brazil.

Article Synopsis
  • The study aimed to examine muscle loss in critically ill COVID-19 patients on mechanical ventilation by measuring muscle thickness and cross-sectional area using ultrasound on Days 1, 3, 5, and 7 post-admission.
  • Analysis of 5460 ultrasound images from 30 patients revealed significant muscle thickness decreases, particularly in the lower limbs and specific abdominal muscles, with the most notable losses occurring between Days 1 to 5.
  • Results indicate that peripheral and abdominal muscle loss is progressive in the first week of mechanical ventilation, with greater losses observed in the quadriceps and rectus femoris muscles.

Background: Since the publication of the 2018 Clinical Guidelines about sedation, analgesia, delirium, mobilization, and sleep deprivation in critically ill patients, no evaluation and adequacy assessment of these recommendations were studied in an international context. This survey aimed to investigate these current practices and if the COVID-19 pandemic has changed them.

Methods: This study was an open multinational electronic survey directed to physicians working in adult intensive care units (ICUs), which was performed in two steps: before and during the COVID-19 pandemic.

Background: Malaria elimination in Brazil poses several challenges, including the control of Plasmodium falciparum foci and the hidden burden of Plasmodium vivax in pregnancy. Maternal malaria and fetal health outcomes were investigated with a perinatal surveillance study in the municipality of Cruzeiro do Sul, Acre state, Brazilian Amazon. The research questions are: what are the causal effects of low birth weight on low Apgar at 5-min and of perinatal anaemia on stillbirth?

Methods: From November 2018 to October 2019, pregnant women of ≥ 22 weeks or puerperal mothers, who delivered at the referral maternity hospital (Juruá Women and Children's Hospital), were recruited to participate in a malaria surveillance study.

Background: Vaginal laxity is an underreported condition that negatively affects women's sexual function and their relationships. Evidence-based studies are needed to better understand this complaint and to discuss its treatment options. Thus, we present a study protocol to compare the effect of radiofrequency and pelvic floor muscle training in the treatment of women with complaints of vaginal laxity.

Article Synopsis
  • The study investigates the experiences of family members and health care providers regarding the use of ICU diaries, which are intended to prevent psychiatric symptoms in critical illness survivors and provide other benefits.
  • A thorough literature search identified 28 studies revealing that families find the diaries informative and a means to express their emotions, while healthcare providers see value but also face challenges in writing them.
  • The findings suggest that while ICU diaries are seen as a beneficial intervention, attention is needed to address the difficulties encountered in the writing process for both families and healthcare staff.

Different classes of antibiotics given to women routinely for preventing infection at caesarean section.

Cochrane Database Syst Rev

March 2021

Cochrane Pregnancy and Childbirth Group, Department of Women's and Children's Health, University of Liverpool, Liverpool, UK.

Background: Caesarean section increases the risk of postpartum infection for women and prophylactic antibiotics have been shown to reduce the incidence; however, there are adverse effects. It is important to identify the most effective class of antibiotics to use and those with the least adverse effects.  OBJECTIVES: To determine, from the best available evidence, the balance of benefits and harms between different classes of antibiotic given prophylactically to women undergoing caesarean section, considering their effectiveness in reducing infectious complications for women and adverse effects on both mother and infant.

Sigh in Patients With Acute Hypoxemic Respiratory Failure and ARDS: The PROTECTION Pilot Randomized Clinical Trial.

Chest

April 2021

Department of Pathophysiology and Transplantation, University of Milan, Milan, Italy; Department of Anesthesia, Critical Care and Emergency, Foundation IRCCS Cà Granda Maggiore Policlinico Hospital, Milan, Italy.

Background: Sigh is a cyclic brief recruitment maneuver: previous physiologic studies showed that its use could be an interesting addition to pressure support ventilation to improve lung elastance, decrease regional heterogeneity, and increase release of surfactant.

Research Question: Is the clinical application of sigh during pressure support ventilation (PSV) feasible?

Study Design And Methods: We conducted a multicenter noninferiority randomized clinical trial on adult intubated patients with acute hypoxemic respiratory failure or ARDS undergoing PSV. Patients were randomized to the no-sigh group and treated by PSV alone, or to the sigh group, treated by PSV plus sigh (increase in airway pressure to 30 cm HO for 3 s once per minute) until day 28 or death or successful spontaneous breathing trial.
