Background: Fractional flow reserve (FFR) or non-hyperaemic pressure ratios are recommended to assess functional relevance of intermediate coronary stenosis. Both diagnostic methods require the placement of a pressure wire in the coronary artery during invasive coronary angiography. Quantitative flow ratio (QFR) is an angiography-based computational method for the estimation of FFR that does not require the use of pressure wires.

Can J Cardiol

October 2024

Department of Cardiology, Hospital Universitario Puerta de Hierro Majadahonda, IDIPHISA, Madrid, Spain; CIBERCV, Instituto de Salud Carlos III, Madrid, Spain; Centro Nacional de Investigaciones Cardiovasculares (CNIC), Madrid, Spain; Universidad Francisco de Vitoria (UFV), Pozuelo de Alarcón, Spain. Electronic address:

Article Synopsis
  • Transthyretin cardiac amyloidosis (ATTR-CA) is a treatable form of heart failure, often associated with atrial fibrillation (AF), and the study explores if recent-onset AF can indicate ATTR-CA in patients over 65.
  • In a study of 121 patients recently diagnosed with nonvalvular AF, 10 were found to have cardiac amyloidosis, with notable differences in age, sex, and heart metrics observed between those with and without the condition.
  • The findings suggest that routine screening for ATTR-CA using echocardiography and other tests may benefit older patients with AF and specific heart features, although more extensive research is needed to confirm these results in varied clinical situations.

Article Synopsis
  • - The study analyzed 202 patients with head and neck paragangliomas (HN-PGL) across 16 Spanish hospitals, noting that 67.8% of participants were women, with a mean age of 53.1 years and 264 tumors examined, primarily located in the carotid body.
  • - Among patients, 20.8% had multiple tumors and 4% had metastatic disease; genetic testing revealed a pathogenic variant in SDHx in half of those tested, which was linked to younger patients and more complex tumor profiles.
  • - The results showed that while surgery was the most common treatment, radiotherapy and observation (monitoring without active treatment) were also effective, and long-term survival rates remained high

The working group set up by the SEAP-IAP addresses in this Part II some general considerations and five particular considerations to be taken into account when a biological sample of human origin, coming from our archives, acquires a different destination from the usual one, in this case for research. From this moment on, we must follow mandatory legal and ethical rules, and the different recitals provide us with guidelines to ensure good practice, both for biological material and its associated data. The traditional task of custody given to the Pathological Anatomy is approached, as always, from the point of view of responsibility and, in this article, adjusted to its time.

Article Synopsis
  • Proper storage of pathology specimens is crucial for patient safety, but there are no established guidelines detailing the minimum storage duration for samples.
  • A working group from the Spanish Society of Anatomic Pathology reviewed literature and drew up recommendations after discussions and voting.
  • The established minimum storage times include at least 10 years for paraffin embedded blocks and histological preparations, 3 years for cervico-vaginal cytology, and 5 years for extracted nucleic acids, among other classifications.

Article Synopsis
  • The main goals of treating heart failure with reduced ejection fraction (HFrEF) are to lower mortality rates and reduce hospitalizations, while also improving the overall quality of life and cardiac function for patients.
  • Effective treatments such as RAS inhibitors (ACE inhibitors, ARBs, ARNIs), beta-blockers, and SGLT2 inhibitors have shown benefits in reducing mortality, hospitalization, and the risk of sudden death, as well as aiding in cardiac remodeling and renal protection.
  • The ideal HFrEF treatment should be effective across various patient profiles, taking into account individual differences and comorbidities, while maximizing overall health benefits beyond just heart function.

Spontaneous recanalization of a coronary thrombus is an uncommon finding that may be confounded without intravascular imaging. There is little evidence regarding the functional impact of these lesions, which may be relevant to proper management. We present two cases of recanalized thrombus in which functional testing guided appropriate management.

Measurable residual disease by mass spectrometry and next-generation flow to assess treatment response in myeloma.

Blood

December 2024

Hematology Department, IBSAL, Instituto de Biología Molecular y Celular del Cáncer-Consejo Superior de Investigaciones Científicas, Centro de Investigación Biomédica en Red de Cáncer, Hospital Universitario de Salamanca, Salamanca, Spain.

Quantitative immunoprecipitation mass spectrometry (QIP-MS) allows the identification of the M-protein in patients with multiple myeloma (MM) otherwise in complete response, and could be considered suitable for measurable residual disease (MRD) evaluation in peripheral blood. In the context of the GEM2012MENOS65 and GEM2014MAIN trials, we compared the performance of QIP-MS in serum with next-generation flow (NGF) cytometry in bone marrow to assess MRD in paired samples obtained postinduction, transplant, consolidation and after 24 cycles of maintenance. At each time point, both NGF and QIP-MS were able to segregate 2 groups of patients with significantly different progression-free survival; when the evolution of the results obtained with either method was considered, maintaining or converting to MRD negativity was associated with longer survival, significantly better when compared with sustaining or converting to MRD positivity.

  • The MOSCA-FRAIL trial examined the effects of invasive versus conservative treatment strategies in frail patients aged 70 and older with non-ST-segment elevation myocardial infarction (NSTEMI).
  • The study included 167 participants and focused on various geriatric conditions, finding that invasive management had a notable survival benefit primarily for those with lower frailty levels (CFS=4).
  • Results suggested that frailty, as indicated by the Clinical Frailty Scale (CFS), is an important factor in determining the risks and benefits of treatment, with a CFS score greater than 4 potentially signaling a need for more cautious decision-making.
