Background: Randomized controlled trials (RCTs) have failed to demonstrate the beneficial effects of the pharmacological treatment of patent ductus arteriosus (PDA) in preterm infants. We conducted a Bayesian model averaged (BMA) meta-analysis of RCTs comparing the pharmacological treatment of PDA with placebo or expectant treatment.

Methods: We searched for RCTs including infants with gestational age (GA) ≤ 32 weeks and with a rate of open-label treatment of less than 25% in the control arm.

Article Synopsis
  • Lung ultrasound could be a valuable tool in predicting the severity of lung disease and the need for extended ventilatory support in critically ill children.
  • In a study involving 538 children in the ICU, those requiring prolonged mechanical ventilation (over 7 days) had significantly higher lung ultrasound scores at 12 and 48-72 hours post-admission.
  • The lung ultrasound score demonstrated a strong correlation with oxygen levels and was effective in predicting prolonged mechanical ventilation, showing high accuracy in both initial assessment and multivariable analysis.

Patent Ductus Arteriosus and Bronchopulmonary Dysplasia-Associated Pulmonary Hypertension: A Bayesian Meta-Analysis.

JAMA Netw Open

November 2023

Division of Neonatology, Department of Pediatrics, MosaKids Children's Hospital, Maastricht University Medical Center (MUMC+), School for Oncology and Reproduction (GROW), Maastricht University, Maastricht, The Netherlands.

Importance: Bronchopulmonary dysplasia (BPD) is often associated with pulmonary vascular disease and secondary pulmonary hypertension (PH). The pathogenesis of BPD-associated PH (BPD-PH) is complex and involves prenatal and postnatal factors that disrupt pulmonary vascular development, and patent ductus arteriosus (PDA) is a factor potentially associated with risk of BPD-PH that has been identified in very recent studies.

Objective: To explore the association of PDA with BPD-PH using a bayesian model-averaged (BMA) meta-analysis of studies.

Cervical pessary and cerclage placement for preterm birth prevention and cervicovaginal microbiome changes.

Acta Obstet Gynecol Scand

December 2022

Maternal and Child Health and Development Network (SAMID), Instituto de Salud Carlos III, Madrid, Spain.

Article Synopsis
  • The study aimed to compare the vaginal microbiome of low-risk and high-risk pregnant women to see if it relates to preterm birth.
  • Pregnant women were divided into three groups: control (normal cervix), pessary, and cerclage, and their vaginal microbiomes were analyzed at two different stages during their pregnancy.
  • Results showed that women with a short cervix had a different microbiome composition and a significant increase in microbial diversity after cerclage intervention, while such changes were not observed in the pessary or control groups, along with an increased risk of preterm labor in the cerclage group.

The NDDG criteria versus the IADPSG or the ADA criteria for diagnosing early-onset gestational diabetes mellitus or abnormal glucose tolerance.

Int J Gynaecol Obstet

March 2023

Spanish Diabetes and Pregnancy Study Group. Sociedad Española de Ginecología y Obstetricia (SEGO) and Sociedad Española de Diabetes (SED, Madrid, Spain.

Objective: To analyze the effects of substituting the National Diabetes Data Group (NDDG) criteria with the International Association of Diabetes and Pregnancy Study Groups (IADPSG) or American Diabetes Association (ADA) criteria for the diagnosis of early-onset gestational diabetes mellitus (Early-GDM) or first trimester abnormal glucose tolerance (1 t-AGT).

Methods: A retrospective cohort study was conducted of 3200 women: 400 with Early-GDM, 800 with GDM, and 2000 with Non-GDM, according to the NDDG criteria. Rates of women with missed and new Early-GDM according to the IADPSG or ADA criteria were calculated.

Epidemiological evidence and animal studies support that intrauterine exposure to tobacco smoke disturbs lung development and has a negative effect in the pulmonary health of the offspring. Individual studies suggest an association between fetal exposure to maternal smoking and risk of developing bronchopulmonary dysplasia (BPD). However, this association has not yet been systematically investigated.
