Paratesticular adenomatoid tumors are benign and rare neoplasms, and the management of these lesions is challenging as many cases end up in the operation room due to the lack of specific clinical and radiological features to differentiate them from malignant lesions. We report a case of adenomatoid tumor of the tunica albuginea in a 48-year-old male along with a review of the literature for similar cases in the last 10 years.

Here, we present a unique case involving a 23-year-old female patient with a large urethrovaginal fistula and complete anterior vaginal wall sloughing following prolonged obstructed labor. The objective of this report is to highlight its unusual findings on MRI including internal capsule, brainstem, cerebellum involvement; subependymal nodular heterotopia, subependymal cysts, cortical laminar necrosis along with typical findings of megalencephalic leukoencephalopathy and subcortical cysts. The study also underscores the clinical implications of this complex pathology, with emphasis on comprehensive neuroradiological evaluation for atypical presentations to guide better diagnostic and management outcomes.
