Meta-analysis of deferral versus performance of coronary intervention based on coronary pressure-derived fractional flow reserve.

Am J Cardiol

February 2015

Division of Cardiology and Cardiovascular Surgery, Hospital das Clínicas, Universidade Federal de Minas Gerais, Belo Horizonte, Minas Gerais, Brazil; Faculdade de Medicina da Universidade Federal de Minas Gerais, Belo Horizonte, Minas Gerais, Brazil.

Fractional flow reserve (FFR) has been proposed as the gold standard to assess functional severity of coronary artery stenosis and to stratify which lesions should be subjected to intervention (percutaneous coronary intervention [PCI]). A systematic review was performed in MEDLINE and EMBASE including studies indexed until November 2013 that used FFR for deferral or performance of PCI. Outcomes of interest were death, acute myocardial infarction (AMI), and new revascularization (RV).

Safety of I.V. Nonnitrogen bisphosphonates on the occurrence of osteonecrosis of the jaw: long-term follow-up on prostate cancer patients.

Clin Genitourin Cancer

June 2015

Urology Clinic, Hospital Beneficência Portguesa of São Paulo, São Paulo, Brazil; Urology Department, Hospital Santa Helena of São Paulo, São Paulo, Brazil.

Introduction: The aim of the study was to disclose information about the recently incorporated bisphosphonates therapies used to treat prostate cancer patients and their potential risks because the chemical nature and nitrogen content varies among the available drugs on the market.

Patients And Methods: Three hundred twenty-four consecutive prostate cancer patients were submitted to bisphosphonates therapy after antiandrogen treatment was started. Zoledronic acid was administered monthly (n = 45), bimonthly (n = 15), trimonthly (n = 19), and semestrally (n = 15), and monthly intravenous clodronate was administered in an additional 156 cases.

Impact of Urodynamic Learning on the Management of Benign Prostatic Hyperplasia Issue on Young and Senior Urologists and Reasons to Attend a Voiding Dysfunction Course.

Low Urin Tract Symptoms

January 2014

Division of Neurourology and Voiding Disturbances, Beneficência Portuguesa Hospital of São Paulo and Department of Urology, Hospital Santa Helena of São Paulo, São Paulo, Brazil.

Objectives: The aims of this study were to compare the impact of urodynamic training on the young urologists after fellowship training as well as on senior urologists who attend regular courses on the management of benign prostatic hyperplasia (BPH) and their capacity to do and interpret urodynamic studies.

Methods: Sixty-four consecutive young urologists admitted to fellowship program on voiding dysfunctions and 110 senior urologists attending to periodical meetings were interviewed before and after the 3-day-courses regarding their ability to set, interpret and do urodynamic studies. They were also questioned on the reasons that led them to attend the courses and how they use the new concepts to manage BPH.

Clinical results of transferring a motor branch of the tibial nerve to the deep peroneal nerve for treatment of foot drop.

Neurosurgery

October 2013

†Unit of Neurosurgery, Hospital de Base do Distrito Federal, Brasília, Distrito Federal, Brazil; ‡Hospital Santa Helena, Brasília, Distrito Federal, Brazil; §Peripheral Nerve Unit, Department of Neurosurgery, University of São Paulo Medical School, São Paulo, Brazil.

Background: Foot drop is a very debilitating condition affecting patients' daily activities, and its treatment has been a challenge for neurosurgeons. Grafting the peroneal or sciatic nerve usually results in poor outcomes. Our previous anatomic study demonstrated the feasibility of transferring a motor branch of the tibial nerve to the deep peroneal nerve at the level of the popliteal fossa.

Endoscopic treatment of biliary tract complications after Roux-en-Y surgery is still a challenge. With balloon enteroscopy, we can reach previously inaccessible areas changing the management of biliopancreatic diseases in patients with surgically altered anatomy. We report a case of single-balloon enteroscopy plus endoscopic retrograde cholangiopancreatography for the treatment of a pinpoint stricture in a hepaticojejunal anastomosis after liver transplantation.

Background: Bisphosphonates were incorporated as agents for the long-term effect of androgen deprivation therapy (ADT) but no comparative study was established for the optimal schedule for bone preservation.

Methods: Ninety-five consecutive prostate cancer patients submitted to radical retropubic prostatectomy were recruited. At rising PSA they were prospectively enrolled in nonrandomized fashion and grouped to receive no treatment (21 - control group), and monthly (17), bi-monthly (15), tri-monthly (19) or semestral (15) 4 mg zoledronic acid infusions.

Introduction: The prevalence of bladder outlet obstruction in men has been overestimated leading to improper clinical results after transurethral resection of the prostate.

Patients And Methods: 3,830 consecutive male cases submitted for urodynamic evaluation were prospectively analyzed using a Schaefer nomogram. The prevalence of detrusor overactivity and the occurrence of obstruction were prospectively studied using standardized urodynamic practice.

Colorectal anastomotic benign strictures represent a challenging complication. Endoscopic dilation represents a valid and safe treatment. The purpose of this study is to retrospectively investigate the results of endoscopic balloon dilation for anastomotic stricture after anterior rectal resection for cancer in an institution.

Purpose: Mechanical linear staplers have been safely used in urology with an acceptable 0% to 7.9% rate of stone formation in long-term follow-up. We sought to evaluate the feasibility of using mechanical circular stapler devices to perform ileocapsuloplasty following cystoadenomectomy in cadavers.

Background/aims: We compared stapled hemorrhoidectomy to closed diathermy-excision hemorrhoidectomy without suture-ligation regarding postoperative pain, complications and long-term efficacy.

Methodology: A series of 108 patients had indication for stapled hemorrhoidectomy. Patients who underwent stapled hemorrhoidectomy (76) were compared to patients submitted to closed diathermy-excision hemorrhoidectomy (32) due to non-insurance cover.

Introduction: Valsalva leak point pressure (VLPP) represents the global competence of the conjunctive forces around the urethra to support increased pressure from the abdominal cavity with transmission to the bladder. Assessment of VLPP has prognostic meaning, but measurement techniques are still subject to controversy.

Patients And Methods: One hundred and eight consecutive women with no genital prolapse or bladder hyperactivity exclusively presenting with urinary stress incontinence were submitted to VLPP determination during urodynamic evaluation using a rectal and urethral catheter.

Introduction: Indigo carmine dye is usually spread directly over the colon in many chromoscopic techniques aiming better visualization of a lesion already detected by conventional colonoscopy. Examination of the colon already stained by oral administration of indigo carmine dye may increase detection of small lesions resulting in higher sensibility of the colonoscopy in diagnosing diminutive lesions.

Objective: Analyze the results regarding the quality of chromoscopic technique and the indigo carmine dye distribution over the colon after oral administration.
