Background: Despite scarce data, invasive mechanical ventilation (MV) is widely recommended over non-invasive ventilation (NIV) for ventilatory support in cardiogenic shock (CS). We assessed the real-life use of different ventilation strategies in CS and their influence on outcome focusing on the use of NIV and MV.

Methods: 219 CS patients were categorized by the maximum intensity of ventilatory support they needed during the first 24h into MV (n=137; 63%) , NIV (n=26; 12%), and supplementary oxygen (n=56; 26%) groups.

Chronic rejection is the most important cause of renal graft dysfunction. Non-immunological mechanisms have been suggested as a probable origin of chronic graft rejection, provoking a decrease in renal mass function, followed by glomerular hyperfiltration in the remnant nephrons, which could cause progressive glomerulosclerosis and functional loss. Early, or preclinical, identification of patients with glomerular hyperfiltration, defined as an increase in glomerular filtration fraction (GFF) and in glomerular capillary pressure (GCP), could prolong graft life.

We present the case of a 41-year-old man admitted to the hospital with fever and toxic syndrome possibly having an oncological or infectious origin. A whole body scan with 67Ga-citrate showed an intense and diffuse radiotracer accumulation in the thyroid gland. The patient was re-evaluated and studied with other diagnostic tests including a thyroid 99mTc-pertechnetate scintigraphy and a thyroidal radioiodine uptake.

Background: Sinus bradycardia in trained athletes is predominantly a manifestation of increased vagal tone, but it is not known whether an alteration in the cardiac sympathetic system can contribute to blunted chronotropic response. This study assessed the integrity of the sympathetic system in trained athletes with sinus bradycardia by means of the iodine-123-metaiodobenzylguanidine (123I-MIBG) procedure.

Methods And Results: Fourteen athletes with sinus bradycardia and 8 athletes with a normal heart rate were explored by means of planar and single photon emission computed tomography MIBG studies.

Thirty-one cases of sporotrichosis diagnosed in the central region of Rio Grande do Sul from 1988 to 1997 were studied. Clinical data were compared with a study concerning three past decades, clearly showing a decrease in the incidence of the mycosis, and an alteration in the profile of the infection, with a decrease of sporotrichosis in rural patients, children, women and farmers. In the past decade the mycosis was most frequent among urban adults with different professions, with the onset of the disease being associated with rural leisure activities such as fishing and hunting.

Exercise rehabilitation improves the clinical status in ischaemic heart disease. The purpose of this study was to assess the influence of exercise rehabilitation on myocardial perfusion and sympathetic heart innervation. Detection of recurrence of medullary thyroid carcinoma (MTC) remains a diagnostic problem. Increased serum tumour marker levels frequently indicate recurrence while conventional imaging techniques (CIT) are non-diagnostic. A comparison was performed between technetium-99m tetrofosmin myocardial perfusion tomography at baseline and after nitrate administration, using a 2-day protocol, and rest-reinjection thallium-201 single-photon emission tomography (SPET) studies in order to assess whether nitrates enhance the detection of viable myocardium with 99mTc-tetrofosmin. Fifteen patients with coronary artery disease, previous myocardial infarction and a left ventricular ejection fraction <40% underwent 201Tl rest-injection and 99mTc-tetrofosmin baseline-postnitroglycerin (0.4 mg sublingually) SPET studies, within 48 h.

The General Health Questionnaire (GHQ) and the Clinical Interview Schedule (CIS) were translated into Portuguese and were used in a pilot study of psychiatric morbidity in primary care at 'The Health Centre Barra Funda/Bom Retiro' of the Faculty of Medicine of Santa Casa Hospital at Sao Paulo, Brazil. The Portuguese translation of the CIS was found to be a feasible instrument for community studies in Brazil, but the Portuguese translation of the GHQ-30 was shown to be of limited use in this particular setting. The extent of minor psychiatric morbidity was very high (46%); 33% was regarded as conspicuous (CPM) and 13% as hidden (HPM).
