Article Synopsis
  • Coronary functional disorders (CFD) significantly contribute to angina in patients with non-obstructed coronary arteries (ANOCA), with different endotypes identified, such as microvascular spasm and coronary microvascular dysfunction (CMD).
  • A study involving 89 ANOCA patients using comprehensive coronary functional testing (CFT) found that 91% had at least one disorder, with microvascular spasm being the most common (61%).
  • The results highlighted that isolated CMD is rare and emphasized the necessity of spasm testing for diagnosing and characterizing CFD in ANOCA patients.

Introduction: Men are generally more susceptible to bacterial infections than women. Central venous catheters (CVCs), often used to administer systemic treatment in patients with cancer, are an important source of infection. However, little is known about sex-specific differences of CVC-related bloodstream infections (CRBSIs) in patients with cancer.

In 2022, the European LeukemiaNet (ELN) risk stratification for patients with AML has been updated. We aimed to validate the prognostic value of the 2022 ELN classification (ELN22) evaluating 1,570 newly diagnosed AML patients (median age, 56 years) treated with cytarabine-based intensive chemotherapy regimens. As compared to the 2017 ELN classification (ELN17), allocating 595 (38%), 413 (26%) and 562 (36%) patients to the favorable, intermediate and adverse risk category, ELN22 risk was favorable, intermediate, and adverse in 575 (37%), 410 (26%), and 585 (37%) patients, respectively.

Acute myeloid leukemia (AML) is a malignant disease of the blood and bone marrow that is characterized by uncontrolled clonal proliferation of abnormal myeloid progenitor cells. Nucleophosmin 1 (NPM1) gene mutations are the most common genetic abnormality in AML, detectable in blast cells from about one-third of adults with AML. AML NPM1 is recognized as a separate entity in the World Health Organization classification of AML.

Introduction: The impact of deceased donor characteristics on kidney transplant outcomes is controversial. Correspondingly, the predictive performance of deceased donor scores remains moderate, and many transplant centers lack validated criteria for graft acceptance decisions. To better dissect donor-related risk from recipient and periprocedural variables, we analyzed outcomes of kidney donor pairs transplanted in different individuals.

Background: MR perfusion is a standard marker to distinguish progression and therapy-associated changes after surgery and radiochemotherapy for glioblastoma. TRAMs (Treatment Response Assessment Maps) were introduced, which are intended to facilitate the differentiation of vital tumor cells and radiation necrosis by means of late (20-90 min) contrast clearance and enhancement. The differences of MR perfusion and late-enhancement are not fully understood yet.

Differentiation between multifocal CNS lymphoma and glioblastoma based on MRI criteria.

Discov Oncol

September 2024

Department of Neurosurgery, University Medical Center, Georg-August-University, Robert-Koch-Straße 40, 37075, Göttingen, Germany.

Purpose: Differentiating between glioblastoma (GB) with multiple foci (mGB) and multifocal central nervous system lymphoma (mCNSL) can be challenging because these cancers share several features at first appearance on magnetic resonance imaging (MRI). The aim of this study was to explore morphological differences in MRI findings for mGB versus mCNSL and to develop an interpretation algorithm with high diagnostic accuracy.

Methods: In this retrospective study, MRI characteristics were compared between 50 patients with mGB and 50 patients with mCNSL treated between 2015 and 2020.

The impact of MICB mismatches in unrelated haematopoietic stem cell transplantation.

HLA

June 2024

Institute of Clinical Transfusion Medicine and Immunogenetics Ulm, German Red Cross Blood Transfusion Service, Baden Wuerttemberg - Hessen, Ulm, and University Hospital Ulm, Ulm, Germany.

Article Synopsis
  • MICA polymorphisms are linked to higher rates of acute GvHD and poor outcomes in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT), with MICB's role still being unclear.
  • Researchers analyzed a large cohort for MICB polymorphisms, assessing how MICB matching affects outcomes after unrelated HSCT, finding that 69.2% were 10/10 HLA matched.
  • In the 9/10 HLA matched group, MICB mismatches led to worse disease-free survival and relapse-free survival, while MICA mismatches did not significantly impact outcomes, suggesting MICB typing might aid in donor selection among 9/10 matched donors.

Aims: Intrahepatic cholangiocarcinoma (iCCA) is a diagnosis of exclusion that can pose a challenge to the pathologist despite thorough clinical workup. Although several immunohistochemical markers have been proposed for iCCA, none of them reached clinical practice. We here assessed the combined usage of two promising diagnostic approaches, albumin in situ hybridisation (Alb-ISH) and C reactive protein (CRP) immunohistochemistry, for distinguishing iCCA from other adenocarcinoma primaries.

The therapy of relapsed or refractory (r/r) mantle cell lymphoma (MCL) patients remains a major clinical challenge to date. We conducted a randomized, open-label, parallel-group phase-III trial hypothesizing superior efficacy of rituximab, high-dose cytarabine and dexamethasone with bortezomib (R-HAD + B) versus without (R-HAD) in r/r MCL ineligible for or relapsed after autologous stem cell transplant (ASCT). Primary endpoint was time to treatment failure (TTF), secondary endpoints included response rates, progression free survival, overall survival, and safety.

Article Synopsis
  • In newly diagnosed acute myeloid leukemia (AML), immediate treatment is typical; however, sometimes treatment may be delayed to evaluate other health issues or risks.
  • A study analyzed the effects of the timing of treatment initiation on outcomes for patients receiving venetoclax-based therapy, comparing two patient groups with average ages of 76 and 72.
  • Results showed no significant difference in overall survival or complications (like severe infections) between those who started treatment within 10 days versus later, suggesting that delaying therapy may be safe for certain patients with proper monitoring.

Objective: Fetal magnetic resonance imaging (MRI) is broadly used as a method for assessing prognosis in congenital diaphragmatic hernia (CDH). In addition to the extent of lung hypoplasia, determined by measuring the lung volume, cardiac impairment due to pulmonary hypertension and left cardiac hypoplasia is decisive for the prognosis. The percentage area of left ventricle (pALV) describes the percentage of the inner area of the left ventricle in relation to the total area, whereas the mediastinal shift angle (MSA) quantifies the extent of cardiac displacement.
