ACR Appropriateness Criteria Ataxia.

J Am Coll Radiol

May 2019

Specialty Chair, Emory University, Atlanta, Georgia.

Ataxia can result from an abnormality in the cerebellum, spinal cord, peripheral nerves, and/or vestibular system. Pathology involving the brain, such as infarct or hydrocephalus, can also present with ataxia as part of the symptom constitution, or result in symptoms that mimic ataxia. Clinical evaluation by history and careful neurological examination is important to help with lesion localization, and helps determine where imaging should be focused.

Patterns of Management of Patients With Dual Disorder (Psychosis) in Italy: A Survey of Psychiatrists and Other Physicians Focusing on Clinical Practice.

Front Psychiatry

November 2018

Psychopharmacology, Drug Misuse and Novel Psychoactive Substances Research Unit, School of Life and Medical Sciences, University of Hertfordshire, Hatfield, United Kingdom.

Patients with severe psychotic disorders such as schizophrenia, schizoaffective, and bipolar disorders frequently suffer from concomitant substance use disorders (SUDs)-Dual Disorder (DD) patients. In order to better understand current practices for management of patients with psychotic episodes and concomitant SUD in Italy, we carried out a survey of psychiatrists on current routine practice among prescribers. These aspects can help to identify at-risk patients, improve current prescribing practices, and favor early intervention.

Objective: The purpose of this study is to determine whether the type of feedback on evidence-based guideline adherence influences adult primary care provider (PCP) lumbar spine (LS) MRI orders for low back pain (LBP).

Materials And Methods: Four types of guideline adherence feedback were tested on eight tertiary health care system outpatient PCP practices: no feedback during baseline (March 1, 2012-October 4, 2012), randomization by practice to either clinical decision support (CDS)-generated report cards comparing providers to peers only or real-time CDS alerts at order entry during intervention 1 (February 6, 2013-December 31, 2013), and both feedback types for all practices during intervention 2 (January 14, 2014-June 20, 2014, and September 4, 2014-January 21, 2015). International Classification of Disease codes identified LBP visits (excluding Medicare fee-for-service).

Asbestosis is diagnosed with a combination of historical, clinical and radiological findings in the absence of another cause. Histology is required when uncertainty exists, with lung biopsy via VATs being gold standard. Transbronchial cryobiopsy is becoming increasingly popular for diagnosing interstitial lung disease and may provide sufficient lung sample to demonstrate asbestosis.

Article Synopsis
  • The study aimed to evaluate health-related quality of life (HRQOL) in children and young adults with Marfan syndrome participating in a trial, using the Pediatric Quality of Life Inventory (PedsQL).
  • Results showed that younger patients (5-18 years) had significantly lower HRQOL scores in both physical and psychosocial areas compared to healthy peers, while older patients (19-25 years) scored higher, indicating a shift in HRQOL with age.
  • Factors like age, sex, and neurodevelopmental disorders were found to significantly impact HRQOL, whereas treatment type (atenolol vs. losartan) and physical severity of Marfan syndrome did not exhibit significant differences.

Airway stents are commonly used in the management of patients suffering from central airway obstruction (CAO). CAO may occur directly from airway strictures, obstructing airway cancers, airway fistulas or tracheobronchomalacia, resulting from the weakening and dynamic collapse of the airway wall. Current airway stents are constructed from biocompatible medical-grade silicone or from a nickel-titanium (nitinol) alloy with fixed geometry.

Article Synopsis
  • Diagnostic apraxia is a condition linked to corpus callosal disconnection, and while lesions in the corpus callosum have been noted in Neuromyelitis optica spectrum disorder (NMOSD), such cases are rare.
  • A 48-year-old woman presented with symptoms including balance issues, slurred speech, and uncoordinated movements of her left hand while using her right, alongside other signs of callosal disconnection like left ear extinction and crossed optic ataxia.
  • MRI scans revealed a "marbled pattern" in the corpus callosum, and after being diagnosed with NMOSD and treated with steroid pulse therapy, her symptoms improved.

ACR Appropriateness Criteria Orbits Vision and Visual Loss.

J Am Coll Radiol

May 2018

Specialty Chair, UC San Diego Health, San Diego, California.

Visual loss can be the result of an abnormality anywhere along the visual pathway including the globe, optic nerve, optic chiasm, optic tract, thalamus, optic radiations or primary visual cortex. Appropriate imaging analysis of visual loss is facilitated by a compartmental approach that establishes a differential diagnosis on the basis of suspected lesion location and specific clinical features. CT and MRI are the primary imaging modalities used to evaluate patients with visual loss and are often complementary in evaluating these patients.

Remarks on Perfection in Plastic Surgery of the Face.

Biomed Res Int

September 2018

Department of Maxillofacial Orthopedics and Orthodontics, Wrocław Medical University, Wrocław, Poland.

An expectation of perfect and close to ideal outcomes is attributed not only to aesthetic, but also to reconstructive surgery. Contrary to quite common belief and despite great potential, the real chances for achieving attractive appearance are limited to relatively simple cases with moderately abnormal morphology, sufficient homogenous material, and hardly conspicuous scars potential. Therefore, the expectations for fully satisfactory outcomes should be limited to jaw surgery, cosmetic rhinoplasty, otoplasty, and some rejuvenation procedures, provided the best and uneventful surgery is secured.

Significant progress has been made in the percutaneous coronary intervention technique from the days of balloon angioplasty to modern-day metallic drug-eluting stents (DES). Although metallic stents solve a temporary problem of acute recoil following balloon angioplasty, they leave behind a permanent problem implicated in very late events (in addition to neoatherosclerosis). BRS were developed as a potential solution to this permanent problem, but the promise of these devices has been tempered by clinical trials showing increased risk of safety outcomes, both early and late.

ACR Appropriateness Criteria Penetrating Neck Injury.

J Am Coll Radiol

November 2017

Panel Chair (Vascular), UMass Memorial Medical Center, Worcester, Massachusetts.

In patients with penetrating neck injuries with clinical soft injury signs, and patients with hard signs of injury who do not require immediate surgery, CT angiography of the neck is the preferred imaging procedure to evaluate extent of injury. Other modalities, such as radiography and fluoroscopy, catheter-based angiography, ultrasound, and MR angiography have their place in the evaluation of the patient, depending on the specific clinical situation and question at hand. The ACR Appropriateness Criteria are evidence-based guidelines for specific clinical conditions that are reviewed annually by a multidisciplinary expert panel.

ACR Appropriateness Criteria Cranial Neuropathy.

J Am Coll Radiol

November 2017

Specialty Chair, University of Cincinnati Medical Center, Cincinnati, Ohio.

Evaluation of cranial neuropathy can be complex given the different pathway of each cranial nerve as well as the associated anatomic landmarks. Radiological evaluation requires imaging of the entire course of the nerve from its nucleus to the end organ. MRI is the modality of choice with CT playing a complementary role, particularly in the evaluation of the bone anatomy.
